According to the *Baojian*, a newborn infant is merely a lump of blood. There are no fixed symptom patterns; if there is no pulse but convulsions occur, this is fetal convulsion (Tai Jing); if there is fever, this is fetal heat (Tai Re).
The infant and the nursing mother are physiologically one. Whenever an infant suffers from sores or ulcers, one must examine whether the nursing mother's Liver Channel has heat. If so, use formulas such as Jiawei Xiaochaihu Tang (Modified Minor Bupleurum Decoction). If it is deficiency-heat in the Liver Channel, use formulas such as Jiawei Xiaoyao San (Modified Free Wanderer Powder). If Kidney Water fails to nourish Liver Wood (Water generates Wood), use Dihuang Wan (Rehmannia Pill). If there is accumulated heat in the Heart Channel, use Chaihu Zhizi San (Bupleurum and Gardenia Powder). If there is deficiency-heat in the Heart Channel, use Fuling Buxin Tang (Poria Tonifying Heart Decoction). If there is accumulated heat due to rich and greasy food, use Dongyuan Qingwei San (Dongyuan Stomach-Clearing Powder). If there is constrained heat in the Spleen Channel, use Qianshi Xiehuang San (Qian's Draining Yellow Powder). If there is deficiency-heat in the Spleen Channel, use Qianshi Yigong San (Qian's Yi Gong Powder). If one administers Xijiao Wan (Rhinoceros Horn Pill) or Huadu Dan (Toxin-Dissolving Pellet), and applies cold-natured medicines externally, it will damage the body's vital energy (Sheng Qi), thereby accelerating critical danger.
One infant, after birth, had a swelling on the front of the ear like a small chestnut. After more than ten days, it became red, swollen, and elevated, crying upon touch. This belongs to the Gallbladder Channel area. The nursing mother's pulse was examined and found to be rapid in the Liver and Gallbladder channels. This was caused by inheriting the mother's Liver Fire, which was exacerbated by the mother's existing Liver Fire. A few days later, the infant became restless and irritable, with occasional twitching of the hands and feet, caused by pain from suppuration. Three days later, in the morning, gentle pressure on the head of the sore caused the swelling to rebound immediately, indicating that pus had formed. By noon, a thin skin appeared on the top of the sore, indicating the pus was ripe. Applying Daizhen Gao (Needle-Replacing Ointment), pus drained in the evening, and the infant instantly calmed down, with the redness and swelling disappearing immediately. This is the most favorable outcome for treating sores. Applying Taiyi Gao (Great Unity Ointment) protected against wind and cold, and the nursing mother took Xiaoyao San (Free Wanderer Powder) and recovered.
Another infant, after birth, had a swelling about one cun in size on the outer arm. Over a month, it suddenly became red and swollen to about two cun, with a spreading red halo, indicating internal accumulation of pus toxin. Pricking released pus, the red halo receded, and the infant settled. The nursing mother's Liver and Gallbladder pulses were wiry and rapid, forceful upon pressure. First, Jiawei Xiaochaihu Tang (Modified Minor Bupleurum Decoction) with two qian of Huanglian (Coptis Rhizome) was given for two doses. Then, Huanglian was removed for another two doses. Subsequently, the nursing mother took Jiawei Xiaoyao San (Modified Free Wanderer Powder), and the infant soon recovered.
Yet another infant, after birth, had a swelling about one cun in size on the thigh, with a pale complexion. When the time came to apply medicine and ulcerate, the pus water was clear and thin, and it did not heal after two cycles. Later, yawning and teeth-grinding occurred, indicating inherited Kidney Deficiency. In the morning, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was administered; in the evening, Dihuang Wan (Rehmannia Pill) material was given. Half a cup was shared between mother and son, and recovery took over a year.
Another infant, after birth, had a diffuse, headless swelling in the left hypochondrium with unchanged flesh color. Tiegu San (Iron-Hoop Powder) was applied. Upon ulceration, the pus was clear, accompanied by nausea. I reasoned that this was due to inherited Liver Channel Qi stagnation and Spleen Qi deficiency, making healing difficult. First, Yigong San (Yi Gong Powder) with Chaihu (Bupleurum Root) and Shengma (Cimicifuga Rhizome) was used to tonify the Spleen and Stomach. Then, Tuoli San (Support the Interior Powder) with Chaihu (Bupleurum Root) and Shanzhi (Gardenia Fruit) was used to support the interior and clear Liver fire. The child also drank several spoonfuls, recovering in three months.
Another infant, after birth, had a swelling in the lower abdomen that did not ulcerate for over a year, accompanied by alternating chills and fever. This was due to inherited Liver Fire. The mother indeed suffered from irregular menstruation, internal heat, and fatigue. Dihuang Wan (Rehmannia Pill) and Bazhen Tang (Eight-Treasure Decoction) were given to the mother, and the child took half a cup daily, recovering soon.
Another infant, after birth, had red and swollen chest and hypochondrium that did not subside for over a year. I reasoned this was inherited Liver Blood Heat and advised treating only the mother. Disbelieving, they used Tiegu San (Iron-Hoop Powder) and Xijiao Wan (Rhinoceros Horn Pill), causing vomiting and refusal to nurse. This indicated damaged Stomach Qi. I used Wuyi Yigong San (Five-Essence Yi Gong Powder) to save the child's Stomach Qi and Jiawei Xiaoyao San (Modified Free Wanderer Powder) to treat the mother's Liver Fire, resulting in immediate recovery.
Another infant, after birth, had exposed, red skin all over the body. This originated from the mother's habitual consumption of rich foods. Using 1 liang of Hanshui Shi (Calcitum), 2 liang of fried Huangbai (Phellodendron Bark), and 4 liang of clean Huangtu (Yellow Earth), all ground into fine powder, it was applied frequently over the body. The mother took Qingwei San (Stomach-Clearing Powder) with Loulu (Euphorbia Root). After five days, the redness diminished and lightened. Then, 5 liang of Huangtu (Yellow Earth) and 1 liang of fried Huangbai (Phellodendron Bark) were applied. The mother took Jiawei Xiaoyao San (Modified Free Wanderer Powder). After three more days, the redness faded significantly, and exudate decreased. After another three days, only Huangtu (Yellow Earth) was applied, and the mother took Bazhen Tang (Eight-Treasure Decoction) with Mudanpi (Moutan Cortex) and Chaihu (Bupleurum Root), leading to recovery.
Another infant born with scabies: examining the mother revealed chronic depression and anger. Xiaodu San (Detoxification Powder) was mixed with Danggui Gao (Angelica Ointment) for application. The mother took Jiawei Xiaoyao San (Modified Free Wanderer Powder) with Loulu (Euphorbia Root) and Jiawei Guipi Tang (Modified Restore the Spleen Decoction), recovering. Later recurrence was due to the mother eating rich foods; Qingwei San (Stomach-Clearing Powder) and previous external applications cured it.
Another infant born with hemorrhoids began experiencing pain at age three. Taking Huadu Dan (Toxin-Dissolving Pellet) and Xijiao Wan (Rhinoceros Horn Pill) to treat Large Intestine Fire resulted in abdominal pain, diarrhea, teeth-grinding, and yawning, yet treatment still aimed at clearing fire. I stated: yawning and teeth-grinding belong to the Liver Channel. The *Neijing* says: "Due to overeating, tendons and vessels relax transversely, forming hemorrhoids in the intestines." This was caused by inherited Liver Fire. The child took Dihuang Wan (Rehmannia Pill); the mother took Xiaoyao San (Free Wanderer Powder) with Loulu (Euphorbia Root), and both recovered.
Another infant had red, itchy scrotum, sometimes appearing skinless, with eyes often closed. Taking Huadu Dan (Toxin-Dissolving Pellet) worsened the condition. I stated: Huadu Dan (Toxin-Dissolving Pellet) and Xijiao Wan (Rhinoceros Horn Pill) treat Stomach and Spleen Excess Fire. The previous symptoms were due to inherited Liver and Kid Yin Deficiency. Disbelieving, they continued administration, nearly causing death. I used Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) and Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill); the mother took Jiawei Xiaoyao San (Modified Free Wanderer Powder), and recovery ensued.
Another infant had a red and swollen scrotum after birth. I reasoned this was inherited Liver and Kidney Yin Deficiency. Disbelieving, they used Huadu Dan (Toxin-Dissolving Pellet) and similar remedies, worsening symptoms, followed by vomiting, refusal to nurse, and cold extremities, indicating Spleen and Stomach damage. First, Wuyi Yigong San (Five-Essence Yi Gong Powder) was used. The mother took a large dose of Dihuang Wan (Rehmannia Pill) material with fried-black Huangbai (Phellodendron Bark) and Loulu (Euphorbia Root) for several doses, reducing the swelling. At that time, those suffering from this condition who took Huadu Dan (Toxin-Dissolving Pellet) and applied cold medicines were not saved.
Another infant had a slight swelling, about one cun, at the tip of the buttocks. Applying Tiegu San (Iron-Hoop Powder) and taking Huadu Dan (Toxin-Dissolving Pellet), after a month the swelling rose, turned red, and the infant cried incessantly. Pressing with a finger caused immediate rebound, indicating mature pus and pain. Pricking released thick pus, and crying stopped immediately. I stated: Blood and Qi were not deficient, so medication was unnecessary. Desiring quick results, they took Xijiao Wan (Rhinoceros Horn Pill), causing vomiting, diarrhea, and convulsions, prompting attempts to use anti-convulsant drugs. I stated: This is wind generated internally due to Spleen and Stomach deficiency. Urgently, 1 liang of Renshen (Ginseng Root) was finely sliced and mixed with one zhong of milk from a healthy woman, boiled in a porridge pot for a long time, then strained. Cotton was shaped like a nipple, dipped in the milk, and frequently offered to the infant to suckle. Consumed in one day, followed by Emu Dihuang Wan (Emulsified Rehmannia Pill). The mother took Bazhen Tang (Eight-Treasure Decoction) with Loulu (Euphorbia Root) daily, recovering in less than a month.
Another infant had a red and swollen patch, about two cun, inside the buttock, with internal pus rupture. Pricking released much pus, followed by eye-closing and teeth-grinding. I stated: Eye-closing indicates Spleen Qi deficiency unable to open; convulsions and teeth-grinding indicate Spleen Qi deficiency with stirring Liver Fire. Renshen (Ginseng Root) was soaked in milk as before for the infant to suckle. The mother took Bazhen Tang (Eight-Treasure Decoction) with Loulu (Euphorbia Root), and the sore healed in over a month.
An infant, more than ten days old, developed a toxic sore on the head, rising one cun high with a red halo, in critical condition. Lying down, a lancet scarification released black blood, and the infant calmed. The next day, a lesion appeared between the eyebrows with a red halo. I intended to scarify immediately, but others suggested waiting until the second day. Consequently, blood coagulated and could not flow out, leading to abdominal distension and death.
Tiegu San (Iron-Hoop Powder) treats all sores, boils, and carbuncles.
Furong Ye (Hibiscus Leaf) Huangbai (Phellodendron Bark) Dahuang (Rhubarb Root) Wubeizi (Chinese Gall) Baiji (Bletilla Rhizome)
Grind into powder, mix with water, and apply around the periphery.
Note: The above formula is a cold-natured agent for clearing heat and astringing. Some use Bailian (Ampelopsis Radix) or Shanglugen (Phytolacca Root); some use Hanshui Shi (Calcitum) or Tianhuafen (Trichosanthes Root); some use Canger (Xanthium Fruit) or Jinyinhua (Lonicera Flower); some use Bajiao (Banana) or Chixiaodou (Adzuki Bean); some use Caowu (Aconite Root) or Baizhi (Angelica Dahurica Root). These are indiscriminate mixtures regardless of cold, heat, warm, or cool nature. The *Neijing* states: "Swelling first, then pain, means shape injury affecting Qi; pain first, then swelling, means Qi injury affecting shape." It also states: "Disorder of the Five Zang organs leads to blockage of the Nine Orifices; disorder of the Six Fu organs leads to nodules forming abscesses." The *Waike Jingyi* states: "Sores that are high and soft originate in the Blood and Vessels; sores that are low and hard originate in the Tendons and Vessels; sores with unchanged flesh color originate in the Marrow and Bones." Internal pathology must manifest externally. Therefore, the affected meridian and location vary; how can one uniformly apply cold medicines externally? Only when the Spleen and Stomach are intact and Blood and Qi are harmonious might there be efficacy. If Huadu Dan (Toxin-Dissolving Pellet) and similar agents are taken, damaging the Spleen and Stomach again, causing stagnation of Qi transformation, resolution cannot occur. For conditions like Ding Chuang (Carbuncles), the goal is to draw out toxicity; using the aforementioned cold medicines would chill the muscles, stagnate Blood and Qi, and inevitably cause toxic pathogens to enter internally, leading to untreatable cases. Treatment must observe whether the swelling is high or diffuse, the color red or white, the pain mild or severe, the ease or difficulty of suppuration, the thickness or thinness of pus, and the speed of tissue regeneration, to distinguish Yin or Yang syndromes, or semi-Yin semi-Yang, or pure Yin or pure Yang, and use appropriate cooling, warming, or harmonizing medications. One must also assess the transmission of the disease and the mutual overcoming of the Five Zang organs, focusing primarily on regulating and tonifying the Spleen and Stomach, to avoid transformation into malignant conditions.
Wufu Huadu Dan (Five-Fortunes Toxin-Dissolving Pellet) treats fetal toxins and sores arising on the head and face after smallpox/variolae, with eye swelling and pain.
Sheng Dihuang (Raw Rehmannia Root, pounded into paste) Shu Dihuang (Prepared Rehmannia Root, homemade pounded into paste) Tianmendong (Asparagus Tuber, heart removed, pounded into paste) Maimendong (Ophiopogon Tuber, heart removed, pounded into paste) Xuanshen (Scrophularia Root) (each 3 liang) Gancao (Licorice Root) Tianxiao (Sweet Niter) (each 1 liang) Qingdai (Indigo Naturalis) (1.5 liang)
Grind into powder, refine honey, and make pills the size of a Chinese quince seed. Take one pill per dose, dissolved in boiling water. Tianxiao (Sweet Niter) is Puxiao (Mirabilite), processed with boiling water.
Note: For the previous symptoms, if taking this formula causes phlegm-wheezing and convulsions, it indicates injury to Middle Qi transforming into deficiency-heat. Urgently take Wuyi Yigong San (Five-Essence Yi Gong Powder). If extremities are cold, it indicates Middle Qi deficiency-cold; adding Ginger and Cinnamon to the previous decoction often yields survival.
Xijiao Xiaodu Wan (Rhinoceros Horn Detoxification Pill) treats accumulated heat and sores arising from residual toxins after smallpox/variolae.
Sheng Dihuang (Raw Rehmannia Root) Fangfeng (Saposhnikovia Root) Danggui (Angelica Sinensis Root) Xijiao Xie (Rhinoceros Horn Shavings, sliced) Jingjie Sui (Schizonepeta Spike) (each 1 liang) Niubangzi (Arctium Seed, pounded and fried) Chi Shaoyao (Red Peony Root) Lianqiao (Forsythia Fruit) Jiegeng (Platycodon Root) (each 7 qian)
Grind into powder, refine honey, and make pills the size of a Chinese quince seed. Take one pill per dose, dissolved in Mint Decoction.
Note: The previous two formulas easily damage Middle Qi and injure Yin Blood. They may be used only if adults present with excess syndrome and robust Spleen and Stomach function. Infants have fragile and tender viscera and cannot withstand these. The Classics state: "Qi governs pushing/venting, Blood governs moistening." Qi is the harmonious Original Qi in the Stomach. If Stomach Qi is injured, it cannot push, vent, and moisten; formed pus cannot undergo putrefaction and discharge; discharged pus cannot regenerate tissue and close wounds. Thus, it becomes difficult to treat, even fatal. One must be extremely cautious!
Bawei Fuling Buxin Tang (Eight-Ingredient Poria Tonifying Heart Decoction) treats insufficient Heart Qi, disharmony of Blood and Qi, and sores. (Formulated by the author)
Fuling (Poria, 5 fen) Suanzaoren (Spina Date Seed, fried, each 2 qian) Wuweizi (Schisandra Berry, fried) Danggui (Angelica Sinensis Root, each 1 qian) Renshen (Ginseng Root, 1.5 qian) Baizhu (Atractylodes Macrocephala Rhizome, fried, 1 qian) Changpu (Acorus Rhizome, 5 fen) Yuanzhi (Polygala Root, heart removed, 6 fen) Gancao (Licorice Root, fried, 5 fen)
Divide into two or three doses, decoct in water.
Chaihu Zhizi San (Bupleurum and Gardenia Powder) (See formula for Hypochondriac Carbuncle)
Tuoli San (Support the Interior Powder) (See formula for Heat Toxicity)
