Postpartum speech may become inverted, or manifest as delirious speech and manic muttering, or even seeing spirits and ghosts. The etiology is diverse; careful differentiation of syndromes (Bian Zheng) is required before administering medication. First, postpartum Heart deficiency with stagnant blood (Blood Stasis) accumulating and disturbing the Heart leads to manic muttering; this should be treated according to the section on "Suddenly Seeing Spirits." Second, postpartum Zang-Fu deficiency causing Heart spirit fright palpitations, disordered speech without self-awareness, and mental unrest should be addressed under the section on "Fright Palpitations." Third, pre-existing wind-toxin combined with postpartum Heart Qi deficiency leading to rigid back and neck, erratic singing/crying/anger/laughter, and disordered speech should be treated as Wind Spasm. Fourth, postpartum stagnant blood confusing the Heart channel resulting in epilepsy or mania, irregular speech, or dizziness/fainting should be treated as Blood Syncope. Fifth, postpartum contraction of Wind-Cold causing sudden cessation of Lochia, alternating chills and fever resembling malaria, clear consciousness during the day but delirious speech resembling seeing ghosts at night, should be treated as Heat Entering the Blood Chamber. Additionally, postpartum fright causing stagnant blood rushing to the Heart, leading to coma and mania as if possessed by spirits, is suitable for Da Sheng Ze Lan San, or high-quality Cinnabar ground into fine powder, taken with Suan Zao Ren Tang, one Qian, which yields immediate results. Xue Lizhai stated: For the aforementioned conditions, the primary approach should be to consolidate Stomach Qi, supplemented by herbs targeting specific symptoms; attacking Phlegm exclusively would be a mistake.
