Jieguo stated: Regarding the disease of sha (a type of rash characterized by swelling), if the swelling manifests externally, it belongs to the Shaoyang Ministerial Fire and is called "sha"; if small red spots are hidden within the skin, it belongs to the Shaoyin Sovereign Fire and is called "zhen" (rash).
When sha and zhen appear simultaneously, the child may find it difficult to endure. However, purgative methods should not be used from beginning to end. Generally, there are more herbs that stabilize the interior than those that release the exterior. If urination is scanty, mild diuresis should be employed. A warm body indicates a favorable prognosis (shun), while a cool body indicates an unfavorable one (ni). It is strictly forbidden to apply cold-cooling herbs externally or use draining/purgative herbs internally. Without these two errors, the skin can be preserved.
A child suffered from sha-rash with fever, fatigue, and poor appetite. This was due to deficiency of Spleen and Lung Qi combined with external pathogenic factors. Initially, Xiaofeng San (Wind-Dispersing Powder) was administered for two doses, followed by Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Fuling (Poria) and Shaoyao (Peony Root), resulting in recovery.
Another child suffered from sha-rash accompanied by pain, fever, and thirst. After taking exterior-releasing formulas, the condition worsened. The patient exhibited physical exhaustion and a floating, rapid pulse. This indicated further damage to the True Qi. Consequently, Ren'an Wei San (Ginseng Formula to Calm the Stomach) and Buzhong Yiqi Tang were used, leading to recovery.
A child suffering from sha-rash fever took Xijiao Xiaodu San (Rhinoceros Horn Detoxification Powder) for one dose, which immediately induced vomiting and diarrhea. I stated: "The pathogenic factors are being expelled from both upper and lower pathways." No medication was needed; the child recovered spontaneously shortly thereafter.
A child, who typically had a pale complexion, suffered from itchy sha-rash, nasal congestion, runny nose, and incessant coughing. After using Baidu San (Poison-Expelling Powder), copious pus and fluid oozed out, accompanied by aversion to cold, wheezing, morning chills, and evening fevers. I reasoned that the Qi of the Spleen and Lung had been injured again. Buzhong Yiqi Tang was used to alleviate symptoms slightly, and Wuyi Yigong San (Five-Agent Extraordinary Merit Powder) was added as an adjuvant, resulting in full recovery.
A child suffered from itchy sha-rash with alternating chills and fever. The child was treated first with Xiaofeng San. Subsequently, the mother was treated with Jiawei Xiaoyao San (Modified Free Wanderer Powder). Both recovered after two months.
A child suffered from sha-rash with thirst, fever, and coughing. Since the pathogen was at the exterior, sweating therapy was appropriate. Gegen Jupi Tang (Kudzu Root and Tangerine Peel Decoction) was administered first for one dose, followed by Xuanshen Jupi Tang (Scrophularia Root and Tangerine Peel Decoction), which stabilized the condition. In the year Guichou, for patients with this syndrome, I initially used Gegend Jupi Tang to disperse the pathogen. If the pathogen was gone but fever persisted, Huangqin (Scutellaria Root) and Lianzi (Lotus Seed Plumule/Phellodendron Bark contextually likely Phellodendron here given 'Lian' usually refers to Huanglian in heat-clearing contexts, but text says 'Qin, Lian', standardly Huangqin & Huanglian) were added. If the fever had subsided, Xuanshen Shengma Tang (Scrophularia and Cimicifuga Decoction) was used, yielding rapid effects without exception.
A child suffered from red, painful sha-rash. Shengma Gegen Tang (Cimicifuga and Kudzu Root Decoction) was used first to reduce symptoms, followed by Xuanshen Shengma Tang to ensure stability.
A child, due to consuming rich foods and strong alcohol, developed a body rash resembling sha-rash. Xiaowei San (Stomach-Clearing Powder) was administered, and both mother and child recovered after taking it.
A child accidentally ingested realgar (Xinshi) and became crimson all over like sha-rash. (See Shangfa Dan entry).
A child’s body became crimson like sha-rash because the mother drank distilled liquor. (See Shangfa Dan entry).
