A child presented with fever, wheezing, cough, vomiting, and inability to eat. I diagnosed this as a rash disease (Zhen Zheng). This is caused by wind-cold sealing the couli (skin pores), causing the pathogenic factor to remain latent and unable to erupt. By using Mahuang Gegen Tang (Ephedra and Pueraria Decoction) to release the exterior, sweating was achieved, allowing the skin to become unobstructed, the rash to emerge fully, and the symptoms to resolve completely. In some cases, despite diaphoretic treatment, there is no sweat or eruption; or if the rash emerges briefly then disappears, it may worsen with wheezing and abdominal distension, becoming incurable.
Another child had fever and cough, with only faint traces of rash. The physician used eruptive medicines, but the rash neither appeared nor disappeared. I diagnosed this as Urticaria (Yin Zhen), caused by exogenous wind-cold stagnating and failing to disperse, which differs from the true measles affecting the fu organs. It was cured with Xiong Su San (Ligusticum and Perilla Powder).
A girl developed a rash, but was treated with cold-natured herbs. She also consumed one or two raw pears, after which the rash vanished. She presented with rapid wheezing, chest fullness, cyanotic face, cold limbs, closed eyes, hoarse voice with vomiting, and syncope. I reasoned that the toxin was constrained by cold and counter-attacked internally. I administered Mahuang Tang (Ephedra Decoction) augmented with Gegen (Pueraria Root), Zisu (Perilla Leaf), Gancao (Licorice), Jiegeng (Platycodon Root), and Shengjiang (Fresh Ginger). Covering her with blankets induced sweating, leading to recovery and re-emergence of the rash. However, wheezing worsened. I attributed this to the sudden use of the drying and acrid nature of Mahuang, causing intense toxin-fire and lung heat. Treatment should focus on clearing and moistening. I prescribed Gancao, Jiegeng, Niubangzi (Arctium Fruit), Qianhu (Peucedanum Root), Xingren (Apricot Seed), Xuanshen (Scrophularia Root), Zhimu (Anemarrhena Rhizome), Tianhuafen (Trichosanthes Root), Huangqin (Scutellaria Root), and Maimendong (Ophiopogon Tuber). The wheezing resolved.
A child had fever, rapid wheezing, and abdominal distension. The attending physician diagnosed internal injury combined with external contraction, but treatment failed. Upon my examination, faint redness was visible on the chest and back, indicating a rash disease. I used Mahuang Gegen Tang to release the exterior. Although the rash appeared on the head and face, it failed to fully emerge and subsequently vanished, leading to death.
A child had fever, wheezing, abdominal distension, unconsciousness, bleeding from the mouth and nose, cyanotic-white complexion, and dryness. I diagnosed this as "Menzhen" (Stifled Rash, where toxins are trapped internally), which is incurable.
A child had fever, rash, vomiting, and diarrhea. I reasoned that vomiting and diarrhea during the initial eruption of rash indicate the venting of yang fire, which is an auspicious sign. I used Shengma Gegen Tang (Cimicifuga and Pueraria Decoction) to release the exterior. The rash fully emerged, and the child recovered. In cases complicated by food stagnation causing vomiting and diarrhea, digestive aids were added to the formula.
A child had fever, headache, and bone pain (symptoms of Shanghan/Taiyang syndrome), along with cough and rapid breathing (symptoms of rash disease), hiccups, nausea, and anorexia. I diagnosed this as Shanghan combined with rash disease. I treated with Baijie San (Hundred Resolutions Powder) and Shishen Jiedu Tang (Ten Spirits Detoxification Decoction) to induce sweating. The symptoms resolved, the rash emerged, and the child recovered. Conversely, if similar symptoms are treated slowly by physicians, even if the rash emerges, a purple-black color with wheezing, distension, and mental confusion indicates an incurable condition.
A child had half-emerged rash, high fever, wheezing, abdominal distension, irritability, and mental confusion. I reasoned that incomplete eruption allows evil toxins to attack internally. I treated with Mahuang, Gancao, Jiegeng, Gange (Kudzu Root), Jingjie (Schizonepeta), Qianhu, Zhike (Aurantium Fructus), and Niubangzi. The rash fully emerged, faded gradually over two or three episodes, and the child recovered.
A child had difficulty in rash eruption, accompanied by wheezing, distension, and coma. I reasoned that exogenous wind-cold caused toxin stagnation, hindering eruption. I administered Guizhi Tang (Cinnamon Twig Decoction) augmented with Mahuang, Gegen, and Qianhu. Additionally, Fangfeng (Saposhnikovia Root) decoction was prepared in a basin and placed under the patient's bed for fumigation. Covered with thick quilts, sweating occurred, allowing the rash toxins to fully emerge and the child to recover. Alternatively, bathing the head, face, hands, and feet in Yanxu Fangfeng Tang (Celery and Saposhnikovia Decoction) is effective. Rubbing the body with Ramie dipped in Yanxu wine is even more beneficial.
A child sweated and the rash emerged, but wheezing persisted. I reasoned that pathogenic qi was excessive. I treated with charred Mahuang, Xingren, Gancao, and Shigao (Gypsum) to cure the condition.
A child had purple-colored rash, constipation, dark urine, irritability, and mental confusion. I reasoned that the rash toxin was hyperactive. I used Dachaihu Tang (Major Bupleurum Decoction) to promote bowel movements three times, resolving all prior symptoms.
A child had profuse rash resembling brocade patterns, interspersed with granules, red in color, with high fever and irritability, coated tongue, and constipation. I diagnosed this as simultaneous Ban (macules) and Zhen (papules). I treated with Tiaowei Chengqi Tang (Regulating the Stomach and Directing Downward Magnolia Bark Decoction) to purge, followed by Baihu Tang (White Tiger Decoction) combined with Gegen Tang to cure.
A child had purple rash, wheezing, cough, hiccups, diarrhea, and anorexia. I reasoned that the rash toxin was hyperactive. I used Jiedu Yin (Detoxification Drink) combined with Sha (plucking sand/scarification) therapy to cure.
A child’s rash fully emerged but was prematurely suppressed due to exogenous wind-cold, resulting in wheezing and distension, which was incurable. However, urgent administration of Mahuang Guizhi Tang (Ephedra and Cinnamon Twig Decoction) can sometimes cause the rash to reappear and lead to recovery.
A child in the first lunar month developed rash, with faint visibility on the body and limbs, poor spirit, and no rash on the head and face. The complexion was cyanotic-white, with wheezing, distension, mental confusion, and a faint pulse at the right Cun position. I reasoned that righteous qi was deficient and unable to drive pathogens upward to the head and face. Treatment should prioritize tonifying and assisting ascent. I administered Mahuang Guizhi Tang plus 2 qian of Renshen (Ginseng) decocted in water. Bathing the head, face, hands, and feet in Yanxu Fangfeng Tang helped the rash emerge and symptoms resolve.
A child had purple rash, constipation, dark urine, irritability, and mental confusion. I reasoned that the rash toxin was hyperactive. Dachaihu Tang cured the condition.
A child was erupting rash but indulged in food, leading to stagnation, abdominal fullness, constipation, high fever, and delirium. I reasoned that food stagnation prevented toxin resolution. I treated with Dahuang (Rhubarb), Zhishi (Bitter Orange), Houpo (Magnolia Bark), Gualou Ren (Trichosanthes Seed), Gancao, and Huanglian (Coptis Rhizome) to purge. However, wheezing, cough, high fever, slow pulse, and cold limbs persisted. I treated with Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction) and Guishao Liujunzi Tang (Angelica and Peony Six Gentlemen Decoction) to cure.
A child had erupted rash but presented with wheezing, distension, constipation, high fever, and delirium. I reasoned that toxins were stagnant and not fully expelled. I treated with Huanglian, Zhishi, Gualou Ren, Sangbaipi (Mulberry Root Bark), Digupi (Groundcherry Root Bark), Zhimu, Shigao, and Renzhonghuang (Human Waste Charcoal) to cure.
A child exposed to the smell of vinegar while erupting rash suffered hoarseness, resulting in a lifelong chronic ailment.
In years governed by the Sovereign and Ministerial Qi, epidemic rashes are common. Treatment with cooling and dispersing agents invariably leads to recovery. However, using harsh, drying, and aggressive dispersing agents often causes complications. The Classics state: "First understand the seasonal qi, do not violate the harmony of heaven and humanity." This applies here.
De notes: In years of Jia, Geng, Wu, Ren, Chen, Xu, Si, and Hai, when Shaoyang guest qi prevails, epidemic rashes are frequent. Treatment should prioritize releasing toxins and the exterior.
A child with difficult rash eruption was cured by Er Ren Gao (Two Seeds Paste), which caused the rash to fully emerge.
A child with rash, abdominal fullness, and constipation was diagnosed with internal injury triggering rash. Chengqi Tang (Magnolia Bark Decoction) purged the bowels, leading to recovery.
A child had purple rash, premature suppression, rapid wheezing without cough, blocked nasal passages, open mouth and shrugging shoulders, convex chest like a turtle shell, dry tongue and lips, head shaking, and hand grasping. The complexion was withered and cyanotic-white. I reasoned that fierce evil fire obstructed the lung orifices, making it incurable. Thus, it is said: Wheezing with cough is treatable; wheezing without cough is difficult to heal.
A child with emerging rash had choking on water in the throat, coated tongue, and dry lips. I reasoned that toxins lingered in the heart and stomach. Huanglian Jiedu Tang (Coptis Detoxification Decoction) augmented with Lianqiao (Forsythia) and Niubangzi cured the condition.
A child with post-rash fever and persistent cough was diagnosed with residual toxins attacking the Metal (Lung). Qingjin Huadu Tang (Clear Metal Transform Toxin Decoction) cured the condition.
A child with rash had profuse sweating and severe wheezing. I cited Zhang Zhongjing: "Wheezing with high fever indicates intense internal heat." Mahuang, Xingren, and Shigao cured the condition.
A child in summer had difficult rash eruption, no sweat, and wheezing. Mahuang Tang augmented with Zhimu, Shigao, and Huangqin cured the condition.
A child in summer had rash, fever, headache, wheezing, cough, and no sweat. I reasoned that wind-cold obstructed the couli. Shengma Gegen Tang augmented with Qianghuo (Notopterygium), Baizhi (Angelica Dahurica), Jingjie, Jiegeng, Qianhu, and Zhimu allowed the rash to fully emerge. For residual fever, Xiangru Yin (Elsholtzia Decoction) combined with Huaban Tang (Transform Macules Decoction) cured the condition.
A child in summer had severe rash heat, irritability, and thirst. I reasoned that rash combined with summerheat toxin. Xiangru Yin combined with Gegen Tang cured the condition.
A child developed rash after smallpox. Others believed righteous qi was unrecovered and used tonifying and ascending agents, resulting in rapid wheezing and stifled rash, which was incurable.
A man had fever, wheezing, and cough. The physician used antipyretic and antitussive agents. The body cooled, but wheezing worsened, and sore throat appeared. I diagnosed rash disease, mismanaged by cold-natured drugs causing toxin constraint by cold, preventing eruption. Mahuang Guizhi Tang augmented with Gange induced slight cold sweat, rash emergence, and recovery.
A child with unresolved rash pathogens had fever, wheezing, cough, and hoarseness. Ganjie Niubang Tang (Licorice, Platycodon, and Arctium Decoction) augmented with Suzi (Perilla Fruit), Qianhu, Sangbaipi, Xingren, and Lianqiao cured the condition. Another child with similar symptoms was cured by Ganjie Niubang Tang augmented with Xingren, Zhimu, Xuanshen, Qianhu, Tianhuafen, Maimendong, and Danzhuye (Lophatherum Herb).
A child with post-rash fever was cured by Liangge San (Cool the Diaphragm Powder).
A child with post-rash intermittent dry cough was diagnosed with lung qi deficiency due to excessive dispersion. Xiaoyigong San (Minor Extraordinary Merit Powder) augmented with Maimendong, Wuweizi (Schisandra), Beimu (Fritillaria), and Jiegeng cured the condition.
A child with post-rash dry cough, loose stools, and reduced appetite was diagnosed with middle qi depletion. Tonification with Liujunzi Tang (Six Gentlemen Decoction) cured the condition.
A child with persistent post-rash fever, worse in the afternoon, was diagnosed with yin deficiency and blood depletion due to previous yin consumption. Treatment should nourish yin and clear fire, following the principle of nourishing yin to retreat yang.
A child with post-rash sore throat and swelling was diagnosed with unresolved residual toxins. Ganjie Tang augmented with Niubangzi, Yeyan (Belamcanda), Xuanshen, Lianqiao, and Zhimu, combined with Gejin San (Kadsura Vine Powder) insufflated into the throat, cured the condition.
A child with post-rash fever and persistent irritability/thirst was diagnosed with deficiency vexation. Zhuyeshigu Tang (Bamboo Leaf and Gypsum Decoction) minus Banxia, augmented with Gange and Tianhuafen, cured the condition.
A child with post-rash phlegm-cough, oral ulcers, fever, and abdominal fullness was diagnosed with premature tonification. Qingwei Tang (Clear Stomach Decoction) augmented with Fupi (Areca Peel) and Zhike cured the condition.
A child with post-rash epistaxis was cured by drinking concentrated Maohua (Rhododendron Flower) decoction, or using Baimaogen (Imperata Rhizome).
A child with post-rash high fever, cough, and hemoptysis was diagnosed with toxins lingering in the lungs and stomach. Huanglian, Huangqin, Shanzhi (Gardenia), Zhimu, Tianhuafen, Xuanshen, and Renzhonghuang cured the condition.
A child with post-rash high fever, thirst, and continuous bloody diarrhea was cured by Baitouweng Tang (Pulsatilla Decoction).
A child with post-rash dysentery (pus and blood, tenesmus) was diagnosed with toxins entering the large intestine. Sanhuang Wan (Three Yellows Pill) was used first to purge, followed by Huanglian Shaoyao Tang (Coptis and Peony Decoction) to cure.
A child with post-rash persistent dysentery, poor intake, slow pulse, and cold limbs was diagnosed with spleen-stomach qi deficiency. Lizhong Tang (Regulate the Middle Decoction) and Guishao Liujunzi Tang cured the condition. Non-responsive cases may result in death from Jin Kou Li (Closed Mouth Dysentery) or Sun Xie (Undigested Food Diarrhea).
A child with post-rash high fever, emaciation, irritability, and mental clouding was diagnosed with unresolved pathogens attacking the heart and liver. Clearing and resolving agents cured the condition. Non-responsive cases progressed to withered skin and hair, forming Gan Zhai (Infantile Malnutrition and Consumption), dried fluids, and death.
A child with post-rash high fever, dry cough, thirst, and constipation was diagnosed with unresolved rash pathogens. Sanhuang Tang purged, followed by Zhimu, Maimendong, Qianhu, Xuanshen, Huanglian, Danggui (Angelica Sinensis), Tianhuafen, and Danzhuye. After fever subsided, persistent cough and thirst indicated lung qi injury and fluid deficiency. Renshen, Maimendong, Wuweizi, Beimu, Chenpi (Tangerine Peel), Gancao, Jiegeng, Tianhuafen, and Zhimu led to gradual recovery.
A child with similar symptoms but persistent fever progressed to muscle wasting, pale complexion, hiccups, nausea, and diarrhea. Diagnosed as post-rash Gan Lao (Infantile Consumption), it was incurable. Death occurred at age sixty.
A child with post-rash dry cough, loose stools, fever, emaciation, and withered skin/hair was diagnosed with post-rash Gan Zhai. Xiaoyigong San augmented with Beimu, Huanglian, Qinghao (Artemisia Annua), Digupi, Longdancao (Gentian Root), and Shaoyao cured the condition gradually. Non-responsive cases may develop Man Pi Feng (Slow Spleen Wind) and die.
A son developed persistent phlegm-cough after post-rash illness. Others attributed it to residual toxins and used clearing agents, worsening the phlegm. I diagnosed yin deficiency with unconstrained fire, causing afternoon tidal fever and severe cough. Treatment prioritized strengthening water (nourishing yin). Liuwei Dihuang Wan (Six Ingredient Pill with Rehmannia) material augmented with Maimendong and Zhimu cured the condition. A woman with similar symptoms was cured by adding Danggui.
A child with post-rash red swollen eyes, high fever, and thirst was diagnosed with unresolved toxins attacking the liver and stomach. Qingwei Jiedu Tang (Clear Stomach and Detoxify Decoction) cured the condition. Delayed treatment may lead to blindness.
A child with post-rash widespread sores, high fever, and restlessness was diagnosed with unresolved rash toxins. Gegen Tang augmented with Jingjie and Fangfeng released the exterior, followed by Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) to cure.
A child with post-rash Zouma Yagan (Running Horse Gum Rot), gum ulceration penetrating the nose, and ineffective treatments was treated. De notes: Suspect Plum Blossom Gan toxin. I used Huanggu Niu Fen (Yellow Ox Dung) charred on tile, Renzhongbai (Calcinis Humanis), and Huangbai (Phellodendron Bark) powdered and insufflated. Ulcers healed, gums and teeth regenerated.
A child with unresolved rash pathogens developed Ding Du (Carbuncle). I diagnosed Sha Ding (Sand Carbuncle), treating similarly to Dou Ding (Smallpox Carbuncle). Another child developed Zhong Yong (Swelling Abscess) from unresolved rash toxins. I diagnosed Sha Yong (Sand Abscess), treating similarly to Dou Yong.
A child with persistent post-rash dry cough was diagnosed with lung qi deficiency from excessive dispersion. Spleen and lung tonification was advised, but ignored. Only clearing fire and stopping cough agents were used, leading to kyphosis and thin legs, a lifelong ailment. Some developed convex chest (Gui Xiong). Mr. Qiyun stated: Post-rash chronic cough weakens Metal; Metal weakness fails to generate Kidney Water. Kidney governs marrow; lack of kidney vitality leads to bone withering and marrow depletion. Wind-cold enters marrow, stagnates, and causes spinal immobility. Treatment: First dispel evil with Fangfeng San. Then, Bajia Dihuang Wan (Eight Ingredient Rehmannia Pill) augmented with Renshen, Duzhong (Eucommia), Niuxi (Achyranthes), Danggui, Shihu (Dendrobium), Heshouwu (Polygonum Multiflorum), Yimi (Coix Seed), Tusizi (Cuscuta), Bibi (Dioscorea), and Lurong (Deer Antler) honey pills. Apply Qufeng Zhuangjin Huoxue Gao (Wind-Dispersing, Tendon-Strengthening, Blood-Activating Ointment) to protrusions. Moxibustion on Feishu (BL13) and Geshu (BL17) points. For a child with slow-weak right pulse indicating spleen-lung deficiency, Renshen, Baizhu (Atractylodes), Fuling (Poria), Chenpi, Gancao, Beimu, Danggui, Shaoyao, Yimi, and Shihu decocted strengthened the spleen earth. Subsequent treatment allowed leg flesh regeneration and slight improvement in kyphosis, though not complete recovery.
Generally, post-rash care requires avoiding wind-cold and regulating diet to protect spleen earth. Tonification or purgation should be applied based on excess or deficiency. Do not neglect care to avoid deterioration. Chicken, meat, salty, sour, pungent, and spicy foods should be avoided for 49 days (seven sevens) before gradual reintroduction. Thus, early chicken consumption risks spleen diarrhea; unrestricted salty/sour intake risks asthma. Carelessness may cause lifelong ailments. Parents must be vigilant.
