Mr. Chen Wensu said: In the disease of smallpox, there are conditions of excess Yang and deficiency Yin, as well as conditions of excess Yin and deficiency Yang. Those with excess Yang drink ice and snow water without feeling cold; those with excess Yin drink boiling water without feeling heat. When Yang is in excess, one should tonify Yin, using Muxiang San (Wood Fragrance Powder) added with Dingxiang (Clove) and Guangui (Cinnamon Bark). When Yin is in excess, one should tonify Yang, using Yigong San (Extraordinary Efficacy Powder) added with Muxiang (Wood Fragrance) and Danggui (Angelica Sinensis). I hold that: The Classic says, "In cases of severe cold that do not respond to heat therapy, it indicates a lack of Fire (Kidney Yang), and one should benefit the source of Fire to dispel the shadows of Yin (warm and tonify Kidney Yang to dissipate Yin-cold)." In cases of severe heat that do not respond to cold therapy, it indicates a lack of Water (Kidney Yin), and one should strengthen the master of Water to suppress the sunlight (nourish Kidney Yin to control Yang hyperactivity). If the aforementioned symptoms manifest as fever with thirst, coldness of both hands and feet, spontaneous diarrhea, and a preference for hot drinks, this indicates excess Yin, and Shierwei Yigong San (Twelve-Ingredient Extraordinary Efficacy Powder) or Bawei Wan (Eight-Ingredient Pill) should be used. If the symptoms manifest as fever with thirst, constipation, heat in both hands and feet, and a preference for cold drinks, this indicates excess Yang, and Sishun San (Four-Smooth Powder) or Liuwei Wan (Six-Ingredient Pill) should be used. If the symptoms manifest as vexatious heat with thirst, a red face but pale sclera, this indicates deficiency heat of the Kidney channel, and Dihuang Wan (Rehmannia Pill) or similar formulas should be used. If treated in time, survival is possible. In the capital, children contracting smallpox sometimes present with thirst and a preference for cold water; allowing them to drink freely without administering other medicines often leads to recovery by the expected time, without complications from smallpox toxins. This is because northerners sleep on heated kang beds and drink distilled liquor, leading to an internal condition where heat and water interact. Children whose facial complexion and sclera appear mostly white have inherited weakness of Kidney Qi; when they contract smallpox, they will inevitably experience thirst. Administering Dihuang Wan decoction for free drinking often results in survival.
A child presented with chills, teeth grinding, diarrhea, thirst, abdominal distension, and coldness of both hands and feet during smallpox eruption. It was midsummer, yet he drank boiling water without feeling heat. This was false heat due to deficiency-cold of the Spleen and Stomach. Initially, one dose of Shierwei Yigong San brought immediate relief. Then, one dose of Liujunzi Tang (Six-Gentlemen Decoction) added with Fuzi (Aconite Lateral Root) was administered. Finally, Wuwei Yigong San (Five-Ingredient Extraordinary Efficacy Powder) led to full recovery.
A man presenting with smallpox had purple-colored lesions, thirst with desire to drink, lower back pain, heat in the feet, and deafness. This indicated inherited insufficiency of Kidney Qi. Jiajian Bawei Wan (Modified Eight-Ingredient Pill) decoction was administered for free drinking, which immediately stopped the heat and thirst. This was supplemented with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Wuweizi (Schisandra Fruit) and Maidong (Ophiopogon Tuber), nourishing the source of transformation until recovery.
A fourteen-year-old child presented with dark-colored smallpox lesions, heat and pain in both legs and waist, constipation, dry throat, and thirst with desire to drink. My late father attributed this to inherited Kidney insufficiency. A large dose of Jiajian Bawei Wan decoction was administered for free drinking. After consuming two jin (approx. 1 kg), all symptoms subsided. This was followed by Yi Qi Tang (Boost Qi Decoction) and Bazhen Tang (Eight-Treasure Decoction), each administered for over ten doses, resulting in complete recovery.
A child presented with fever and thirst due to smallpox, abdominal distension, chills, teeth grinding, and drinking ice/snow water without feeling cold. The presentation resembled fire syndromes entirely, except for the coldness of both feet, indicating deficiency-cold of Yang Qi. Initially, one dose of Shierwei Yigong San was given. Subsequently, Wuwei Yigong San added with Shengjiang (Fresh Ginger) and Rongui (Cinnamon Twig) gradually improved the condition. Later, Rongui was removed, and after two more doses, full recovery was achieved.
Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill): Treats symptoms such as fever and thirst in smallpox due to Kidney deficiency.
Shudihuang (Prepared Rehmannia Root, 8 liang pounded into paste) Gan Shanyao (Dried Dioscorea Rhizome) Shanzhuyu (Cornus Officinalis Fruit flesh, 4 liang each) Zexie (Alisma Rhizome) Baifuling (Poria Cocos, 4 liang each) Mudanpi (Moutan Cortex, 3 liang each).
Grind the above into powder, add the Dihuang paste, mix with rice paste in appropriate amounts to form pills the size of small beans. Decocting and taking the medicine is particularly effective.
Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder, also known as Ren Shen Baizhu San): Treats weak Stomach Qi, or dry mouth and thirst with poor appetite caused by excessive purgation or vomiting/diarrhea. If the patient prefers cold drinks, remove Muxiang.
Huoxiang (Patchouli Herb) Baizhu (Atractylodes Macrocephala Rhizome) Muxiang (Vladimiria Root) Baifuling (Poria Cocos) Gancao (Licorice Root, stir-fried) Renshen (Ginseng Root) Gange (Kudzu Root).
Take 5 qian per dose, decoct in water, and take slowly.
