Monk (52 years old): Recently contracted Wind-Warmth (Fengwen, wind-heat pathogen) affecting the upper jiao. The Cun position of the pulse is exclusively large. The Lung is scorched by Heat, resulting in a hoarse voice. Initially administer acrid-cool herbs to clear the upper jiao. Regulate and nourish with a light diet for ten days. (Wind-Warmth injuring the Lung)
Babaozi (Fructus Armeniacae Amarum), Bohe (Menthae Haplocalycis Herba), Xiangbeimu (Fritillariae Thunbergii Bulbus), Xingren (Armeniacae Semen), Dong Sangye (Mori Folium), Dashashen (Glehniae Radix), Nan Huafen (Trichosanthis Radix), Hei Shanzhipi (Gardeniae Pericarpium).
Mr. Yang: The left pulse is solid and large. The head and eyes feel as if covered or obscured. The clear orifices are not unobstructed. This indicates Wind-Warmth remains in the upper jiao. Propose a method to regulate ascending and descending qi.
Gan Heye (Nelumbinis Folium), Bohe (Menthae Haplocalycis Herba), Xiangbei (Fritillariae Thunbergii Bulbus), Lianqiao (Forsythiae Fructum), Gouteng (Uncariae Ramulus cum Uncis), Sheng Shigaomo (Gypsum Fibrosum).
Certain patient: Wind-Warmth enters from above. Wind belongs to Yang; Warmth transforms into Heat. The location is near the Lung in the upper jiao. Lung Qi cannot circulate freely, causing obstruction in the circulation of Qi throughout the body, leading to body pain, epigastric fullness, and lack of appetite. It is appropriate to use slightly bitter herbs to clear and descend, and slightly acrid herbs to diffuse and unblock. The physician attributed this to the Six Channels and administered Qianghuo (Notopterygii Rhizoma et Radix) and Fangfeng (Saposhnikoviae Radix), which deplete Yang Qi and seize Stomach fluids. Warm pathogens contraindicate sweating; why was this forgotten so quickly?
Xingren (Armeniacae Semen), Xiangchi (Sojae Preparata), Yujin (Curcumae Radix), Shanzhi (Gardeniae Fructus), Gualoupi (Trichosanthis Pericarpium), Mi Chao Juhong (Citri Reticulatae Pericarpium).
Mr. Ye: Wind-Warmth entered the Lung. Lung Qi is not unobstructed, and Heat gradually stagnates internally. Symptoms include tongue coating changes, head distension, cough, skin eruption, mental restlessness (Ao Nu), and epigastric痞满 (fullness). This is still due to Qi failing to expand smoothly, with pathogens attempting to bind and obstruct. The patient has chronic Phlegm-Fluids and does not desire water. Propose using the formula principle of Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) combined with Liangge San (Cooling the Diaphragm Powder).
Chuzhi Pi (Gardeniae Pericarpium), Douchi (Sojae Preparata), Xingren (Armeniacae Semen), Huangqin (Scutellariae Radix), Gualoupi (Trichosanthis Pericarpium), Zhishi Zhi (Aurantii Fructus Immaturus Juice).
Mr. Guo: Wind-Warmth entered the Lung. Qi refuses to descend. The body feels cold while internal heat exists, along with chest fullness. These are all signs of Qi stagnation. Use acrid-cool herbs assisted by slightly bitter ones, targeting the Hand-Taiyin Lung Meridian.
Hei Shanzhi (Gardeniae Fructus), Xiangchi (Sojae Preparata), Xingren (Armeniacae Semen), Sangye (Mori Folium), Gualoupi (Trichosanthis Pericarpium), Yujin (Curcumae Radix).
Mr. Qin (63 years old): Constitutionally deficient in Blood. Wind-Warmth was contracted from above. Nourishing and clearing methods were ineffective because the Qi level is dry. Propose clearing the upper jiao. (Wind-Warmth transforming into Dryness-Heat)
Shigao (Gypsum Fibrosum), Sheng Gancao (Glycyrrhizae Radix et Rhizoma), Bohe (Menthae Haplocalycis Herba), Sangye (Mori Folium), Xingren (Armeniacae Semen), Lianqiao (Forsythiae Fructum).
Second visit: Follow the previous prescription, remove Lianqiao (Forsythiae Fructum) and Bohe (Menthae Haplocalycis Herba); add Chen Loupi (Trichosanthis Pericarpium), Yujin (Curcumae Radix), and Zhipi (Gardeniae Pericarpium).
Certain patient: Wind-Fire pathogens stagnate upward, causing tubercles behind the ears and orbital pain. (Shaoyang Wind-Fire)
Bohe (Menthae Haplocalycis Herba), Niubangzi (Arctii Fructus), Qianhu (Peucedani Radix), Xiangbei (Fritillariae Thunbergii Bulbus), Lianqiao (Forsythiae Fructum), Hei Zhipi (Gardeniae Pericarpium), Chishao (Paeoniae Radix Rubra), Sheng Gancao (Glycyrrhizae Radix et Rhizoma).
Certain patient: Wind-Warmth-Heat lies latent, further seizing Yin. Symptoms are lighter during the day and heavier at night, with restlessness and agitation. (Wind-Warmth injuring Yin)
Shengdi (Rehmanniae Radix), Ejiao (Asini Corii Colla), Maidong (Ophiopogonis Radix), Baishao (Paeoniae Radix Alba), Zhicao (Glycyrrhizae Radix et Rhizoma preparata), Zhejiang (Saccharum Indicum).
Mr. Ma (35 years old): After being scorched by Wind-Warmth-Heat, body fluids have not yet recovered. the Maiyang (Yangming) meridians are weak. There is bone soreness and back discomfort. Treat with harmonizing and tonifying methods.
Sheng Huangqi (Astragali Radix), Xian Shengdi (Rehmanniae Radix recens), Beishashen (Glehniae Radix), Yuzhu (Polygonati Odorati Rhizoma), Maidong (Ophiopogonis Radix), Guishen (Angelicae Sinensis Radix), Honey pills.
Certain patient: Yin deficiency with Damp-Wind. Qi ascends from the left. Use Guizhi Tang (Cinnamon Twig Decoction) plus Huafen (Trichosanthis Radix) and Xingren (Armeniacae Semen).
Wind is the Yang Qi of Heaven; Warmth is the pathogen that transforms into Heat. The two Yangs scorch and burn, first injuring the Upper Jiao. All varying manifestations do not exceed the Hand-Taiyin (Lung), Hand-Shaoyin (Heart), and Hand-Jueyin (Pericardium) meridians as the source of disease.
Head distension, sweating, body heat, and cough must appear together. Administer a mild acrid-cool agent, prioritizing clearing and resolving. Strictly avoid acrid-warm dispersants that seize and consume clear fluids. If the Taiyin Lung loses its function of descent and transformation, rescue measures require Saccharum Indicum, Ludgen (Phragmitis Rhizoma), Yuzhu (Polygonati Odorati Rhizoma), and Maidong (Ophiopogonis Radix). Bitter-cold sinking agents damage the Stomach appetite, causing the Yangming to lose its orderly governance; rescue measures require Fumai Tang (Restore the Pulse Decoction) or Jianzhong Tang (Establish the Middle Decoction). Generally, this disease, if changing rapidly, becomes Spasm and Convulsion (Jingjue); if changing slowly, it becomes Deficiency Consumption (Xulao). Therefore, the governing formulas always prioritize Sweet Herbs. Whether combining Cold or Warmth depends on the practitioner's flexibility. (Commentary by Shao Xinpu)
