(Rash, also known as Sha Zi [Sha Rash], Ma Zi [Measles], or [missing character].)

Zhu Jichuan stated: Rash is also caused by fetal toxin (Tai Du), slightly milder than smallpox (Dou Zheng), yet it involves different organs and viscera. Its onset is triggered by seasonal pathogens (Shi Qi). (Spring warmth, summer heat, autumn coolness, winter cold are the normal qi of the four seasons. If winter should be cold but turns warm instead, yang qi releases too early, leading to rash in spring. Thus, the classics state: "Living in warmth during winter leads to measles and smallpox in spring," and "When Shaoyang ministerial fire prevails externally, red rashes erupt.") Syndromes are differentiated into deficiency (Xu) and excess (Shi), with treatments varying between tonification (Bu) and purgation (Xie). However, its clinical presentation resembles Taiyang disease (Tai Yang Bing), featuring alternating chills and fever, headache, watery eyes, clear nasal discharge, vomiting and diarrhea, wheezing and coughing, sneezing, delirium, dysuria, anorexia, irritability and restlessness, and disturbed sleep. This occurs because yang fire attacks, allowing toxin to invade the Spleen (Pi), heat to linger in the Heart (Xin), and interfere with the Lung (Fei). Since the Lung governs skin and hair, and the Spleen governs muscles, the eruption of rash relies on these two organs to transport the toxin. Only if the rash is thoroughly vented can the toxin be fully expelled through the skin, clearing internal heat and preventing disaster. Therefore, the eruption of rash must be accompanied by coughing, sneezing, epistaxis, vomiting, and diarrhea. If the rash initially appears on the head and face, extending down to the feet marks its limit. A lighter red color on the head and face, appearing in greater abundance, is auspicious; the lesions resemble mustard seeds, dense and fine, colored like peach blossoms, vibrant and alive. Appearing and disappearing two or three times, fading after three to four days, with calm demeanor, normal diet, and regulated bowel and bladder movements, this indicates a favorable course (Shun Zheng). Conversely, high fever with rapid breathing, failure of the rash to erupt, immediate disappearance upon appearance, premature disappearance due to wind exposure, or eruption that is purple-black and spiritless, pale white and dry, or present on limbs but absent on the head, accompanied by wheezing, chest distension, orthopnea (shoulder breathing), mania and delirium, bleeding from mouth and nose, hand clenching and head shaking, groping at bedding, anorexia, hiccups, nausea, constipation, and fetor oris resembling corpse odor, are all untreatable conditions. If there is wheezing, coughing, irritability, restlessness, insomnia, constipation and urinary retention, anorexia, and although the rash has erupted, it is purple and stagnant, this indicates blazing toxic fire; treatment should prioritize clearing heat and resolving toxicity. If the rash has penetrated but is pale white, with dry non-stop coughing, reduced appetite, loose stools, and mental fatigue, this indicates insufficiency of Middle Qi (Zhong Qi); treatment should consolidate Middle Qi while simultaneously clearing the Lung. Once righteous qi is insufficient to expel pathogenic factors, causing toxin to lie dormant internally, death may result from wheezing and abdominal distension, commonly known as "Stifled Rash" (Men Zhen). Occasionally, external wind-cold invades and closes the Cou Li (interstitial spaces), or food stagnation obstructs the airways, making eruption difficult; treatment should prioritize dispersing and promoting circulation. The treatise states: Mild sweating prevents pathogen accumulation; clear urination prevents toxin stagnation. Furthermore, if intestinal blockage exists, purgation should not be delayed; if Cou Li are obstructed, diaphoresis should not be迟 (delayed). Epistaxis allows pathogen release via blood; diarrhea allows toxin release via bowels; sore throat requires urgent fire-descending action; persistent thirst and irritability require priority detoxification. Reduced diet necessitates stomach protection; delirious speech requires heart-clearing. In cold seasons, use acrid-hot herbs; in cool seasons, use acrid-warm herbs; in warm seasons, acrid-cool herbs are unobstructed; in hot seasons, acrid-cold herbs may be applied. Thus it is said: One must first understand the annual climate and not violate the harmony of Heaven and Humanity. However, treatment must primarily focus on dispersion. Dispersion ensures smooth skin and open Cou Li, allowing complete resolution of toxin, leaving no residual pathogen for future complications. Ignoring dispersion, or mistakenly using warming/cold herbs, or neglecting contraindications, allows pathogens to remain trapped internally, generating myriad complications such as irritability, diarrhea, hemorrhage, red eyes, oral ulcers, anorexia, constipation, sore throat, hoarseness, wheezing with phlegm, carbuncles, and sores. The ancients said: Treatment must distinguish deficiency and excess, and methods must be flexible. Such adaptability is divine skill.

Modern people consider rash mild and fail to provide proper care, leading to external constraint by wind-cold and internal injury by raw/cold foods, which stifles toxic vapors and prevents outward expression. The desire to erupt fails, or it erupts and immediately vanishes, causing toxin to attack inwardly, resulting in irreversible regret. Physicians also deem it easy to treat, prescribing recklessly without regard for contraindications, often harming patients without taking responsibility, which is truly pitiable.

Rash eruptions include correct rash of the Fu organs, wind-cold induced rash, severe toxic rash, and internal injury induced rash; these must be distinguished. Correct rash of the Fu organs is treated with acrid-cool dispersants; wind-cold rash with acrid-warm diaphoretics; severe toxic rash with acrid-cool heat-clearers; internal injury rash with bitter-neutral purgatives.

Although dispersion is paramount for rash eruption, if the exterior is excess and penetration is difficult, or if constrained by wind-cold, dispersion is appropriate. However, if the exterior is deficient with spontaneous sweating, and the rash toxin easily emerges, reckless dispersion will inevitably cause the harm of "deficiency compounding deficiency." Clinical judgment must differentiate deficiency and excess, avoiding uniform treatment.

Throughout the course of rash, harsh and drying herbs must be strictly avoided. Rash originates in the Lung, which belongs to Metal and corresponds to Dui in the Bagua. It prefers moistening and fears dryness. Formulas that are vigorous and harsh lack gentleness; they favor clarity and moisture but dread dryness and acridity. Thus, it is said: Rash requires cooling. Cooling promotes ascent; dryness causes rapid deterioration. If Mahuang Tang (Ephedra Decoction) or Sang Bai Pi (Morus Bark) must be used despite their drying nature, they must be honey-fried and combined with moistening herbs to neutralize their harshness. Misuse leads to excessive dryness of Metal, akin to adding oil to a raging fire, guaranteeing death.

Qian Yi argued that rash requires cooling and acrid-cool dispersion, which is correct. Yet ignorant practitioners mistake "cooling" for "cold," applying Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), and Zhi Zi Gao (Gardenia Paste) immediately upon eruption to cool pathogenic heat. Consequently, many children die successively. Rash is essentially turbid qi, hidden within the body, initially formless and odorless. It requires wind-cold or seasonal pathogens to stimulate its eruption, making diaphoresis and acrid-dispersion appropriate. How can bitter-astringents be used to firm the skin? How can cold-constrictors be used to close the pores? The classics state: "Excess of pathogen constitutes excess syndrome." When pathogen is abundant, how else can it be resolved except through sweating and dispersion? Using bitter herbs to firm the skin and cold herbs to constrict the pores traps the emerging rash pathogen, preventing its excretion. This leads to counter-attack inwardly, resulting in death from wheezing, distension, and confusion. In cases of light toxin or shallow pathogen, or where most toxin has already erupted and violence has subsided, lingering effects may erode the throat and tongue causing aphthous ulcers, or linger in the intestines causing stagnation, prolonging illness until the child wastes away and dies. Cold herbs should only be used during periods of Ministerial Fire command or midsummer, and after the rash has fully erupted, stopping once the disease is addressed. If applied during Cold Water command or severe cold before the rash has fully emerged, the fire will be suppressed by cold and unable to vent outward.
