Gu Zhang (Drum Distension) presents with a hard exterior and hollow interior, resembling a drum. The treatment principle should involve tonifying Spleen Earth, while nourishing Lung Metal to control Liver Wood, thereby preventing the Spleen from being invaded by pathogenic factors (Thief Evil). Nourish Kidney Water to control Ministerial Fire, allowing the Lung to receive instructions for clear transformation. Avoid salty flavors to prevent assisting evil; cut off delusions to preserve Mother Qi (Pre-natal Qi). Do not use purgatives. It is advisable to stay away from music and entertainment, avoid rich flavors, and guard against violent anger; thus, peace will be achieved. A pulse that is floating, large, slippery, and solid indicates survival; a pulse that is soggy, small, deficient, and faint indicates death. Fullness with a wiry pulse indicates the Spleen is controlled by the Liver. A surging and rapid pulse indicates heat-type distension; a slow and faint pulse indicates cold-type distension. A floating pulse indicates deficiency-type fullness; a tight pulse indicates excess in the middle jiao. A floating pulse suggests the condition is treatable; a deficient pulse indicates critical danger.
