Mr. Chen Wensu said: If a child with smallpox has not had a bowel movement for four or five days, boiled lean pork in plain water, cut into pieces the size of beans (about 5-7 pieces), and let the child eat it to moisten the zang-fu organs, facilitating the falling of scabs. One must absolutely avoid recklessly administering purgative or laxative medicines, lest this deplete the true qi internally and cause the toxin to sink inward. If after six or seven days the body has high fever and no bowel movement, and the pulse is tight and vigorous, administer Sanwei Xiaodu Yin (Three-Ingredient Toxin-Resolving Decoction) to gently promote evacuation. I suggest: For the aforementioned symptoms, if the toxin is in the muscles and has not fully erupted, and the pulse is floating and tight, this medicine is most suitable to disperse and resolve the toxin. If the pulse is deep and tight, Qianhu Zhike San (Peucedanum and Bitter Orange Powder) should be used to unblock the toxic qi, cutting off the source. If there is swelling and pain in the mouth, tongue, and throat, indicating severe toxin, use Shegan Niuzibang Tang (Belamcanda and Burdock Decoction). If the bowel movement has already cleared, but thirst and desire for warm drinks persist, this indicates deficiency of Spleen and Stomach qi; Ren Shen Bai Zhu San (Ginseng and Atractylodes Macrocephala Powder) should be used. For dry feces stuck in the rectum unable to pass, pig bile suppository method is appropriate; avoid using purgative agents, lest they further damage Stomach qi, causing un-erupted lesions to fail to emerge and erupted lesions to fail to crust over. Generally, to distinguish between excess and deficiency, one should verify by observing the temperature of the hands and feet, or by asking about the desire to drink water or warm fluids.
One child presented with constipation, red and painful lesions, fever, thirst, and hot extremities. This was residual toxin acting internally. After one dose of Qianhu Zhike San, the bowels moved freely, and all symptoms subsided immediately. Subsequently, Liuwei Huoxue San (Six-Ingredient Blood-Invigorating Powder) was administered, leading to recovery.
Another child presented with fever and thirst due to smallpox, red and swollen painful lesions, and constipation. This indicated internal heat toxin. Initially, Qingliang Yin (Cooling Drink) was given for one dose, slightly reducing symptoms. Then, Niuzibang Tang (Burdock Decoction) was given for one dose, completely resolving symptoms. Next, Zicao Kuiban Tang (Arnebia Rapid-Eruption Decoction) was used to promote suppuration, followed by Xiaodu Yin (Toxin-Resolving Decoction) to facilitate crusting.
Another child presented with red lesions, manic panting, and difficult defecation. This indicated heat in the Stomach meridian. My father treated him with Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) combined with Celery Juice, resulting in recovery.
Sishun Qingliang Yin (Four-Harmony Cooling Drink): Treats accumulated heat causing facial redness, thirst, limb convulsions/tremors, and constipation.
Ingredients: Chi Shaoyao (Red Peony Root), Danggui (Angelica Sinensis), Gancao (Licorice Root), Dahuang (Rhubarb Root and Rhizome) (equal parts).
Usage: Take one qian per dose, decoct in water.
Note: Qingliang Yin is a bitter-sweet formula that drains internal heat. It is suitable when heat toxin is internal, causing constipation, without external symptoms. Lianqiao Yin (Forsythia Decoction) is a bitter-cold formula that releases the exterior and drains internally. It is suitable for excess heat in both interior and exterior, accompanied by irritability, thirst for cold drinks, constipation, and scanty dark urine. Do not overdose. Reckless sweating may lead to ulceration, while reckless purging may lead to collapse.
Xiaodu Yin: Jingjie (Schizonepeta Spike), Fangfeng (Saposhnikovia Root), Niuzibang (Burdock Fruit) (each 1.5 qian), Gancao (Licorice Root) (1 qian).
Usage: Decoct in water, take according to condition.
