Due to postpartum fatigue and overexertion damaging the Spleen Qi, resulting in sluggish transportation and transformation (Yunhuan), this condition is named Sundry Diarrhea (Sun Xie), characterized by undigested food in the stool; similarly, excessive dietary intake injuring the Spleen and Stomach yields the same result, commonly known as Water-Grain Dysentery. However, within the first three days postpartum, when lochia clots have not yet been digested and expelled, given the weakness of the Spleen and Stomach, tonics such as Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) should not be administered hastily. Instead, one should initially take Modified Shenghua Decoction (Jiawei Shenghua Tang) with added Alpinia Oxyphylla Fruit (Yizhi) and Amomi Fructus Rotundus (Xiangsha) to gently warm the Spleen Qi. Once the clots dissipate, add Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) to tonify Qi, Myristicae Semen (Rouguo), Aucklandiae Radix (Muxiang), and Amomi Fructus Rotundus (Sharen) to warm the Stomach, Cimicifugae Rhizoma (Shengma) and Bupleuri Radix (Chaihu) to clear Stomach Qi, and Alismatis Rhizoma (Zexie), Poria (Fuling), and Citri Reticulatae Pericarpium (Chenpi) to promote water metabolism. This constitutes the superior strategy.
Modified Shenghua Decoction (Jiawei Shenghua Tang): Treats postpartum sundry diarrhea within three days where clots have not yet dissipated.
Ligustici Chuanxiong Rhizoma (Chuanxiong) [1 qian], Alpinia Oxyphylla Fruit (Yizhi) [1 qian], Angelicae Sinensis Radix (Danggui) [4 qian], Zingiberis Rhizoma Carbonisatum (Heijiang) [0.4 qian], Glycyrrhizae Radix et Rhizoma Praeparata cum Melle (Zhigancao) [0.4 qian], Persicae Semen (Taoren) [10 grains], Poria (Fuling) [1.5 qian].
Marginal Note: Another version uses Danggui 3 qian and adds one Jujubae Fructus (Zao).
Ginseng-Poria Modified Shenghua Decoction (Canling Shenghua Tang): Treats cases where clots have dissipated but digestion remains poor within three days postpartum, or in patients with inherent constitutional weakness prior to pregnancy who suffer from this disease.
Ligustici Chuanxiong Rhizoma (Chuanxiong) [1 qian], Angelicae Sinensis Radix (Danggui) [2 qian], Zingiberis Rhizoma Carbonisatum (Heijiang) [0.4 qian], Glycyrrhizae Radix et Rhizoma Praeparata cum Melle (Zhigancao) [0.5 qian], Ginseng Radix et Rhizoma (Renshen) [2 qian], Poria (Fuling) [1 qian], Paeoniae Radix Alba (Baishao) [1 qian, stir-fried], Alpinia Oxyphylla Fruit (Yizhi) [1 qian, stir-fried], Atractylodis Macrocephalae Rhizoma (Baizhu) [2 qian, soil-fried], Myristicae Semen (Rouguo) [1 fruit, processed].
If diarrhea is profuse, add Alismatis Rhizoma (Zexie) and Akebiae Caulis (Mutong) 0.8 fen each; if abdominal pain occurs, add Amomi Fructus Rotundus (Sharen) 0.8 fen; if thirsty, add Ophiopogonis Radix (Maidong) and Schisandrae Fructus (Wuweizi); if cold-type diarrhea, add Zingiberis Rhizoma Carbonisatum (Heijiang) 1 qian and Aucklandiae Radix (Muxiang) 0.4 fen; for food stagnation, add Massae Medicatae Fermentatae (Shenqu) and Hordeum Fructus Germinatus (Maiya) to digest rice/noodles, and Amomi Fructus Rotundus (Sharen) plus Crataegi Fructus (Shanzha) to digest meat. For prolonged postpartum diarrhea/dysentery leading to weak Stomach Qi and sundry diarrhea, it is appropriate to warm and assist Stomach Qi using Six-Ingredient Gentlemen's Decoction (Liujunzi Tang) with added Aucklandiae Radix (Muxiang) 0.4 fen and Myristicae Semen (Rouguo) 1 fruit (processed).
Marginal Note: Another version includes eight Lotus Seeds (Lianzi, heart removed) and three Jujubae Fructus (Zao).
Rhyme Verse:
Within three days postpartum, if clots have not dispersed,
Fatigue injures the Spleen, making transformation difficult.
Excessive eating may damage Spleen and Stomach,
Sundry diarrhea manifests with undigested food.
Modified Shenghua Decoction contains Chuanxiong, Yizhi, Danggui, and Fuling.
Heijiang, Taoren, and Zhigancao, taken for several doses stops the diarrhea.
For strengthening the Spleen, use Canling Shenghua Tang,
Comprising Danggui, Chuanxiong, Zhigancao, Rouguo, and Jiang.
Baishao, Baizhu, and Yizhiren,
This formula is suitable for those with inherent weakness prior to pregnancy.
