Postpartum edema of the four limbs, alternating chills and fever (alternating aversion to cold and fever), asthma and cough, discomfort in the chest and diaphragm, spitting out sour fluid, and pain in both hypochondriac regions. People generally attribute this to residual blood entering the meridians and seeping into the four limbs, causing qi rebellion. Little do they know that this is due to deficiency of both the Liver and Kidney, where Yin fails to control Yang (Yin failing to anchor Yang).
In postpartum women, Qi and Blood are severely depleted, naturally leading to insufficient Kidney Water, which then boils; however, when Water is insufficient, it cannot nourish Liver Wood, causing Liver Wood to become extremely dry and lacking in fluids within the Wood. Dryness of Wood generates Fire, and with Kidney Fire as its ally, the mother and child organs burn each other. The flames rush upward to attack Lung Metal. Lung Metal, subjected to Fire's punishment, lacks the strength to control Liver Wood, giving rise to diseases such as cough, wheezing, and distension. Since Liver Fire is vigorous, it attacks Spleen Earth downward; Spleen Earth, subjected to Wood's punishment, lacks the strength to control Kidney Water, resulting in the disease of edema of the four limbs.
However, the vigor of Liver Wood's Fire is a false appearance, not true excess. This falsely excessive Qi appears abundant but is actually deficient; thus, there are times of heat and times of cold, with alternating chills and fever without fixed patterns. This alternation follows the fluctuation of Qi's abundance and deficiency. The cold here is not true cold, nor is the heat true heat; rather, it is simply Qi rebelling and stagnating in the chest and diaphragm, causing discomfort. The hypochondriac regions are the domain of the Liver, and sourness is the flavor associated with the Liver. Spitting sour fluid and hypochondriac pain are both manifestations of Liver deficiency and the Kidney's failure to nourish it.
The treatment method should involve tonifying Blood to nourish the Liver and tonifying Essence to generate Blood. When Essence and Blood are sufficient, Qi will naturally flow smoothly, and all symptoms such as chills, fever, cough, and edema will recede.
Use the Formula for Transforming Qi (Zhuanqi Tang).
Ren Shen (Ginseng Radix) (3 qian), Fu Ling (Poria) (3 qian, peeled), Bai Zhu (Atractylodis Macrocephalae Rhizoma) (3 qian, stir-fried with soil), Dang Gui (Angelicae Sinensis Radix) (5 qian, washed with wine), Bai Shao (Paeoniae Radix Alba) (5 qian, stir-fried with wine), Shu Di Huang (Rehmanniae Radix Praeparata) (1 liang, steamed nine times), Shan Yu Rou (Corni Fructus) (3 qian, steamed), Shan Yao (Dioscoreae Rhizoma) (5 qian, stir-fried), Qian Shi (Euryales Semen) (3 qian, stir-fried), Chai Hu (Bupleuri Radix) (0.5 qian), Gu Zhi (Psoraleae Fructus) (1 qian, stir-fried with salt water).
Decoct in water and take orally. Effective after three doses, cured after ten doses. This formula consists entirely of substances that tonify Blood and Essence. Why is it named "Transforming Qi"? It is unknown that Qi rebellion stems from Qi deficiency, specifically deficiency of Liver and Kidney Qi. Tonifying the Essence and Blood of the Liver and Kidney is precisely the method to tonify their Qi. Deficiency leads to rebellion, while vigor leads to smooth flow; thus, tonification is transformation. Once Qi transforms, all symptoms heal, Yin emerges to harmonize with Yang, eliminating any concern for Yin-Yang rejection.
Marginal Note: The efficacy of this formula lies in its precision; it should not be arbitrarily modified or added to. Bai Shao should preferably be used charred.

Postpartum edema affects all four limbs,
Coughing, spitting sour fluid, and pain in both hypochondria.
Alternating chills and fever indicate Yin-Yang conflict,
Great depletion of Qi and Blood leaves Kidney Water empty.
