Peng Qifeng wrote in the preface: The categories of medicine are most numerous, and its principles are extremely difficult to master. Among them, the specialty of the throat is particularly difficult. The pharynx (throat for swallowing) receives food, and the larynx (throat for breathing) receives air. The path receiving food connects to the stomach cavity and communicates with the spleen, transforming according to the Earth element. The path receiving air connects to the lung cavity and communicates with the heart, transforming according to the Metal element. The nature of Metal is dry; its pathological change manifests as stagnation or roughness. Stagnation leads to obstruction and insensibility; therefore, diseases of the larynx are called "Bi" (obstruction). The nature of Earth is damp; its pathological change manifests like mud. Mud leads to blockage, swelling, and lack of flow; therefore, diseases of the pharynx are called "Zhong" (swelling). People know the existence of the throat, but regarding the syndrome differentiation of deficiency/excess and cold/heat, as well as the therapeutic methods of attacking/supplementing and descending/rising, few can analyze with precision down to the finest detail and make correct judgments amidst ambiguity. Without listening to the voice, palpating the pulse, distinguishing Qi, and observing form, even a slight difference will lead to a vast diagnostic error. Even if one acts with caution and tries to probe and guess, one often fails to hit the mark. If urgent conditions are treated with slow methods, or severe injuries are supported with light measures, the harm of procrastination and the deviation of treatment methods would be immeasurable. Thus it is said: this is the most difficult of difficulties. Mr. Liuxian Zhang from our county has always been proficient in medical principles. He has studied the throat specialty deeply and for a long time. He collected and compiled existing formulas, incorporated his own insights, detailed the regulations, and compiled them into a book. From spiritual energy and pulse principles to colors of the face (blue, red, purple, white) and tones of voice (high, low, deep, floating), everything is annotated clearly and distinctly, like listing eyebrows side by side. Combined with the treated syndromes, it is as accurate as taking shadows under a lamp, with hardly any failure. This is truly a boat of compassion saving the world, and a raft of treasures rescuing the critical. The cases where he achieved efficacy through experience are countless. His colleagues urged him to have it printed and published, so that those learning this art near and far could circulate his theories and unfold their profound meanings. His merit to the world is immeasurable. This is the preface. Spring, third month of the Dingchou year of the Qianlong reign.
