Dry Huoluan (a critical condition characterized by abdominal colic, inability to vomit or defecate) is a fatal syndrome. It presents with restlessness and discomfort, panting, abdominal distension, and the inability to vomit or have a bowel movement.
Xueyu (Blood Stasis) refers to blood coagulating in the Lower Jiao (Lower Burner) and failing to circulate.
This occurs when pathogenic factors from Taiyang disease (Taiyang Syndrome) follow the meridians into the interior, forming blood stasis where blood is constrained by heat.
If Taiyang disease persists for seven to eight days with exterior symptoms still present, and the pulse is faint and deep, improper use of purgatives may lead to Jie Xiong (Chest Binding). If the patient exhibits manic-like behavior, it indicates heat in the Lower Jiao; the lower abdomen should be hard and full. If urination is normal and free, it is Xueyu (Blood Stasis), treated primarily with Didang Tang (Madder, Persimmon Seed, and Cinnamon Twig Decoction). If urination is difficult, it is not blood stasis but rather internal congealing of body fluids.
Furthermore, in Yangming disease (Yangming Syndrome), if the patient suffers from forgetfulness, and although stool is hard, its color must be black, this is also Xueyu (Blood Stasis). Forgetfulness indicates blood stasis, which is a more severe condition than previously described. For mild cases, Taoren Chengqi Tang (Peach Kernel and Rhubarb Decoction) is used; for severe cases, Didang Wan (Madder, Persimmon Seed, and Cinnamon Pill) is used to purge.
Similarly, if a patient has neither exterior nor typical interior symptoms but has had fever for seven to eight days, even if the pulse is floating and rapid, purgatives may be used. Suppose purgatives have already been administered, yet the rapid pulse persists: a floating pulse indicates Qi injury; after purgation, a floating pulse means heat in Ying (Nutritive) level has cleared while heat in Wei (Defensive) level remains. A rapid pulse indicates Blood injury; after purgation, a rapid pulse means heat in Wei (Defensive) level has cleared while heat in Ying (Nutritive) level remains. Combined heat consumes food leading to hunger, whereas pathogenic heat fails to digest grain. By the sixth or seventh day, if there is no bowel movement, it is blood stasis, treated with Didang Tang (Madder, Persimmon Seed, and Cinnamon Twig Decoction).
In all cases of treating Shanghan (Cold Damage), first observe the eyes, then inspect the mouth and tongue, and finally palpate from the epigastrium down to the lower abdomen with the hand. If hardness and fullness are felt, carefully examine and inquire about the condition before treating.
Laofu (Relapse due to Overexertion) refers to a relapse occurring after recovery when Qi and Blood have not yet stabilized, residual heat remains, and physical exertion stirs the heat, causing it to re-activate in the meridians.
The pulse should be floating, rapid, and firm. If fever recurs due to residual heat not yet eliminated, it is not Laofu (Relapse due to Overexertion).
The treatment method differs from the sequential approach of Shanghan (Cold Damage); purgatives can be used rapidly. For Laofu (Relapse due to Overexertion) with high fever after major illness, Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) is primary; add Dahuang (Rhubarb) if there is retained food. It is also stated that for ear heat caused by labor effort, Chaihu Biejia San (Bupleurum and Turtle Shell Powder) should be used to resolve it gently.
For fever caused by overeating, resolving methods (digestives) should be used.
It is also stated that for fever recurring after Shanghan (Cold Damage) recovery, Xiaochaihu Tang (Minor Bupleurum Decoction) is primary. If the pulse is floating, use sweating methods; if deep and solid, use purgative methods.
Additionally, Maidong Tang (Ophiopogon Decoction) treats Laofu (Relapse due to Overexertion). Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) treats Shifu (Relapse due to Dietary Indiscretion).
