Although both are diseases occurring in summer, presenting with headache, fever, greasy face, and spontaneous sweating, there is a distinction between *Shu* (Summerheat) disease and *Re* (Heat) disease. *Shu* pathogen belongs to Yin evil, while *Re* pathogen belongs to Yang evil; this can be understood by observing the natural world. When scorching winds blow, flora flourishes yet approaches withering; when intense sun irradiates and steams, ditches fill with water but quickly dry up. When Yang Qi dissipates outwardly, the interior must inevitably be empty. Wells with deep sources have water as cold as ice; winds from rocky valleys are chilling and piercing like needles. Humans are microcosms of heaven and earth. Those who stay indoors and sit quietly are akin to seeking warmth (approaching heat); those who rush along roads often seek shade. When Yang Qi dissipates outwardly, the interior also becomes empty, leading to mental difficulty in movement, limb fatigue, chronic shortness of breath, and declining vitality. Therefore, these illnesses arise from the body's Qi deficiency allowing external pathogens to invade.

In *Zhong Shu* (Heatstroke), one contracts the damp Qi from subterranean or shaded places. Yin meets Yin. Symptoms include headache, fever, greasy face, and spontaneous sweating, which appear no different from *Zhong Re* (Sunstroke). However, the urine is clear and profuse, stools are loose, vomiting occurs, Qi is deficient, the patient is quiet and prone to sleep, and the pulse is weak and timid (sometimes weak and rapid). Compared to *Zhong Re*, they are vastly different. Summerheat invariably damages Qi; without Huangqi (Astragali Radix), Qi cannot be sufficiently tonified. Summerheat invariably combines with Dampness; without Jiaozhu (Stir-fried Atractylodis Rhizoma), Dampness cannot be sufficiently dried. Summerheat invariably accumulates Cold; without Fuzi (Aconiti Lateralis Radix Praeparata), Cold cannot be sufficiently warmed. (Empirical Formula: Fuzi, Jiaozhu, Huangqi, Ganjiang, Yiyiren, Bian Dou, Yunling, Zhigancao). Zhang Jiegu stated: "Obtained while resting is Zhong Shu."

In *Zhong Re* (Sunstroke), one contracts the scorching Qi from the sky. Yang meets Yang. Symptoms include headache, fever, greasy face, and spontaneous sweating, appearing no different from *Zhong Shu*. However, the urine is red and scanty/difficult, stools are hard, chest fullness and panting occur, restlessness and insomnia are present, and the pulse is surging and rapid. Compared to *Zhong Shu*, they are worlds apart. When Heat is extreme causing dryness, Maidong (Ophiopogonis Radix) is indispensable to clear dryness. When Heat becomes toxic, Huanglian (Coptidis Rhizoma) is indispensable to resolve toxicity. When Heat depletes fluids, Zhuling (Polypori Umbellati Sclerotium) is indispensable to promote urination. (Empirical Formula: Maidong, Huangqin, Zexie, Jiaozhu, Zhuling, Fuling, Qianren, Zhigancao). Zhang Jiegu stated: "Obtained while moving is Zhong Re."

Among the Five Elements, only Fire has two aspects, hence within the Five Movements there are Six Qi. With Six Qi come the six pernicious influences: Wind, Cold, Summerheat, Heat, Dryness, and Fire. Extreme Heat is Fire disease. Medical texts note that "Summerheat is Yang evil, Heart belongs to Li Fire, thus Summerheat enters the Heart"; I do not know where Heat is placed in this logic. Some divide Summerheat into Yin and Yang syndromes, while associating Fire with various Fires; do they realize that Fire is the internal Fire of the six pernicious influences? Some equate Summerheat with Cold Damage in summer; I do not know what disease Summerheat truly is. Thousands of books repeat the same errors, leaving readers bewildered, largely due to a failure to carefully comprehend the Five Movements and Six Qi. I therefore cross-referenced and verified, striving to distinguish them clearly. Validated against symptoms, the distinctions are indeed precise to the hair's breadth.
