Women whose menstruation is delayed due to blood deficiency should use Siwu Tang (Four Substance Decoction) added with Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). If phlegm-dampness is present, add Nanxing (Arisaematis Rhizoma Preparatum), Banxia (Pinelliae Rhizoma), and Chenpi (Citri Reticulatae Pericarpium). Menstruation arriving early indicates blood-heat; treat with Siwu Tang added with Huanglian (Coptidis Rhizome). Delayed menstruation with dark purple blood and clots also indicates blood-heat, accompanied by pain; treat with Xiangfu (Cyperi Rhizoma) and Huanglian. Delayed menstruation with pale blood color indicates excessive phlegm; treat with Erchen Tang (Two Aged Substances Decoction) added with Xiong (Ligustici Chuanxiong Rhizoma) and Gui (Angelicae Sinensis Radix). Delayed menstruation due to blood deficiency requires tonifying blood; use Siwu Tang added with Huangqi (Astragali Membranacei Radix), Chenpi, and Shengma (Cimicifugae Rhizome). Early menstruation indicates heat in both qi and blood; treat with herbs that cool blood and qi. Irregular menstruation with pale, watery blood requires tonifying qi and blood; use Renshen, Huangqi, Xiong, Gui, Xiangfu, and Shaoyao (Paeoniae Radix Alba). Add Jiaozhu (Colla Corii Asini Prepared) and Aiye (Artemisiae Argyi Folium) for abdominal pain. Pain after menstruation indicates deficiency with internal heat. Pain before menstruation indicates excess blood and stagnant qi; treat with Siwu Tang added with Taoren (Persicae Semen), Huanglian, and Xiangfu. Dark purple clots indicate heat; treat with Siwu Tang added with Huanglian and Chaihu (Bupleuri Radix). Lower back and abdominal pain during menstruation indicates stagnation and blood stasis; treat with Siwu Tang added with Taoren, Honghua (Carthami Flos), Ezhu (Curcumae Rhizoma), Xuanshen (Cyperi Rhizoma - *Note: Text says 玄胡 which is Yanhusuo*, Xiangfu, and Muxiang (Aucklandiae Radix). For fever, add Chaihu and Huangqin (Scutellariae Radix). Excessive phlegm occupying the position of the Blood Sea leads to heavy menstruation and gradual dimness of vision. In obese individuals, this condition is treated with pills made from Xiong, Cangzhu (Atractylodis Rhizoma), Nanxing, and Xiangfu. Obese individuals with heavy menstruation before the expected date have excessive phlegm, blood deficiency, and heat; use the aforementioned pill formula added with Huanglian and Baizhu. Amenorrhea due to exhaustion of internal blood is treated with Siwu Tang added with Taoren and Honghua.
To treat deficiency and damage to the Chong and Ren meridians, irregular menstruation, and pain in the umbilical and abdominal regions, modify this formula into pills. The base formula consists of Danggui (Angelicae Sinensis Radix), Xiong, Shaoyao, and Shudihuang (Rehmanniae Radix Preparata) in equal parts. Decoct these in water and take on an empty stomach. If menstruation is scanty and gradually ceases, accompanied by vexing pain in the hands and feet, emaciation, tidal fever, and a faint, rapid pulse, remove Dihuang (Rehmanniae Radix) and Xiong, and add Zelan Ye (Leonuri Herba) in triple quantity and Gancao (Glycyrrhizae Radix et Rhizoma) at 5 fen. For excessive menstruation, remove Shudihuang and add Shengdihuang (Rehmanniae Radix), or add Huangqin and Baizhu. For fever, rapid pulse, and dizziness during menstruation, add Chaihu and Huangqin to the base formula. For scanty menstruation with distension or pain, add Xuanshen (Yanhusuo, Corydalis Rhizoma), Moyao (Myrrha), and Baizhi (Angelicae Dahuricae Radix). For irregular menstruation with heart and abdominal pain, administer only Xiong and Gui. For frequent menstruation due to qi rushing into the meridians and pain below the umbilicus, add Shaoyao. For umbilical and abdominal pain during menstruation, add Xuanshen, Binglang (Arecae Semen), Koushen (Meliae Cortex), and reduce Muxiang. For scanty and sluggish menstruation, add Kuihua (Sunflower Petal) and Honghua. For menstruation that has just started and stopped, or for alternating chills and fever, first administer Xiao Chaihu Tang (Minor Bupleurum Decoction), followed by combining it with Siwu Tang. For pain after menstruation indicating deficiency of both qi and blood, administer the base formula together with Sijunzi Tang (Four Gentlemen Decoction).
