During gestation, the infant receives the Qi of Yin and Yang and the Five Elements (Wood, Fire, Earth, Metal, Water) to form the Zang-Fu organs and skeletal structure, nourished by amniotic fluid. When the prenatal Qi is sufficient, birth occurs naturally. At the time of delivery, the infant holds a blood clot in the mouth; upon crying, it is immediately swallowed, causing toxin to lie dormant in the Mingmen (Gate of Life). If triggered by prolonged heat from epidemic pathogens, food stagnation, external contraction of Wind-Cold, or convulsions with fever, sores and rashes may erupt. It is urgent to use a soft cloth-wrapped finger to remove the blood before the infant cries, and to resolve the toxicity using Huanglian (Coptis Rhizome), Douchi (Fermented Soybean), Zhusha (Cinnabar), honey, and Gancao (Licorice Root). Even if smallpox later appears, the condition will be mild. If the infant swallows the blood and immediately presents with abdominal distension, vomiting, shortness of breath, and refusal to nurse, Fuling Wan (Poria Pill) should be used. However, Huanglian is cold in nature; it is suitable for infants inheriting maternal internal heat from rich diet, but unsuitable for those with bland maternal diet and weak prenatal Qi. Although Zhusha can detoxify, its heavy mineral nature may suppress vital energy; it is better to use a small amount of Niuhuang (Calculus Bovis), mixed with honey for the infant to suck. Many people use Xijiao Jiedu Wan (Rhinoceros Horn Detoxification Pill), which may damage the Spleen-Stomach's generating Qi in infants with deficient-cold prenatal Qi, potentially leading to failure to thrive. For infants in critical condition due to difficult labor or exposure to Wind-Cold, the umbilical cord must not be cut immediately. Quickly wrap the infant in heated cotton and hold them close. Place the placenta in fire to roast, and use a large paper wick to cauterize the umbilical cord repeatedly, allowing warmth to enter the abdomen. After a moment, Qi returns, and the infant revives. Bathing must be strictly avoided to prevent open pores (Couli) from leaking Yuanqi (Original Qi) and allowing external pathogens to invade.
Huanglian Method: Near term, finely chop Huanglian into powder, wrap in cotton, dip in boiling water, and wipe the mouth.
Gancao Method: Finely chop a small amount of Gancao in advance. At delivery, wrap in cotton, soak in a cup of boiling water to keep warm. During birth, use a soft cotton-wrapped finger dipped in Gancao juice to wipe the infant's mouth. Follow with the Huanglian Method and Zhumi Method.
Zhumi Method: Finely chop Huanglian, steep in boiling water for a long time, filter, and wipe the infant's mouth to expel foul fluids. Then give a bean-sized amount of finely ground Zhusha, applied to the mouth with honey from a clam shell. This not only calms the Heart, stabilizes the Spirit, and detoxifies, but also benefits the Liver and Gallbladder, clears irritability and heat, and dispels evil Qi.
The Niuhuang Method is similar to Zhumi, adding a small amount of Niuhuang to benefit the Liver and Gallbladder, clear heat, stabilize the Spirit, stop convulsions, dispel evil Qi, and cure various pediatric diseases.
