The *Suwen* (Basic Questions) records five types of heat diseases: Patients with Liver Heat Disease first exhibit redness on the left cheek; patients with Lung Heat Disease first exhibit redness on the right cheek; patients with Heart Heat Disease first exhibit redness on the forehead (Yan, meaning the forehead); patients with Kidney Heat Disease first exhibit redness on the chin (Yi, meaning the lower part of the cheek/jaw); patients with Spleen Heat Disease first exhibit redness on the nose. (In Five Elements theory, Earth governs the center.) Even if the disease has not yet fully manifested, puncturing upon seeing redness is called "treating before disease arises" (Zhi Wei Bing).
Patients with Liver Heat Disease first present with yellow urine, abdominal pain, lethargy, and body fever. When pathogenic heat contends with healthy Qi (Re Zheng), they may manifest delirious speech and惊厥 (convulsions), distension and pain in the hypochondrium, restlessness of the hands and feet, and inability to lie down comfortably. The condition worsens on Geng-Xin days, resolves with profuse sweating on Jia-Yi days, and death occurs due to Qi reversal on Geng-Xin days. Treatment involves puncturing the Foot-Jueyin (Liver) and Foot-Shaoyang (Gallbladder) meridians. If Qi reverses, it leads to headache, as the meridian Qi rushes upward to the head.
Patients with Heart Heat Disease first present with dissatisfaction or depression, developing fever after several days. When pathogenic heat contends with healthy Qi, they experience sudden heart pain, oppression, a tendency to vomit, headache, facial redness, and absence of sweat. The condition worsens on Ren-Gui days, resolves with profuse sweating on Bing-Ding days, and death occurs due to Qi reversal on Ren-Gui days. Treatment involves puncturing the Hand-Shaoyin (Heart) and Hand-Taiyang (Small Intestine) meridians.
Patients with Spleen Heat Disease first present with heavy head, cheek pain, irritability, bluish-green complexion, nausea, and body fever. When pathogenic heat contends with healthy Qi, they experience lower back pain, inability to bend forward or backward, abdominal fullness, diarrhea, and pain in both mandibles. The condition worsens on Jia-Yi days, resolves with profuse sweating on Wu-Ji days, and death occurs due to Qi reversal on Jia-Yi days. Treatment involves puncturing the Foot-Taiyin (Spleen) and Foot-Yangming (Stomach) meridians.
Patients with Lung Heat Disease first present with chills, erection of fine hairs, aversion to wind and cold, yellow tongue coating, and body fever. When pathogenic heat contends with healthy Qi, they experience wheezing and coughing, pain wandering across the chest, epigastrium, and back, difficulty taking deep breaths, unbearable headache, sweating, and aversion to cold. The condition worsens on Bing-Ding days, resolves with profuse sweating on Geng-Xin days, and death occurs due to Qi reversal on Bing-Ding days. Treatment involves puncturing the Hand-Taiyin (Lung) and Hand-Yangming (Large Intestine) meridians, releasing blood drops the size of a bean, which stops the symptoms immediately.
Patients with Kidney Heat Disease first present with soreness and pain in the lower back, bitter dry mouth with frequent desire to drink, and body fever. When pathogenic heat contends with healthy Qi, they experience neck pain and a sensation of internal turbulence. The condition worsens on Wu-Ji days, resolves with profuse sweating on Ren-Gui days, and death occurs due to Qi reversal on Wu-Ji days. Treatment involves puncturing the Foot-Shaoyin (Kidney) and Foot-Taiyang (Bladder) meridians. All such sweating occurs on the days when the affected organ's element is overcome by or overcomes others (days of resolution).
Pang Anshi stated: Among these five heat diseases, the Liver and Kidney organs have adverse conditions (Qi reversal), while the Heart, Spleen, and Lung organs do not have adverse conditions. For all five heat diseases, encountering an overcoming day within two to three days of onset is not considered adverse, but one must beware of encountering an overcoming day on the fifth or sixth day.
