Cases of hemicephalia (hemianopia/headache) caused by phlegm are common. The etiology includes heat, wind, and blood deficiency. Pain on the left side is attributed to wind, treated with Jingjie (Schizonepetae Herba) and Bohe (Menthae Haplocalycis Herba); if attributed to blood deficiency, use Chuanxiong (Chuanxiong Rhizoma) and Danggui (Angelicae Sinensis Radix). Pain on the right side is attributed to phlegm, treated with Cangzhu (Atractylodis Rhizoma) and Banxia (Pinelliae Rhizoma); if attributed to heat, Jiuhuangqin (Scutellariae Radix processed with wine) is the primary herb. Additionally, for damp-phlegm, Chuanxiong, Nanting (Arisaematis Rhizoma), and Cangzhu may be used. For hemicephalia on the left side attributed to wind, Jingjie and Bohe are employed. These two herbs constitute the principal medicinals. Formulas must adhere to the principles of Jun-Chen-Zuo-Shi (Monarch-Minister-Assistant-Courier). The herb that treats the main disease serves as the Monarch and is used in the largest quantity; the Minister follows, and the Assistant-Courier is next. One must observe the accompanying symptoms to determine the Assistant-Courier herbs. If there is phlegm, Erchen Tang (Two-Aged Decoction) should be used to treat phlegm as an Assistant-Courier; other conditions follow this pattern. Furthermore, one must carefully discern the condition of the disease, relying on flexible application, making additions and subtractions as needed, and not adhering rigidly to fixed formulas.
Formula 1:
Jiu Pian Qin (Scutellariae Radix processed with wine slices, 1 liang), Cangzhu (Atractylodis Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix) (each 5 qian), Xixin (Asari Radix et Rhizoma, 2 qian), Cang'erzi (Xanthii Fructus, 3 qian).
Grind the above into fine powder. Take 3 qian per dose, mix evenly with a large slice of Shengjiang (Zingiberis Rhizoma Recens), and take with tea infusion.
Formula 2:
Jiu Pian Qin (Scutellariae Radix processed with wine slices, 5 qian), Cangzhu (Atractylodis Rhizoma, 2.5 qian), Qianghuo, Cang'erzi, Chuanxiong, Sheng Gancao (Glycyrrhizae Radix et Rhizoma), Jiu Huanglian (Coptidis Rhizoma processed with wine) (each 1.5 qian), Banxia Qu (Pinelliae Fermentatum, stir-fried, 3.5 qian).
Grind the above into fine powder. Administration is the same as above.
Formula 3:
Jiu Pian Qin (Scutellariae Radix processed with wine slices, 1 liang), Cangzhu, Qianghuo, Chuanxiong (each 5 qian), Cang'erzi, Xixin (each 3 qian).
Grind the above into fine powder. Administration is the same as above.
Formula 4: Treats headwind caused by damp-phlegm.
Pian Qin (Scutellariae Radix, wine-fried, 3 qian), Cangzhu (Atractylodis Rhizoma, wine-fried, 1 liang), Chuanxiong, Xixin (each 2 qian), Gancao (Glycyrrhizae Radix et Rhizoma, 1 qian).
Grind the above into fine powder. Administration is the same as above.
Snuffing Powder for Thin Individuals:
Ruan Shigao (Gypsum Fibrosum), Puxiao (Natrii Sulfas) (each 5 qian), Naonao (Borneolum Syntheticum), Jingjie, Tanxiangpi (Santal Albi Cortex), Bohe (each 1 qian), Baizhi (Angelicae Dahuricae Radix), Xixin (each 2 qian).
Grind the above into fine powder and insufflate into the nose.
Formula for Itchy Head and Yellowish Dandruff:
Use Dahuang (Rhei Radix et Rhizoma) soaked in Huangjiu (Chinese Rice Wine) and stir-fried, ground into powder, taken with tea mixture.
Yili Jin Snuffing Formula: Treats hemicephalia.
Bibo (Long Pepper Fruit, amount unspecified, finely ground, mixed evenly with Zhudanshui (Porcine Bile), placed back into the gallbladder, and dried in the shade), Ben (Ligustici Rhizoma), Xuanshusuo (Corydalis Rhizoma), Baizhi, Chuanxiong (each 1 liang), Qingdai (Indigo Naturalis, 2 liang).
Grind the above into fine powder, add 1.5 liang of the prepared Bibo powder, and form pills using Wugen Shui (water collected from rain or dew). Use one pill at a time, dissolve it in Changliushui (flowing water), and instill into the nose. Hold two or three copper coins in the mouth to induce salivation.
Formula for Treating Headwind:
Wutou Jian (Aconiti Lateralis Radix Preparata tip, 7 pieces), Jingjie, Fangfeng, Gancao, Manjingzi (Viticis Semen), Taixiong (Chuanxiong), Jiegeng (Platycodonis Radix), Mahuang (Ephedrae Herba).
Grind the above into fine powder. Take with tea mixture.
Case: A patient with headwind causing nasal congestion.
Nanting (Arisaematis Rhizoma), Cangzhu, Jiu Qin (Scutellariae Radix processed with wine), Xinti (Magnoliae Flos), Chuanxiong.
Grind the above into fine powder. Take with tea mixture.
[Appendix] Treatments for headwind often use hot-natured medicines. Occasionally, patients have concurrent heat and cannot tolerate hot agents; in such cases, Xiaofeng San (Wind-Expelling Powder) or Chatiao San (Tea-Mixed Powder) should be taken. When headwind attacks, if the tendons behind the vertex and on both sides are tight and pulling with pain, assess whether cold or deficiency is involved; Sanwuqi San (Three-Five Seven Powder) is appropriate. For headwind, making pillows with Chujuhua (Chrysanthemi Flos) harvested in the ninth month is most beneficial. *Suwen* states that headwind originates from wind-cold invading the brain marrow. *Benshi Fang* discusses that women suffer from headwind in half of all cases, perhaps because women lack head coverings to ward off wind and cold. Men occasionally suffer from it. For chronic cases lasting years, moxibustion on Xinhui (GV22), Baihui (GV20), Qanding (GV21), and Shangxing (GV23) acupoints can lead to recovery.
