Mr. Chen Wenxiu stated: In cases of smallpox accompanied by diarrhea of watery stool mixed with undigested food, or where the stool is white or pale yellow, it is appropriate to administer Shiyiwei Muxiang San (Eleven-Ingredient Costus Root Powder) and take it with Roudoukou Wan (Nutmeg Pill). If severe diarrhea leads to internal depletion of body fluids and Qi and Blood failing to nourish the skin, even if the pustules have erupted, they will not form scabs. If the body is warm but the abdomen is distended, accompanied by teeth grinding, panting, and thirst, this is difficult to treat. This is because body fluids are exhausted; despite continuous drinking, thirst persists, scattering the True Qi, so death often ensues. One should quickly administer Shiyiwei Muxiang San to rescue the patient; if there is no improvement, urgently use Shi'erwei Yigong San (Twelve-Ingredient Yi Gong Powder). I consider Roudoukou Wan to be an astringent formula treating slippery diarrhea due to deficiency-cold of Yang Qi. Since the Kidney governs defecation, if the aforementioned symptoms are caused by insecurity of Kidney Qi, one should use Shiyiwei Muxiang San, or administer Six-Ingredient Gentlemen's Decoction (Liujunzi Tang) to send down Sishen Wan (Four Spirits Pill). If there is a sensation of needing to defecate but being unable to do so, this indicates deficiency and sinking of Spleen Qi; one should use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Nutmeg. If food is not digested and both hands and feet are cold, this indicates deficiency-cold of Spleen Qi; one should use Four-Ingredient Gentlemen's Decoction (Sijunzi Tang) added with Aconite Root (Fuzi).
A child suffering from smallpox developed diarrhea, abdominal distension, irritability, and thirst, drinking boiling water without feeling the heat. My father considered this to be deficiency-cold of Yang Qi and prescribed two doses of Shiyiwei Muxiang San, which immediately stopped the diarrhea. The child then found the soup too hot, indicating the recovery of Yang Qi. Subsequently, Liujunzi Tang, Dried Ginger (Ganjiang), Aromatic Costus Root (Muxiang), and Angelica Sinensis (Danggui) were used to facilitate scabbing.
Another child suffered from diarrhea after the resolution of smallpox, with inability to digest food. This was attributed to deficiency of Spleen and Kidney Qi. Liu Jun Zi Tang, Psoralea Fruit (Buguzhi), and Nutmeg were administered; once the diarrhea ceased, Shen Ling Sibai San (Ginseng and Poria Powder for the Four Sages) was used to replenish Yuan Qi, leading to recovery.
Another child presented with fever, preference for cold drinks, wrinkled and dry lips and tongue, and foul-smelling feces. My father diagnosed internal accumulation of heat and prescribed Qianhu Zhike San (Peucedanum and Aurantium Powder) for one dose, resulting in slight improvement. Then, Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction) added with Rhapontic Rhizome (Loulu) was given to the nursing mother, bringing immediate relief to the infant.
Another child had prolonged diarrhea following smallpox, resulting in emaciation. This was diagnosed as weakness of the Spleen and Kidney. Ershen Wan (Two Spirits Pill) and Wuwei Yigong San (Five-Ingredient Yi Gong Powder) were used gradually to achieve improvement. Later, due to food stagnation causing acid regurgitation and diarrhea, along with a heavy bearing-down sensation in the abdomen, this was identified as sinking of Spleen Qi. Buzhong Yiqi Tang was used as the primary treatment, assisted by Wuwei Yigong San, leading to gradual recovery. Finally, Shen Ling Sibai San and Tuoli San (Support the Interior Powder) were used to treat the sores, resulting in full recovery.
Another child with smallpox exhibited fever, dryness, and thirst, dark-colored eruptions with bleeding, hot feet, and lower back pain. This was diagnosed as deficiency-heat of the Spleen and Kidney. Shengji Xijiao Dihuang Tang (Shengji Rhinoceros Horn and Rehmannia Decoction) was administered for one dose, followed by several doses of Dihuang Wan material to consolidate the effect, and finally, Shenqi Neituo San (Ginseng and Astragalus Internal Support Powder) was used for recovery.
Another child experienced morning diarrhea just as the smallpox was healing, with undigested food in the stool. I diagnosed this as Kidney Diarrhea. Bu Zhong Yi Qi Tang was administered in the morning, and Ershen Wan in the evening, leading to recovery.
