Tremor and convulsion (limb traction and spasm) are similar. Convulsion manifests as limb traction, with extension or flexion; whereas tremor is merely trembling and shaking, without extension or flexion. The *Huangdi Neijing* states: "Because the Stomach Qi is not substantial, all meridians become empty." Treatment should follow the movement of Yin Qi, tonifying the Qi in the muscle interspaces at the site of the lesion. However, when children suffer from ulcerated and putrid sores, or injuries, with excessive discharge of pus and blood, it belongs to Spleen and Stomach Qi deficiency and Blood weakness. The primary treatment should be Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) and Wuyi Yigong San (Five Agents for Extraordinary Merit Powder), modified with Baizhu (Atractylodis Rhizoma), Danggui (Angelicae Sinensis Radix), and Shengma (Cimicifugae Rhizoma). If there is deficient heat in the Liver meridian, Liuwei Wan (Six-Ingredient Pill with Rehmannia) is used. For Spleen Blood deficiency, Sijunzi Tang (Four Gentlemen Decoction) modified with Xiong (Ligustici Chuanxiong Rhizoma) and Gui (Angelicae Sinensis Radix) is used. For Stomach Qi deficiency, Buzhong Yiqi Tang is used.
A child developed a carbuncle on the leg, which internally ulcerated and discharged approximately one bowl of pus. Immediately thereafter, tremors occurred, with a pale complexion and sweating. This indicates Yang Qi collapse and depletion, requiring major tonification. Ren Shen (Ginseng Radix et Rhizoma) 1 liang was decocted and administered. The sweating worsened, and both hands and feet became cold. Subsequently, Ren Shen 2 liang and Gan Jiang (Zingiberis Rhizoma) 2 qian were decocted and administered. After a long time, the sweating subsided slightly. After two more doses, all symptoms resolved abruptly. Thereafter, Buzhong Yiqi Tang modified with Ren Shen 5 qian was administered for several doses until recovery.
Another child had an arm carbuncle that ulcerated, followed by tremors and shortness of breath. The pulse was floating and rapid, but weak upon deep pressure (not drumming against the finger), indicating Yuan Qi (Original Qi) deficiency. In the morning, Buzhong Yiqi Tang was administered; in the evening, Yigong San was administered. After more than twenty doses of each, no effect was seen. This was because the deficiency was severe, and the medicinal power had not yet reached the disease site. The previous medicines were continued for another twenty-plus doses each, after which the tremors gradually improved. Later, Tuoli San (Support the Interior Powder) was added as an adjuvant, and the sore also healed.
A sixteen-year-old child had a buttock carbuncle that ulcerated, leading to tremors. Large-dose Qi-tonifying medicines were used, stopping the tremors. Later, due to fatigue-induced fever, the carbuncle internally ulcerated again, triggering tremors. The pulse was floating and rapid, weak upon deep pressure, and the Cun (inch) pulses on both wrists were short and small, failing to reach their normal position. Someone suggested dispelling wind. I said: A long pulse indicates regulated Qi; a short pulse indicates Qi pathology. This is caused by extreme Stomach Qi deficiency. Dushen Tang (Single Ginseng Decoction) was administered for several doses, resulting in recovery. Then, Buzhong Yiqi Tang was added as an adjuvant, administered for fifty-plus doses of each until recovery. If one slice of Fuzi (Aconiti Lateralis Radix Praeparata) had been added, recovery could have been achieved in a few doses.
A sixteen-year-old female had a swelling on her arm, with unchanged skin color, painful upon pressure. After taking Jiudu Liuqi agents (antidotal and Qi-moving preparations), she developed increased tremors. It was early spring; her face appeared flushed as if covered by sunlight, and her hands were not afraid of cold. The pulse was floating and rapid, weak upon deep pressure and short. She desired to attack the toxin. I said: This is due to Ying and Wei (Nutritive and Defensive) Qi weakness, attacked by external cold. Adding more antidotal medicines damages Stomach Qi; would this not exacerbate the tremors? Therefore, Ren Shen 5 qian, Huangqi (Astragali Radix) 3 qian, Danggui 3 qian, Shudi (Rehmanniae Radix Preparata) 3 qian, Shengma 5 fen, and Chaihu (Bupleuri Radix) 5 fen were administered. After twenty-plus doses, the tremors eased slightly. Then, Buzhong Yiqi Tang was added as an adjuvant, containing Ren Shen 5 qian. After another twenty-plus doses, combined with Congyun method (scallion moxibustion/fomentation), the swelling also healed.
A female suffered from scrofula (luoli). Due to anger, her hands trembled. Her complexion alternated between green and red. This indicated Liver meridian Blood deficiency with excess fire generating wind. Siwu Tang (Four Substances Decoction) modified with Zhizi (Gardeniae Fructus), Goutenggou (Uncariae Ramulus cum Uncis), Longdancao (Gentianae Radix), and Gancao (Glycyrrhizae Radix et Rhizoma) was used, and the tremors gradually improved. Longdancao was then removed, and Dihuang Wan (Rehmannia Pill) was administered alternately until recovery. Later, due to mental exertion causing fever, her hands trembled again. Buzhong Yiqi Tang and Dihuang Wan were used until recovery.
A child suffered from an arm carbuncle. His complexion alternated between yellow and red. Initially, Buzhong Yiqi Tang and Dihuang Wan were used, and he recovered quickly. Later, due to anger, tremors occurred. Buzhong Yiqi Tang modified with Goutenggou and Chao Zhizi (Stir-fried Gardeniae Fructus) was used first; then JiaWei Xiaoyao San (Modified Free and Easy Wanderer Powder) modified with Goutenggou was used until recovery. Later, due to food stagnation, vomiting and diarrhea with sour odor occurred, accompanied by spasms. Wuyi Yigong San modified with Goutenggou was used until recovery.
A female suffered from liuzhu (flowing abscesses), with fever and tremors. This was a syndrome of Liver and Spleen Qi and Blood deficiency, delayed menstruation, and deficiency fire generating wind. Buzhong Yiqi Tang modified with Goutenggou was used initially, leading to gradual improvement. Then JiaWei Dihuang Wan (Modified Rehmannia Pill) was used until complete recovery.
A female did not receive affection from her stepmother. Over time, depression and anger led to tremors. Her face was red and she had a fever. JiaWei Xiao Chaihu Tang (Modified Minor Bupleurum Decoction) was used first, followed by JiaWei Guipi Tang (Modified Restore the Spleen Decoction) and JiaWei Xiaoyao San, administered alternately until recovery. However, her complexion sometimes turned green, so Dihuang Wan and Xiaoyao San were used to stabilize her condition.
A female suffered from an abdominal carbuncle with this condition. Her limbs were sometimes rigid and sometimes flaccid. Siwu Tang modified with Chaihu, Zhizi, Danpi (Moutan Cortex), and Goutenggou was used first to nourish Blood and clear Liver fire. Then Dihuang Wan was used to nourish Kidney and generate Liver Blood until recovery.
