Pulse at the Cun Kou (Cun position) presenting as Xian (Wiry) and Jin (Tight) indicates Han She (Cold Hernia, a condition of pain in the lower abdomen radiating to the testicles caused by cold pathogen stagnation in the Liver meridian). A Wiry pulse indicates that Wei Qi (Defensive Qi) is not circulating; when it does not circulate, aversion to cold occurs. A Chi (Slow) and Huan (Relaxed) pulse at the Cun Kou also indicates Han She. Slow pulse indicates Cold, Relaxed pulse indicates Qi; the struggle between Cold and Qi causes pain. A Chen (Deep), Jin (Tight), and Huo Da (Hollow and Large) pulse indicates Deficiency. Hua (Slippery) pulse indicates She disease. Ji (Rapid/Tense) pulse indicates She disease. Bo (Beating/Forceful) pulse indicates She disease. By observing which location the pulse appears in, one can determine which Zang organ is diseased. Generally, the disease lies entirely in the Jue Yin Liver Meridian. Some cases arise from Shi Re (Damp-Heat) in the meridians, suppressed for a long time, combined with external Cold; the Damp-Heat is constrained and causes pain. Or, excessive labor causes Fire to arise in the sinews; drunkenness and overeating cause Fire to arise in the Stomach; sexual excess causes Fire to arise in the Kidney; extreme anger causes Fire to arise in this meridian (Liver). When Fire is severely constrained, Dampness becomes abundant, turbid Qi condenses, enters the blood vessels, flows into the Liver Meridian, and is bound by Cold, thus causing severe pain. Some cases arise from Tan Yin (Phlegm-Rheum) and food retention flowing into the Jue Yin, gathering to form nodules. Some cases arise from Phlegm-Blood stagnation in this meridian. Some cases arise from deficiency or Cold in this meridian. However, the Liver Meridian converges with the Chong, Ren, and Du meridians at the genitalia; injury by Cold causes retraction of the penis, while injury by Heat causes flaccidity and inability to retract, because the nature of Wood (Liver) is swift and urgent.

Zhang's Discussion on Seven Types of She Disease
Han She (Cold Hernia) Caused by cold, water exposure, damp living conditions, and excessive intercourse, leading to cold, hard scrotum like stone, impotence, or pain radiating to testicles; treat with warming agents.
Xiao She (Small Hernia) Caused by intercourse after drunkenness and sweating, encountering wind-cold-dampness, accumulating in scrotum causing pain like crystal, scratching yields yellow fluid, water sound upon pressing lower abdomen; treat by draining water.
Jin She (Sinew Hernia) Caused by sexual excess and evil arts, penile swelling, or ulceration with pus, or pain with tenesmus and sinew contraction, or flaccidity without retraction, or white discharge like semen, or penile pain, itching when pain is extreme; treat by descending Fire.
Xue She (Blood Hernia) Caused by blood and qi overflow during intercourse, seeping into the bladder sac, forming abscess/pus, named Bian Yong (Perineal Abscess); treat by harmonizing blood.
Qi She (Qi Hernia) Distension due to anger, dispersing when anger subsides; treat with hernia-Qi resolving methods.
Gu She (Solitary Hernia) Shape like an inverted tile, enters lower abdomen when lying down, emerges from scrotum when standing/walking; treat with agents that drain qi and flow through meridians.
Shi She (Damp Hernia) Caused by dampness, heavy as a sheng/dou measure, no itching or pain; treat with damp-draining herbs.
Three Causes, Four Types
Qi She (Qi Hernia) Due to seven emotions causing visceral qi to sink, scrotal swelling and acute pain, easy to treat.
Shui She (Water Hernia) Similar to Shi She (Damp Hernia).
Chang She (Intestine Hernia) Due to excessive sexual activity, deficiency and cold of Yuan Zang (Primordial Viscera), intestinal mesentery fails to hold, intestine drops into scrotum, moving up and down不定ly, difficult to treat.
Luan She (Testicle Hernia) Due to labor and horse riding, causing testicular swelling, or asymmetry in size, moving up and down不定ly, also difficult to treat.
Penile Priapism (due to Damp-Heat): Use Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Huang Lian (Coptis Rhizome); if masses are present, add Qing Pi (Green Citrus Peel); externally, apply Si Gua Zhi (Loofah Juice) mixed with Wu Bei Zi Mo (Galla Chinensis Powder).
