Bone setting is the process of bringing broken bones together to restore them to their original position. When a bone is fractured into two parts, depressed, shattered and scattered, or displaced with abnormal protrusions, one must observe the specific condition and situation, and slowly perform reduction maneuvers to reconnect the fracture, elevate the depression, reconstruct the integrity of the shattered fragments, and flatten the protrusions. All these depend on manual techniques. If the skin and flesh are intact but the bone is completely fractured, movement produces a rattling sound; if the bone is damaged but not completely fractured, there is no sound upon movement. If there are small pieces of necrotic bone inside, movement produces a rustling sound, which will inevitably lead to ulceration and pus discharge later. These bones have lost vitality and detached from the muscle tissue; their color must be black, ranging in size from rice grains to finger segments. If not removed thoroughly, they will cause chronic ulceration for years, so it is urgent to remove them completely. If the bone has not yet detached from the muscle, do not forcibly cut it, as this may damage the tendons; wait until it decays and falls off before removing it. Treatment method: First, fumigate and wash with a decoction of Daisheng San (Pain-Relieving Powder). Straighten and align the fractured bone ends, massage them to be flat and smooth as before. Secure tightly with cloth strips, then bind with a rounded wooden board from a cake box, wrap again with cloth strips, and finally embrace the outside with a bamboo splint (Shanli), utilizing its tightness and straightness to fix the bone, following the previous method. Change the binding several times between the second and third months; recovery can be expected within one hundred days. The binding methods for fractures of the arm or forearm, thigh or leg are the same, though treatment distinguishes between upper and lower body parts. Instruments may be used according to the body shape, adapting flexibly as needed. If a tendon is ruptured, it is difficult to reconnect. Because tendons are soft, a complete rupture causes them to retract into the muscle, making technical repair impossible; if the rupture is incomplete, applying tendon-repairing medicine externally and taking Zhuangjin Yangxue Tang (Tendon-Strengthening and Blood-Nourishing Decoction) internally can lead to cure.
(Attachment: Mandibular Dislocation)
The mandible refers to the bone at the temporomandibular joint. Dislocation results in difficulty in eating and speaking, caused by Kidney Deficiency. The condyle is curved like a ring. Pushing it upward toward the zygomatic arch, if the joint makes a clicking sound and the teeth can close properly, it indicates successful reduction. Take Bushen Zhuangjin Tang (Kidney-Nourishing and Tendon-Strengthening Decoction).
Human tendons rely on Qi and Blood for nourishment. Cold causes tendon contraction, while heat causes tendon laxity. Loss of nutrition leads to difficulty in extension and relaxation. Contracting wind-cold may result in mandibular dislocation, where the head cannot turn. Have the patient sit low, use massage techniques to frequently rub and knead, hold the head with one hand and pull the mandible with the other, slowly extending and relaxing until alignment is restored. Taking Shufeng Yangxue Tang (Wind-Dispersing and Blood-Nourishing Decoction) is appropriate.
