The "gu" (bone) refers to the bone at the shoulder end, which is the scapula. Its glenoid cavity accommodates the proximal end of the humerus. This area is named "Jianjie" (Shoulder Joint), which is the suture where the shoulder joint and the humerus meet. It is commonly known as "Tunkou" or "Jiantou". Below it, attached to the vertebral column in a wing-like sheet, lies the structure. If this joint dislocates, the arm cannot be raised. Have the patient sit low; one person holds their body while another straightens the arm. Apply tuina manipulation, adjusting the force according to severity, until the tendons relax. One hand grasps the shoulder, pressing against the head of the joint, and applies coordinated traction to reduce it. A sound within the joint indicates successful reduction. Suspend the arm with a cloth bandage under the neck and administer Shujin Huoxue Tang (Relax Tendons and Invigorate Blood Decoction). In cases of upper limb joint dislocation, there must be an audible sound and mobility within the joint, indicating that the joint has been reduced. If there is no sound or mobility, the joint has not been reduced; do not mislead the patient.
The elbow joint is the junction between the proximal and distal segments of the forearm bones, commonly known as "Ebi Gu" (Goose Nose Bone). It articulates superiorly with the humerus, and this joint is referred to as "Quji" (Bent Pool). From the elbow to the wrist, there are two parallel bones: the longer bone below, continuous with the olecranon, is the ulna (arm bone); the shorter, thinner bone above is the radius (turning bone). They overlap and lean against each other, articulating inferiorly with the carpal bones. If this joint dislocates, one hand grasps the head of the joint while the other holds the cubital fossa. First, apply downward traction. An audible sound indicates reduction. Then, flex and rotate the arm, resting it on the shoulder, ensuring the elbow joint sutures align, signifying successful reduction. Administer Shengxue Buisui Tang (Generate Blood and Nourish Marrow Decoction) or Zijin Dan (Purple Gold Pill).
The wrist bones, also known as metacarpals, are the proximal segments of the five fingers, commonly referred to as "Hugu" (Tiger Bone - *note: likely a local colloquialism for the carpal/metacarpal region*). There are six small bones that form the palm, rather than being a single solid bone. Superiorly, they articulate with the distal ends of the two forearm bones. The prominent bone on the lateral side, commonly called "Longgu" (Dragon Bone), allows for flexion and extension, hence the name "wrist". Injury from falling on the palm affects only the wrist. If falling on the fingers causes them to flip and adhere to the forearm, the wrist joint space will open, leading to swelling and pain. Massage both wrists first. One hand stabilizes the joint while the other pulls the fingers and palm, producing a sound and restoring mobility, thereby reducing the joint. Continue massaging the tendons until they are harmonious and smooth. At vital pulse points, when administering Kuanjin San (Loosen Tendons Powder), prevent exposure to cold to avoid soreness. Humans have three segments in each finger; any protruding joints can be straightened and realigned by manual traction, allowing for flexion and extension. Administer Heying Zhitong Tang (Harmonize Ying-Nutrient and Stop Pain Decoction). The method is the same; there is no need to discuss each joint individually.
