Pang. Massive blood loss leads to emptiness in the collaterals (Luo Mai). The damage is no longer confined to a single Zang or Fu organ. During the Autumn Equinox and Cold Dew solar terms, the weather commands descent, yet the body's Qi rebels by ascending. Clearly, the interior fails to consolidate and secure; how can there be doubt about this deficiency pattern? Current pulse diagnosis reveals weak, soggy, and choppy qualities, with slightly cold limbs and joints. Qi injury causes upward rebellion, resembling misty smoke scorching the throat base, thus inducing choking coughs and rebellious Qi. The general principle is that the Upper Jiao should be descended and unblocked, while the Lower Jiao should be sealed and secured. If food intake can be stabilized to support the Earth element (Spleen/Stomach), subsequent methods can be discussed.
Ren Shen (Ginseng Radix et Rhizoma), Hei Qi Zi (Charred Lycii Fructus), Hei Niu Xi (Charred Achyranthis Bidentatae Radix), Fu Shen (Poria with Hostwood), Sheng Yi Yi Ren (Raw Coicis Semen), Chao Shan Yao (Stir-fried Dioscoreae Rhizoma).
Second visit. Bleeding has stopped, but food intake is scant, with no appetite and diarrhea. This indicates severe distress of the Spleen and Stomach, with Yin fire touching upwards, causing facial redness and sudden heartburn. First regulate the Middle Jiao (Spleen/Stomach); increasing meal intake is paramount. Strictly avoid cold-natured herbs for treating coughs, lest they further attack the vital Qi of the Spleen and Stomach.
Ren Shen (Ginseng Radix et Rhizoma), Fu Shen (Poria with Hostwood), Xin Hui Pi (Xinhui Tangerine Peel), Shan Yao (Dioscoreae Rhizoma), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Chao Bai Shao (Stir-fried Paeoniae Radix Alba).
Third visit. Right pulse is soggy; left pulse has not converged.
Ren Shen (Ginseng Radix et Rhizoma), Fu Shen (Poria with Hostwood), Shu Zhu (Prepared Atractylodis Rhizoma), Guang Pi (Tangerine Peel), Nan Zao (Chinese Date).
Fourth visit. Left pulse is calm but deficient; right pulse resembles a rapid pulse. When initially introducing grain food, it is appropriate to cultivate the Middle Jiao. Five days after the Frost Descent solar term, use pill preparations to secure the lower aspect.
Ren Shen (Ginseng Radix et Rhizoma), Fu Shen (Poria with Hostwood), Shu Zhu (Prepared Atractylodis Rhizoma), Guang Pi (Tangerine Peel), Nan Zao (Chinese Date), Chao Bai Shao (Stir-fried Paeoniae Radix Alba), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma).
Chen. Pulse resembles rapid; phlegm-cough and blood loss have persisted for over a hundred days without cessation. Each episode of coughing and vomiting is accompanied by sweating. This belongs to Qi injury failing to exercise control, causing collateral blood to flood upwards. Any use of cold-natured herbs to stop bleeding or treat coughs will not only ruin the Stomach and hinder food intake but will also be completely ineffective. Following Zhang Zhongjing’s principle that when Yuan Qi (Original Qi) is damaged, sweet-tonifying herbs should be advanced, hoping that the Stomach and Earth elements become prosperous, allowing the Metal element (Lung) to naturally tranquilize.
Huang Jing (Polygonati Rhizoma), Sheng Bai Shao (Raw Paeoniae Radix Alba), Wu Wei Zi (Schisandrae Fructus), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Nan Zao (Chinese Date), Yi Tang (Maltose).
Anonymous patient. Blood loss due to labor-induced cough.
Sheng Huang Qi (Raw Astragali Radix), Pi (Tangerine Peel, 3 qian), Fu Ling (Poria, 3 qian), Zhei Gan Cao (Charred Honey-fried Glycyrrhizae Radix et Rhizoma, 5 fen), Huang Jing (Polygonati Rhizoma, 3 qian), Nan Zao (Chinese Date, 3 qian).
Qian (41 years old). Accumulated labor of form and spirit, Qi leakage causing blood loss, reduced food intake, and panting. This results from injury to the Qi and Yang aspects, distinct from labor caused by wine and sexual indulgence. Use Jian Zhong Tang (Cinnamon Twig Decoction for Building the Middle) removing Ginger and Cinnamon Twig.
Lu. Pulse is fine, body is emaciated. After blood loss, chronic cough persists, now compounded by panting. Due to internal damage refusing to replenish, prescribed medications mostly targeted the Lung to treat coughs. At this time of seasonal contraction and肃 (descent), the root is weak and unable to grasp and receive. Yin lacks the ability to retain Yang, resulting in more ascending and less descending. Sitting quietly barely sustains him; any movement causes vigorous Qi flooding. Food ingested only provides its "fierce" Qi, failing to nourish spirit and essence. This disease stems from dark depletion of one's own essence and Qi, not something herbal purgatives can remove. One should reside in quiet mountains, combining with Qigong practices. Adhering to these methods for years allows Yin and Yang to gradually interact, revitalizing life force. Relying solely on medicine may not be adequate. Use Jian Zhong Tang (Cinnamon Twig Decoction for Building the Middle) removing Ginger, adding Poria.
Dong (36 years old). This is an internal damage syndrome: chronic cough, loose stools. In the summer months, the Great Qi governs leakage. After blood vomiting, muscles feel numb, bones are flaccid and ache, and the Yangming meridians are inactive. Treatment must tonify Qi. Strictly avoid Lung-targeted clearing, moistening, and cold-natured herbs.
Huang Qi (Astragali Radix), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Yi Yi Ren (Coicis Semen), Bai Ji (Bletillae Rhizoma), Nan Zao (Chinese Date), Mi Tang (Rice Syrup).
Li (31 years old). Drinking wine with insufficient grain intake leads to prior deficiency of Middle Qi. Wine’s warming and dispersing nature aids heat, causing collateral blood to boil upwards with the heat. After bleeding stops, appetite suddenly drops and epigastric fullness occurs, clearly indicating Middle Qi is distressed and failing. Sitting quietly brings slight relief; irritability and urgent cough follow. Regulating the Middle is urgent. If one sees blood and cough and immediately administers cold-natured herbs, Clear Yang will be further injured, leading to daily deterioration. Although wine drinkers usually avoid sweet flavors, to rescue the damage from bitter-cold herbs, do not adhere to this rule. Use Wu Ji Wan (Wuji Pill) removing Atractylodes, adding Chinese Dates.
Wang (28 years old). Pulse [description missing/implied]. Emaciation and blood loss due to labor. Use Xiao Jian Zhong Tang (Minor Cinnamon Twig Decoction for Building the Middle) adding Yu Zhu (Polygonati Odorati Rhizoma).
Gu (28 years old). Mental labor depletes Spirit and damages Ying (Nutritive) Blood. Blood appears above and below, declining year by year. Now, forcing food intake without hunger indicates Middle Jiao failure to transport. Nourishing Yin and clearing the Lung further injures the Middle. There is no good medicine to cure this disease. To hope for peace, one must reside in the mountains and rest quietly, unaffected by cold and heat, to achieve stability. Use Yi Gong San (Extraordinary Efficacy Powder).
Qian (27 years old). Emaciated body, left pulse rapid, indicating depletion of Essence in the Yin aspect. Self-reported excessive talking/laughing, or stomach hunger-deficiency, inevitably leads to Rebellious Qi and cough. Last year, blood loss filled a bowl. This is lower-level depletion of Essence and Blood; tangible substances are hard to restore. With refined diet, Qi returns and does not rush. Urgently use sweet herbs to benefit the Stomach, acting as a pillar in midstream. When disease reaches the Middle, delay is impossible.
Ren Shen (Ginseng Radix et Rhizoma), Fu Shen (Poria with Hostwood), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Shan Yao (Dioscoreae Rhizoma).
Xu (48 years old). Labor and fatigue injure Yang, causing coldness of form, blood loss, and cough/rebellious Qi. Middle age differs from youth where hyperactive Fire causes cough-blood. Use Huang Qi Jian Zhong Tang (Astragalus Cinnamon Twig Decoction for Building the Middle).
Xu (29 years old). Running for five days constitutes labor exertion injuring Yang Qi. Blood originates from the right side; night sweats occur. This is leakage through gaps in Yang collaterals. Initially treating hematemesis often involves cooling blood and descending Qi to hope for cessation. Who knows that Yang leaks further, worsening the illness repeatedly?
Huang Jing (Polygonati Rhizoma), Huang Qi (Astragali Radix), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Yi Yi Ren (Coicis Semen), Fu Shen (Poria with Hostwood).
Wang. Liver Wind rises violently; Stomach Qi must be deficient. Wine drinkers dislike soft/greasy textures. Skin is soft, complexion tender, constitution Qi is weak. In Qingming spring, Wood Qi is vigorous, inevitably offending Earth. Urgently cultivate the Middle Jiao; having a pillar within prevents Wind-Yang from ascending, allowing blood to stop.
Ren Shen (Ginseng Radix et Rhizoma), Chao Huang Qi (Stir-fried Astragali Radix), Chao Shan Yao (Stir-fried Dioscoreae Rhizoma), Fu Ling (Poria), Chao Bai Shao (Stir-fried Paeoniae Radix Alba), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma).
Zhu (22 years old). Autumn-heat blood loss, recurring in early spring. Right pulse is large. *Jin Gui Yao Lue* states: "In men, a large pulse indicates Lao (exhaustion/labor); an extremely deficient pulse also indicates Lao." Generally, a large pulse Lao is due to mental labor injuring Qi; a deficient pulse Lao is due to emotional/sexual depletion. The main principle is to drive out the disease root completely; resting quietly for one year can lead to recovery.
Sheng Huang Qi (Raw Astragali Radix), Bei Sha Shen (Glehniae Radix), Yi Yi Ren (Coicis Semen), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Bai Ji (Bletillae Rhizoma), Nan Zao (Chinese Date).
Anonymous patient. Blood loss from labor and mental worry. Morning food digests well; evening food causes fullness. Prescribe a formula to disperse deficiency and regulate the Middle.
Ren Shen (Ginseng Radix et Rhizoma), Fu Ling (Poria), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Sheng Gu Ya (Raw Sprouted Grain), Guang Pi (Tangerine Peel), Bai Shao (Paeoniae Radix Alba).
Feng (45 years old). Pulse is wiry and stiff, empty upon pressing. Chronic cough preceded by bloody stool, loose stools. Recently coughing blood. This is accumulated labor and long-term damage, with both Yin and Yang deficient. Currently lacking appetite; first use sweet-warm herbs to tonify Qi. However, poverty makes continuous use of Ginseng and Poria difficult, making regulation hard.
Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), Fu Ling (Poria), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Yi Yi Ren (Coicis Semen), Bai Ji (Bletillae Rhizoma).
Xu (44 years old). Frequent wind invasion, Wei (Defensive) Yang is already loose, and labor/anger also injure Yang. For this blood-loss syndrome, focus solely on regulating Yangming. If Stomach is strong, Liver offense decreases. Right pulse being empty and large confirms this. If Heat steams from the Three Yin, the pulse would involve the left side. (Stomach Yang deficiency, Defensive looseness).
Ren Shen (1 qian), Huang Qi (3 qian), Zhi Cao (5 fen), Wei Jiang (Roasted Ginger, 1 qian), Nan Zao (2 qian).
Second visit. Sweet-warm herbs benefited the Stomach; bleeding stopped for five days. Eating fish/shrimp and anger caused blood/cough to return. Not only is Wei Yang loose, but collaterals are empty and moving like a valley. How can deep-cold obstructing herbs hope for momentary effect? If the Stomach mouth rejects intake, no method applies. Pulse is faint and choppy; discuss treating Heart Ying (Nutritive) and Lung Wei (Defensive).
Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Nan Zao (Chinese Date), Bai Ji (Bletillae Rhizoma), Fu Shen (Poria with Hostwood), Zao Ren (Ziziphi Spinosae Semen).
Tang (23 years old). Pulse is fine and hurried, right side empty and large. Finger nails are gray and dry. Chronic cough shows blood in spring/summer. Reduced food intake, body pain, appearance deteriorates daily. Internal damage is hard to recover; prescribe Ying-nourishing method. (Ying deficiency).
Ren Shen (Ginseng Radix et Rhizoma), Chao Bai Shao (Stir-fried Paeoniae Radix Alba), Gui Shen (Angelicae Sinensis Radix tip), Zhi Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), Gui Zhi Mu (Cinnamomi Ramulus wood), Guang Pi (Tangerine Peel), Wei Jiang (Roasted Ginger), Nan Zao (Chinese Date).
Ding (27 years old). Summer phlegm-cough, winter blood loss. Self-reported daytime rest brings no mild chills/fever, indicating Two Qi (Yin/Yang) are already damaged. Physical activity opposes the disease. Pulse is large without spirit, face lacks luster. Labor-induced consumptive disease fails to recover original vitality. Warm-nourish and sweet-tonify to ensure peace in sleep and eating. If treating phlegm-cough as Heat and daily administering Yin-nourishing/Lung-moistening herbs, Stomach damage and transformation symptoms will occur, making regulation useless. Gui Pi Tang (Restore the Spleen Decoction), Yi Gong San (Extraordinary Efficacy Powder).
Chen (28 years old). Blood loss, chest pain before and after. Gui Pi Jian Zhong Tang removing Ginger.
Anonymous patient. Emaciated, sallow complexion, soggy pulse with chills/fever, blood loss with palpitations. This is Ying injury. Gui Pi Jian Zhong Tang removing Ginger.

Xu (49 years old). School teaching labor injures Heart and Spleen Ying. Sour food aids Wood, further depleting Middle Earth. Spring Yang governs ascent, causing massive vomiting of blood. Moreover, long-term vegetarianism requires cultivating Earth. Ying Yin is extremely depleted, heat blazes internally, not Excess Fire. Gui Pi Tang (Restore the Spleen Decoction) removing Ren Shen.
Chen (23 years old). Previously suffered blood loss, then experienced fright. Usually has nocturnal emission, mutters alone in solitude. Tangible Essence-Blood and intangible Spirit-Qi are mutually injured. Merely discussing Phlegm and Blood treatment is truly crude and lowly skill. Tonifying Essence requires filling; calming Spirit requires stillness. However, intangible True Qi is key. Treat according to Heart and Spleen meridians.

Guan (32 years old). Worry and depression injure Heart and Spleen. These two organs govern Ying Blood. Ying emerges from the Middle Jiao. Visceral Yin is damaged, Yin deficiency generates Heat, steaming collaterals, causing blood unrest. Less food, more phlegm, lackluster complexion—all deficiency signs. Do not long-term administer cooling/moistening cough medicines. Use Qian Yi Gong San (Qian’s Extraordinary Efficacy Powder), occasionally using Gui Pi Tang reducing Mu Xiang.
Ma (67 years old). Lower bleeding last autumn, gushing blood this year. Drinking orange peel soup (sweet/pungent) causes heart pain like needle pricks. Ying is withered, fluids exhausted, cannot tolerate pungent herbs. Only use sweet/herb-soft agents beneficial to Heart and Spleen.

Feng. Left hand pulse is peaceful, Chi position slightly moving. Right hand three positions: Guan front is rapid, Chi pulse carries rapid quality. Insomnia at night, cough-choking with blood. Daytime cough-choking has no blood. Walking causes slight panting. Yin and Yang act as hinges. In deep winter, weather stores and gathers. Due to mental worry and spirit labor, Five Emotions move, Yang does not hide. When drowsy, Qi mechanism descends, touching rising Yang, Chong Mai ascends, blood in collaterals does not tranquilize, overflowing into upper orifices with cough-choking. Walking and talking also aid moving Qi-Fire. This cough-choking, panting, and blood loss share the same origin. Use stillness to control movement; administer herbs to benefit Water generating Metal, controlling Ministerial Fire. Diet should avoid spicy/hot/dry foods. Upper excess implies lower deficiency; light flavors clear the Upper Jiao, which is indeed settling the Lower to allow storage. My rough prescription awaits judgment (Mental labor excess, Yang rising).

Anonymous patient (49 years old). Right pulse choppy, initially Qi surge caused blood loss, cough-rebellion, able to eat, tasteless. Blood comes surging like tide, due to Yangming Stomach collateral emptiness, blood follows Yang ascent. Method must fill the Middle as key point. Do not see blood/cough and use Lung herbs. This syndrome can be managed, precisely here lies the opportunity.

Bathing moves Qi, blood/cough returns. Must use quiet herbs to nourish Stomach Yin. *Jin Gui* Mai Men Dong Tang (Ophiopogon Decoction) removing Ban Xia.
Guo. Right pulse part does not beat against finger, only left Cun is rapid/frequent. Last night’s blood loss cause: stooping to pick up objects caused Yangming collateral blood to rise. Now viewing sallow complexion, sign of blood loss. Do not use cooling/resolving herbs hindering Stomach. Follow ancient saying: blood loss first must tonify Qi. Regulating Stomach should also distance from Liver.

Wang (36 years old). After intestinal redness heals, hematemesis recurs every 1-2 months. This is because Yangming Stomach collaterals have abundant Qi and Blood, so after vomiting, sleep/eating remain normal. Long-term recurrence depletes Yin, still worrying about internal damage. Should nourish Lung/Stomach Yin to harmonize Yang.

Stomach Yang externally corresponds to Wei Qi; nine orifices disharmony all belong to Stomach disease. Due to winter failure to store, Spring generation Qi is scarce, encountering Long Summer heat steaming, True Qi gradually distressed. Xu. Yin viscera lose control, Yang soars, severe cough brings blood, all movement signs. Quiet herbs fit well, but repeated application fails. Upper and lower mutually exploit, Yangming collaterals empty, following Yang Qi ascent/descent freely. First use soft agents to fill Stomach Yin, so-called holding the center to approach correctness. *Jin Gui* Mai Men Dong Tang (Ophiopogon Decoction) removing Ban Xia, adding Huang Qi.
