Pulse is floating and tight; according to the method, body pain should occur, and sweating (Han Jie) should be used to resolve it. If the Chi (尺) pulse is slow, it is due to deficiency of Ying Qi (Nutritive Qi) and blood insufficiency. (In the previous Yangming disease section, a slow pulse with profuse sweating and slight aversion to cold indicated Gui Zhi Tang [Cinnamon Twig Decoction], without attributing it to Ying deficiency, as that involved a large and slow Chi pulse. Here, if the pulse is soft, tight, and slow, sweating is contraindicated; Xiao Jian Zhong Tang [Minor Construct the Middle Decoction] is appropriate.)
Xiao Jian Zhong Tang (Minor Construct the Middle Decoction)
Gui Zhi (Ramulus Cinnamomi, three fen), Sheng Jiang (Rhizoma Zingiberis Recens, three fen), Bai Shao (Radix Paeoniae Alba, one liang half), Gan Cao (Radix Glycyrrhizae, half liang), Da Zao (Fructus Jujubae, six pieces), Yi Tang (Maltose, two he half).
Crush the herbs, add two sheng and a half of water, decoct until nine he remain, remove the dregs, then add Yi Tang and heat until dissolved. Take warm, one cup per dose, two to three times daily. If the Chi pulse remains slow, prepare another dose adding Ren Shen (Radix Ginseng, half liang) for best results. For those with prior mild diarrhea or vomiting, do not use Yi Tang. It also treats Shang Han (Cold Damage) in its first or second day, presenting with palpitations and irritability in the chest, or body pain and deep-slow pulse after sweating. Furthermore, it treats Shang Han where the Yang pulse is choppy and the Yin pulse is string-taut, typically indicating acute abdominal pain; administer Xiao Jian Zhong Tang first; if not cured, give Chai Hu Tang [Bupleurum Decoction]. (This formula is located in the Harmonizing Exterior Syndrome section.)
Pang stated: Generally, a tight pulse indicates no sweating; only when the pulse is soft and tight does spontaneous sweating occur. Do not mistakenly use Gui Zhi Tang; use Jian Zhong Tang instead.
A weak and soggy pulse indicates that sweating is contraindicated; if sweating is induced, it leads to cold extremities (Jue), irritability, and insomnia.
If there are moving Qi sensations above, below, left, or right in the heart and abdomen, sweating is contraindicated.
Pang stated: Any pulse that is rapid, forceful, yet weak and minute indicates that sweating is contraindicated. (All the above cases are suitable for Jian Zhong Tang. If irritability is present, Zhu Ye Tang [Bamboo Leaf Decoction] is appropriate.)
Zhu Ye Tang treats deficiency irritability, where the condition resembles Shang Han, with fever and irritability, but without headache or body ache, and a non-rapid pulse. It also treats damp-heat in the middle jiao causing thirst, vomiting, and reflux with a slippery-rapid pulse, as well as post-Shang Han weakness, emaciation, shortness of breath, and rebellious Qi desiring to vomit. All these conditions are suitable for this decoction.
Dan Zhu Ye (Herba Lophatheri, half a handful), Shi Gao (Gypsum Fibrosum, four liang), Ban Xia (Rhizoma Pinelliae, three fen), Ren Shen (Radix Ginseng, half liang), Gan Cao (Radix Glycyrrhizae, half liang), Mai Men Dong (Radix Ophiopogonis, two liang), Jing Mi (Semen Oryzae Sativum, one he, washed).
Finely chop, add five sheng of water, decoct until two sheng and a half remain, remove dregs when rice is cooked. Take warm, one cup per dose, three times daily and twice at night. If vomiting occurs, add Sheng Jiang (Rhizoma Zingiberis Recens, one liang half); if no vomiting, do not add.
For deficiency irritability or vomiting, with string-taut, fine, hollow, or slow pulses, slightly cold hands and feet, and chest fullness, Ju Pi Tang [Tangerine Peel Decoction] is the primary treatment; it also treats sudden irritability with diarrhea. (This formula is in the Taiyin Syndrome section.)
Si Ni syndrome (Four Extremities Cold) is contraindicated for sweating; inducing sweating leads to hoarseness, tongue atrophy unable to extend, disordered speech, and vertigo, endangering life.
Throat obstruction is contraindicated for sweating; inducing sweating leads to hematemesis. For slight and nearly exhausted Qi and Blood, with inability to warm the hands and feet, Gan Jiang Gan Cao Tang [Dried Ginger and Licorice Decoction] is the primary treatment. (This formula is in the Taiyin Syndrome section.)
Patients with Lin syndrome (Strangury) are contraindicated for sweating; inducing sweating will inevitably lead to hematuria.
Patients with Chuang (sores/ulcers) are contraindicated for sweating even if they have body pain; inducing sweating leads to exhaustion. (Because patients with abscesses and sores have already lost excessive pus and blood.)
Moving Qi on the right side is contraindicated for sweating; inducing sweating leads to epistaxis and thirst, severe mental irritability, and vomiting water upon drinking. ("Moving" refers to empty Qi movement in the heart and abdomen; if mistaken sweating causes these symptoms, first administer San Fu Wu Ling San [Five苓 Powder, three doses], found in the Water Retention section. Then administer Zhu Ye Tang, found earlier.)
Moving Qi on the left side is contraindicated for sweating; inducing sweating leads to vertigo, and sweating causes muscle twitching and tremors. This is an adverse reaction and difficult to treat. (First administer Fang Feng Bai Zhu San [Saposhnikovia and Atractylodes Powder], then Jian Zhong Tang, found earlier.)
Fang Feng Bai Zhu San [Sapothnikovia and Atractylodes Powder]
Fang Feng (Radix Saposhnikoviae, one liang), Mu Li Fen (Concha Ostreae powder, half liang), Bai Zhu (Rhizoma Atractylodis Macrocephalae, three fen).
Grind into fine powder, take two qian mixed with warm millet drink, two to three times daily. After sweating, continue with Jian Zhong Tang.
Moving Qi on the upper side is contraindicated for sweating; inducing sweating leads to Qi rushing upward, centering at the heart. Li Gen Tang [Plum Root Bark Decoction] is the primary treatment.
Ban Xia (Rhizoma Pinelliae, half liang), Gui Zhi (Ramulus Cinnamomi), Dang Gui (Radix Angelicae Sinensis), Bai Shao (Radix Paeoniae Alba), Huang Qin (Radix Scutellariae), Gan Cao (Radix Glycyrrhizae), Ren Shen (Radix Ginseng, each one fen), Fu Ling (Sclerotium Poriae, three fen).
Cut into coarse pieces, take five qian per dose, add two zhan of water, three slices of ginger, and a piece of Gan Li Gen Bai Pi (Cortex Pruni Spinosa Albus, size of a chicken egg yolk). Decoct until eight fen remain. Drink through the mouth, three to five times daily.
Moving Qi on the lower side is contraindicated for sweating; inducing sweating leads to extreme mental irritability, painful bone joints, vertigo, aversion to cold, vomiting after eating, and food failing to descend. First administer Da Ju Pi Tang [Major Tangerine Peel Decoction]; once vomiting stops, then administer Jian Zhong Tang. Da Ju Pi Tang also treats cold hands and feet with vomiting and hiccups.
Ju Pi (Pericarpium Citri Reticulatae, one liang half), Sheng Jiang (Rhizoma Zingiberis Recens, two liang), Da Zao (Fructus Jujubae, twenty-four pieces), Gan Cao (Radix Glycyrrhizae, half liang), Ren Shen (Radix Ginseng, one fen), Zhu Ru (Caulis Bambusae in Taeniam, half liang).
Crush the herbs, add three sheng of water, decoct until one sheng and a half remain, remove dregs. Take warm, one cup, repeat shortly after meals. Forcing sweating when it is not indicated will lead to death due to exhaustion of Jin Ye (Body Fluids).
