A woman, after childbirth, had a fleshy cord protruding from the vaginal canal, two or three chi long. Touching it caused excruciating pain, nearly causing fainting. People assumed it was the prolapse of the placenta (Baotai), but who would know that this is actually due to the deficiency and collapse of the Dai Mai (Girdle Vessel)!
The Dai Mai binds between the Ren Mai (Conception Vessel) and Du Mai (Governor Vessel). The Ren Mai is anterior and the Du Mai is posterior. When these two vessels have strength, the Dai Mai becomes firm and secure; when they lack strength, the Dai Mai collapses and sags. After childbirth, excessive blood loss means there is no blood to nourish the Ren Mai and Du Mai, leading to the collapse of the Dai Mai. Its strength is insufficient to lift, so it descends along with urination. The sagging Dai Mai often causes pain in the waist and umbilical region. Moreover, since it descends out of the birth canal, the failure at the critical gate is even more severe, how can it not cause excruciating pain?
Prescribe Liang Shou Tang (Double Astringent Decoction).
Ren Shen (Ginseng Radix) - 1 liang
Bai Zhu (Atractylodis Rhizoma) - 2 liang, stir-fried with soil
Chuan Xiong (Ligusticum Chuanxiong Rhizome) - 3 qian, washed with wine
Jiu Zheng Shu Di Huang (Prepared Rehmanniae Radix, steamed nine times) - 2 liang
Shan Yao (Dioscoreae Rhizoma) - 1 liang, stir-fried
Shan Yu (Corni Fructus) - 4 qian, steamed
Qian Shi (Euryales Semen) - 5 qian, stir-fried
Bian Dou (Lablab Album Seed) - 5 qian, stir-fried
Ba Ji Tian (Morinda Officinalis Root) - 3 qian, soaked in salt water
Du Zhong (Eucommiae Cortex) - 5 qian, stir-fried until black
Bai Guo (Ginkgo Biloba Seed) - 10 pieces, crushed
Decoct in water and take orally. After one dose, half recovers; after two doses, full recovery occurs. This formula tonifies the Ren Mai and Du Mai while also tonifying the waist and umbilicus because the Ren Mai and Du Mai are connected to the waist and umbilicus. If only the Ren Mai and Du Mai are tonified without the waist and umbilicus, the Ren Mai and Du Mai will lack assistance, and how can the Dai Mai be lifted? Only by tonifying both can the Ren Mai and Du Mai receive help from the waist and umbilicus, allowing the Dai Mai to gain the strength of the Ren Mai and Du Mai to retract.
Commentary: This formula can treat Kidney Deficiency, lower back pain, and enuresis. Do not neglect it lightly.
Rhyme:
After childbirth, a fleshy line emerges from the water passage,
Touching it adds to the pain.
This is due to the deficiency and collapse of the Dai Mai,
The Ren Mai and Du Mai lack strength and cannot return.
Liang Shou Tang uses Ren Shen, Bai Zhu, and combined with Shan Yao,
Cooked Shu Di, Qian Shi, and Bian Dou.
Du Zhong, Shan Yu, Ba Ji Rou, Chuan Xiong, and Bai Guo are decocted together.
After childbirth, a mass hangs down from the vulva, shaped like a handkerchief, sometimes with corners or bifurcated. People assume it is uterine prolapse (Chan Tui), but who would know this is due to Liver Atrophy (Gan Wei)? Why does Liver Atrophy occur after childbirth? It is likely due to overexertion before childbirth, combined with emotional disturbance and anger, causing the Liver to fail to store blood. Excessive blood loss leads to the Liver's fatty membrane descending with the blood. Its shape resembles the uterus, but it is not the uterus. If the uterus were prolapsing, it would look like an eggplant and stop at the vaginal opening, unable to extend beyond it. However, the Liver's fatty membrane often extends outside the vaginal opening, reaching six or seven cun, and sometimes a piece dries and falls off, sticking to the mat, as large as a palm. If the uterus were truly prolapsed, the patient would die instantly, so how could she survive? The treatment method should heavily tonify Qi and Blood, with a small addition of lifting herbs. Then Liver Qi will flourish and grow easily, Liver Blood will flourish and nourish easily. With the power of growth and nourishment, the fatty membrane will naturally retract.
Prescribe Shou Mo Tang (Membrane Retaining Decoction).
Sheng Huang Qi (Astragali Radix) - 1 liang
Ren Shen (Ginseng Radix) - 5 qian
Bai Zhu (Atractylodis Rhizoma) - 5 qian, stir-fried with soil
Dang Gui (Angelicae Sinensis Radix) - 3 qian, washed with wine
Sheng Ma (Cimicifugae Rhizoma) - 1 qian
Bai Shao (Paeoniae Radix Alba) - 5 qian, stir-fried with wine until scorched
Decoct in water and take orally. One dose results in retraction. Some doubt the use of Bai Shao postpartum, fearing it damages the source of vital Qi. Why does frequent use achieve success? This is the sigh of those who have not read Zhang Zhongjing's works! The reason Bai Shao should not be used frequently postpartum is to avoid retaining blood stasis; claiming it damages the source of vital Qi is mistaken. Moreover, for diseases located in the Liver, it is especially indispensable. Furthermore, when used within a formula heavily tonifying Qi and Blood, Bai Shao forgets its sour-astringent nature, so what harm could it do? Besides, for the prolapse of the fatty membrane, the sour-astringent power of Bai Shao is precisely needed to assist Sheng Ma in lifting Qi and Blood, which explains the rapid efficacy.
Commentary: Retaining the Liver membrane relies entirely on the merit of Bai Shao; charcoal form should not be used.
Rhyme:
A mass grows in the vulva after childbirth,
Its shape is like a handkerchief or horn-shaped.
The Liver's blood membrane descends with the blood,
The disease name is Liver Atrophy; tonifying Qi is effective.
Shou Mo Tang uses Ren Shen,
Combined with Bai Zhu, Bai Shao, and Dang Gui.
Sheng Ma lifts, dosage should be small,
One dose achieves miraculous results.
