For those injured by various external forces, if they are unable to eat or drink, it indicates that fire-toxin has entered the five zang organs (Zang organs: Heart, Liver, Spleen, Lung, Kidney). The patient will desire cold water; immediately administer Qingxin Qudu Yin (Clear Heart and Remove Toxin Decoction). If they can eat normally, the fire-toxin has not penetrated inward. For injuries involving broken skin and flesh, apply Hupo Gao (Amber Plaster) externally and administer Qudu San (Remove Toxin Powder) internally.
For severed ears or noses, quickly dip hair-ash powder into the warm blood at the wound site and apply it to the original position without deviation, followed by external splinting and binding. For severed arms, upper arms and lower legs, or toes, promptly take the severed part while the blood is still warm, align and reattach it, sprinkle Zhixue Dan (Stop Bleeding Pill) externally, apply Jinchuang Yao (Metal Wound Medicine), and administer Tuoli Zhitong San (Support Interior and Stop Pain Powder) internally. If the blood has cooled, the severed parts cannot adhere, resulting in permanent disability.
In cases of collapse from bridges or walls causing fractures of bone joints or shattered skull bones, remove the bone fragments with forceps and suture the wound to prevent future complications. To prevent infection with Tetanus (Po Shāng Fēng), first administer Tuoli San (Support Interior Powder), followed by Jiagu San (Bone Setting Powder). Injuries to both Taiyang acupoints (Ex-HN5, located at the temples) causing coma, inability to eat or speak, are incurable. Extrusion of brain matter is also incurable.
Injuries to the chest, back, and five zang organs, where there is no speech or food intake, may be due to Qi stagnation in the middle jiao; administer Jili San (Auspicious Powder). For body chills and fever, administer Shufeng Liqi Tang (Dispel Wind and Regulate Qi Decoction). Injuries to the soft ribs on both sides, where diet remains normal, administer Jili San. Severe kidney injuries are incurable. Mild cases should have Sunshang Gao (Trauma Plaster) applied and Bushen Hexue Tang (Tonify Kidney and Harmonize Blood Decoction) administered.
Kicks to the lower abdomen causing pain, urinary obstruction, and inability to walk are treatable within three days due to internal blood stasis; administer Xingqi Huoxue Tang (Move Qi and Invigorate Blood Decoction). Pregnant women kicked in the lower abdomen are incurable.
For injuries presenting with purple swelling and unbearable pain, administer Huoxue Zhitong Tang (Invigorate Blood and Stop Pain Decoction), followed by Jili San. If swelling is blue-purple accompanied by chills/fever, constipation/urinary retention, Qi stagnation, restlessness, anorexia, and testicular migration, this indicates internal blood stasis. First administer Shufeng Liqi Tang, then Hupo Wan (Amber Pill). For red swelling with paroxysmal pain, wheezing, coughing, desire to laugh but dysuria, administer Huoxue Zhitong Tang.
Kicks to the Haidi acupoint (Huiyin, CV1, perineum) causing red-purple swelling and unbearable pain require Sunshang Gao applied and Huoxue Zhitong Tang administered, followed by Jili San. If swelling is blue, accompanied by chills/fever, urinary retention, Qi stagnation, abdominal pain, and penile retraction/prolapse, indicating internal blood stasis, apply Sunshang Gao and administer Xingqi Huoxue Tang, followed by Hupo Wan. For anal swelling/pain with bowel/bladder obstruction, fever, poor appetite, and restlessness, administer Shufeng Shunqi Tang, followed by Hupo Wan. If red swelling persists with paroxysmal pain, wheezing, coughing, laughing/crying, and dysuria, first administer Huoxue Zhitong Tang, then Jili San, and finally medicinal wine for recovery. For initial injury with coma, muteness, salivation, throat/nasal wheezing, pale complexion, abdominal Qi movement, and deep/thin pulse, which are treatable: blow Ya Sao Mo (Gleditsia Sinensis Powder) into the nose to induce sneezing, then administer Sha Ren Tang (Amomum Decoction) decocted with Jili San, followed by Huoxue Zhitong Tang and Sunshang Gao.
Knife or axe wounds are classified as Jin Chuang (Metal Wounds). Pale yellow wounds are easy to treat; purple-black wounds are difficult. Coughing, vomiting, and hiccups must be avoided; keeping away from wind is advisable. If wind enters the wound causing edema, excessive tears/runny nose, phlegm surging, trismus, opisthotonos, and Tetanus, it is incurable. Quickly apply Bixiwo Xiang Hui (Ash from Wall Niche Incense) mixed with Moshe (Musk) to the wound and administer Shufeng Liqi Tang. If anger causes wound rupture or lust causes foul-smelling ulceration, use Rushen (Boswellia), Moyao (Myrrh), Dilushi (Frankincense Stone), and Xuexie (Dragon's Blood) powdered for application.
Patients should avoid wind and cold, abstain from sexual activity, endure anger, calm the mind, regulate diet, and avoid chicken, goose, vinegar, eggs, beef, mutton, bamboo shoots, wheat flour, and fried/spicy foods (stimulating foods).
Xu Zhenren stated: When injured by knives or axes with heavy bleeding, thirst occurs, but water should not be given. Drinking hot soup causes blood boiling and hemorrhage. There are eight prohibitions: 1. Avoid anger; 2. Avoid laughter; 3. Avoid loud talking; 4. Avoid physical labor; 5. Avoid wandering thoughts; 6. Avoid eating gruel/soup; 7. Avoid alcohol; 8. Avoid sour and salty foods. If pre-existing conditions coexist with new injuries, medications should address both simultaneously.
Pregnant women should administer Antai Shunqi medicines (Fetus-Anchoring and Qi-Regulating Medicines).
Those beaten with a staff will have purple-black legs; severe cases suffer Qi stagnation attacking the heart leading to death. Immediately administer Tongbian (Child Urine) flushed with wine. Externally apply hot tofu; when the tofu turns black, it indicates drawn-out blood stasis. Change tofu frequently until the color fades. Alternatively, apply hot crushed scallions via moxibustion-like warming. Or apply Dahuang (Rhubarb) powder mixed with child urine, and internally administer Huayu medicines (Blood-Stasis Resolving Medicines).
Those tortured with leg clamps should immediately soak the injured area in a basin of hot child urine. If the urine cools, heat bricks and quench them in the urine to reheat it. Soak until white oil appears on the surface of the urine. Externally apply crushed soap mixed with egg white, bandaging tightly. Alternatively, apply He Shou Wu (Polygonum multiflorum) paste, and internally administer Zhuyu Dingtong medicines (Stasis-Expelling and Pain-Relieving Medicines).
