Heart pain is actually epigastric pain. Although it may last for many days without eating, it is not fatal. If one eats immediately when the pain stops, the pain returns. One must take medicine three times before eating. In severe pain, the pulse is likely deep and hidden (Fu); use warming herbs like Fuzi (Aconiti Lateralis Radix Preparata), do not use Renshen (Ginseng Radix) or Baizhu (Atractylodis Macrocephalae Rhizoma). All pains should not be treated with Qi-tonifying methods. Generally, heart pain must be distinguished by newness or chronicity. If it is known that the disease was caused by cold exposure or consuming cold foods, at the beginning, use warming dispersing or warming draining herbs. It is said that prolonged illness leads to stagnation (Yu), prolonged stagnation generates heat, and prolonged heat generates fire, as detailed in *Yuan Bing Shi*. If one attempts warming dispersing/draining now, would it not aid fire and worsen the disease? Ancient formulas often use Shanzhizi (Gardeniae Fructus) as a guide for heat-clearing herbs, so that evil qi can be subdued, the disease recedes, healthy qi recovers, and the patient is safe. However, after recovery, if one indulges in taste preferences and does not correct past mistakes, and the disease recurs, blaming the doctor is lamentable! One prescription uses Shanzhizi (Gardeniae Fructus) stir-fried to remove the skin, 15 fruits per dose, decoct strongly, drink one mouthful, add fresh ginger juice to make it spicy, boil slightly, then add 1 qian of Chuanxiong (Ligustici Chuanxiong Rhizoma), which is excellent. For large Shanzhizi, use 7 or 9 fruits, must be stir-fried until black. Generally, for stomach heat causing pain, Shanzhizi (Gardeniae Fructus) is indispensable, assisted by ginger juice, and more Chuanxiong (Ligustici Chuanxiong Rhizoma) to open dispersion. When the disease flares, use Erchen Tang (Two-Aged Herb Decoction) plus Chuanxiong (Ligustici Chuanxiong Rhizoma) and Cangzhu (Atractylodis Rhizoma), double the amount of stir-fried Zhizi (Gardeniae Fructus). For severe pain, add stir-fried Ganjiang (Zingiberis Rhizoma) to follow the pathogenic factor, treating contrary with ten pills, taken with hot spicy ginger soup. For heavy cases, Guizhi (Cinnamomi Ramulus), Mahuang (Ephedrae Herba), and Shijian (Calcium Carbonatum) in equal parts, mixed with ginger juice and steamed biscuit dough to make pills size of paulownia seeds, take 50 pills with hot ginger soup. Another version: Light cases use dispersing methods, such as Mahuang (Ephedrae Herba) and Guizhi (Cinnamomi Ramulus). Heavy cases add Shijian (Calcium Carbonatum), Chuanxiong (Ligustici Chuanxiong Rhizoma), Cangzhu (Atractylodis Rhizoma), and stir-fried Shanzhizi (Gardeniae Fructus) without skin, made into pills. To treat this condition, one must first ask about daily habits. For example, heart pain caused by a habit of eating hot foods, leading to fixed blood stasis causing pain, treat with Tao Ren Cheng Qi Tang (Peach Kernel Magnolia Bark Decoction) to purge. Remember this. For light cases, use Jiuzhi (Allium Tuberosum Juice) and Jiegeng (Platycodonis Radix) to lift Qi, used alongside blood-invigorating herbs. If pressing the painful area with an object stops the pain, it involves deficiency; harmonize with Erchen Tang (Two-Aged Herb Decoction) plus stir-fried Ganjiang (Zingiberis Rhizoma). For worm pain, white spots on the face, red lips, and ability to eat indicate worms; treat by... head down, difficult to treat. First feed meat broth and sugar/honey to guide the worm head upward, then expel with medicine. Lindera root bark, Areca Seed, and Heshen (Vincetoxici Rosae Caulis) juice in summer, or strong decoction in winter, taken with Wanying Wan (Ten-Thousand-Effect Pill) is best. If the pulse is firm and there is no bowel movement, purge. For heart pain, use Zhizi (Gardeniae Fructus) and Jie Yao (Seizing Medicine). If it recurs, previous medicines will fail; use Xuanming Fen (Mirabilite) once to stop immediately. Left hand rapid pulse indicates excess heat, choppy pulse indicates fixed blood stasis; right hand tight/firm pulse indicates phlegm accumulation, wiry/large pulse indicates chronic disease. For dampness in the epigastrium causing pain, use Xiaowei Dan (Minor Stomach Pill) to purge.
Prescription:
Huanglian (Coptidis Rhizoma, stir-fried), Zhizi (Gardeniae Fructus, stir-fried), Wuzhuyu (Evodiae Fructus, washed in water, 5 qian each), Lizhihe (Litchi Semen, burned to retain efficacy, 3 qian), Huashi (Talcum, 5 qian).
Grind into powder, mix with ginger juice to make pills.
Another Prescription:
Zhiziren (Gardeniae Fructus, stir-fried yellow).

Grind into powder, make pills with steamed biscuit dough.

For phlegm, dissolve Mingfan (Alumen) and add to pills the size of a chicken head, swallow one pill with hot ginger water. Qingdai (Indigo Naturalis) also treats heart pain; crush Lan Ye (Isatidis Folium) to extract juice, mix with ginger juice and take. If leaves are unavailable, use a small bottle of water, place Lan'an (Indigo preparation) on the blade of a knife, heat red in fire, quench in water, and drink.
For spleen pain, use Haifen (Sea Shell Powder), assisted by Xiangfu (Cyperi Rhizoma) powder, decoct Chuanxiong (Ligustici Chuanxiong Rhizoma), Zhizi (Gardeniae Fructus), and ginger juice into a spicy soup, and mix for administration is best. Another prescription treats spleen pain with Qi excess.
