Coptidis Rhizoma (Chuanlian, 黄连) [1 qian], Scutellariae Radix (Huangqin, 黄芩) [1 qian], Rehmanniae Radix Recens (Shengdihuang, 生地黄) [3 qian], Gardeniae Fructus (Shanzhi, 山栀子) [1.5 qian], Moutan Cortex (Danpi, 牡丹皮) [1.5 qian], Akebiae Caulis (Mutong, 木通) [1 qian], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [1 qian].
Note: The above formula primarily treats conditions of blazing heart fire ascending, bitter taste in the mouth, sores on the tongue, stagnation and obstruction in the small intestine leading to difficulty in urination or bowel movements.
When the tongue presents as red, swollen, and tender, it indicates deficiency of essence and qi of the Heart and Small Intestine; Yangxin Tang (Nourish the Heart Decoction) should be used as the primary treatment.
Poria Cum Radix Pini (Fushen, 茯神) [2 qian], Polygalae Radix (Yuanzhi, 远志) [5 fen], Ziziphi Spinosomi Semen (Zaoren, 酸枣仁) [5 fen, stir-fried and crushed], Platycladi Semen (Baiziren, 柏子仁) [5 fen, oil removed], Schisandrae Fructus (Wuweizi, 五味子) [5 fen], Ginseng Radix (Renshen, 人参) [5 fen], Astragali Radix (Huangqi, 黄芪) [2 qian, honey-fried], Angelicae Sinensis Radix (Danggui, 当归) [2 qian], Chuanxiong Rhizome (Chuanxiong, 川芎) [2 qian], Pinelliae Tuber (Banxia, 半夏) [2 qian], Cinnamomi Cortex (Rougui, 肉桂) [5 fen], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [5 fen].
Note: The above formula primarily treats deficiency of heart blood and insufficient blood, resulting in unsettled spirit, palpitations (zhengchong), and fright-induced anxiety (jingji).
When the tongue presents as red, dry, and belongs to yin deficiency with hyperactive fire of the Small Intestine, Zishui Qinggan Yin (Moisturize Water and Clear Liver Decoction) without Bupleuri Radix (Chaihu), plus Rehmanniae Radix Recens (Shengdihuang) and Akebiae Caulis (Mutong), should be used as the primary treatment.
Rehmanniae Radix Praeparata (Shudihuang, 熟地黄) [4 liang], Dioscoreae Rhizoma (Shanyao, 山药) [2 qian], Corni Fructus (Yurou, 山茱萸) [2 qian], Moutan Cortex (Danpi, 牡丹皮) [1.5 qian], Poria (Fuling, 茯苓) [1.5 qian], Alismatis Rhizoma (Zexie, 泽泻) [1.5 qian], Ziziphi Spinosomi Semen (Zaoren, 酸枣仁) [1 qian], Paeoniae Radix Alba (Baishao, 白芍) [2 qian], Gardeniae Fructus (Shanzhi, 山栀子) [1.5 qian], Angelicae Sinensis Radix (Danggui, 当归) [2 qian], Rehmanniae Radix Recens (Shengdihuang, 生地黄) [3 qian], Akebiae Caulis (Mutong, 木通) [1.5 qian].
Note: The original formula primarily treats insufficiency of kidney water and liver fire flaming upward, leading to acid regurgitation, hypochondriac pain, dizziness, bitter taste in the mouth, dry throat, difficult and constipated stools, and scanty dark urine. It essentially adopts Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) to address the root cause without obstructing the middle jiao; and adopts Shengdi Huang Tang (Rehmannia Root Decoction) principles to descend fire without violating the lower jiao. This is truly a insight not previously mentioned.
When the tongue presents as red and dry, belonging to blood deficiency with abundant fire in the Heart, Jisheng Guipi Tang (Jisheng Spleen-Nourishing Decoction) without Aquilariae Lignum Resinatum (Muxiang), plus Moutan Cortex (Danpi) and Ophiopogonis Radix (Maidong), should be used as the primary treatment.
Poria Cum Radix Pini (Fushen, 茯神) [1 qian], Polygalae Radix (Yuanzhi, 远志) [1 qian], Ziziphi Spinosomi Semen (Zaoren, 酸枣仁) [1 qian], Angelicae Sinensis Radix (Danggui, 当归) [1.5 qian], Gingeris Rhizoma Preparatum (Weijiang, 煨生姜) [1 qian], Ginseng Radix (Renshen, 人参) [1 qian], Astragali Radix (Huangqi, 黄芪) [2.5 qian], Atractylodis Macrocephalae Rhizoma (Dongshu, 冬术/白术) [1.5 qian], Longan Arillus (Longyanrou, 龙眼肉) [5 pieces, shell removed], Moutan Cortex (Danpi, 牡丹皮) [1.5 qian], Ophiopogonis Radix (Maidong, 麦冬) [1 qian], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [1 qian], Jujubae Fructus (Dazao, 大枣) [5 pieces].
Note: The original formula primarily treats heart decline with abundant fire, failing to generate earth (spleen/stomach), leading to spleen distress and lung failure, accompanied externally by cough with phlegm, alternating chills and fever, and night sweats; all these are treated with this formula. Whenever spleen and stomach weakness, lack of appetite, and diarrhea are observed, it is generally caused by weak heart qi; this is the method of tonifying the root. For various deficiency syndromes where Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) is ineffective, administering this formula plus Schisandrae Fructus (Wuweizi) and Paeoniae Radix Alba (Baishao) to consolidate heart qi yields even more miraculous results. Furthermore, Buzhong Yiqi Tang tonifies yang, while Guipi Tang tonifies yin. For those whose spleen is damaged by hunger, fullness, or overwork leading to qi deficiency, Buzhong Yiqi Tang is appropriate; "Buzhong" means tonifying the middle to benefit qi. For those whose spleen is damaged by excessive thinking and depression leading to blood deficiency, Guipi Tang is appropriate; "Guipi" means nourishing blood and returning it to the spleen. When both heart power and qi/blood are exhausted, taking Buzhong Yiqi Tang alone is not对症 (symptom-specific), and combining them does not form a coherent prescription; only Yangrong Tang (Nourish Vigor Decoction, likely referring to Renshen Yangrong Tang) can unify both conditions of Buzhong and Guipi, avoiding the pitfall of addressing one aspect while neglecting the other.
When the tongue presents as red and slippery, belonging to deficiency and sinking of yang qi in the Small Intestine, Buzhong Yiqi Tang (Tonify the Middle and Augment Qi Decoction) plus Gardeniae Fructus (Shanzhi) and Aconiti Lateralis Radix Praeparata (Chuanwu, 川乌) should be used as the primary treatment.
