A parturient woman immediately experiences fainting and aphasia after the delivery of the infant. This is a syndrome of simultaneous depletion of Qi and Blood (Qi and Blood exhaustion), which is originally considered incurable; however, if treated with the correct method, survival is possible. At this moment, urgently puncture the center of the eyebrows (Yintang, EX-HN3) with a silver needle. Once blood flows out, speech will return. Subsequently, decoct one tael of Renshen (Ginseng Radix et Rhizoma) in water and administer it orally; there is no case that cannot be saved. Alternatively, use two taels of Huangqi (Astragali Radix) and one tael of Danggui (Angelicae Sinensis Radix), named Danggui Buxue Tang (Dong Quai Decoction for Tonifying Blood), decoct one bowl and administer it to save the life. Under no circumstances should Fuzi (Aconiti Lateralis Radix Praeparata) be lightly added to either of these two formulas. This is because Fuzi penetrates all meridians without restriction; its nature is moving and unconsolidating, which would instead guide the Qi- and Blood-tonifying drugs to wander everywhere rather than focusing on the uterus. It is inferior to Renshen, Huangqi, and Danggui, which can directly rescue the collapse of Qi and Blood, gathering them without dispersal. The fainting of parturient women is entirely due to the emptiness of the Blood Chamber (uterus), which fails to nourish the Heart, leading to fainting. The tongue is the sprout of the Heart; since the Heart has lost its mastery, how can the tongue produce sound? The acupoint at the center of the eyebrows connects upward to the brain and downward to the tongue, while its meridian connection links to the Heart. Puncturing the center of the eyebrows allows both the brain and tongue to become unobstructed, causing the clear Qi of the Heart to ascend, thereby naturally descending the stasis. Then, by administering decoctions of Renshen, Huangqi, and Danggui—which are capable of tonifying Qi and generating Blood—Qi and Blood will reconnect, preventing death. Although using Renshen, Huangqi, and Danggui alone can also save lives, it is ultimately not as effective as first puncturing the center of the eyebrows. People only know the method of moxibustion on the center of the eyebrows, not knowing that needling is superior to moxibustion. Moxibustion is slow, while needling is rapid; slowness makes it difficult to rescue critical conditions, whereas rapidity facilitates revival. This illustrates the principle: "In acute cases, treat the branch (symptoms); in chronic cases, treat the root (cause)."
Rhyme:
The infant has already been bathed in the fragrant basin,
Suddenly the mother faints and speaks no words.
Simultaneous depletion of Qi and Blood is a critical condition,
Puncturing the center of the eyebrows can provide emergency relief.
Blood flowing from the center of the eyebrows dispels dizziness,
One tael of Ginseng quickly decocted and swallowed.
Or take Danggui Buxue Tang (Dong Quai Decoction for Tonifying Blood),
To reconnect Qi and Blood, preserving life.
A woman, two or three days postpartum, experiences fever, retention of lochia, and attack of toxic blood on the Heart. She speaks deliriously, shouts loudly, desires to run about, and cannot be restrained. Others attribute this to pathogenic heat in the Stomach, but who knows it arises from Blood deficiency failing to nourish the Heart? Postpartum Blood has all exited with the placenta, leaving the Blood Chamber empty. All Zang-Fu organs lack Blood nourishment, except for a trace amount remaining in the Heart to protect the Heart Monarch. Since the Zang-Fu organs lose their nourishment, they all seek supply from the Heart. The Pericardium acts as the Prime Minister of the Heart Monarch, blocking the Qi of various Zang-Fu organs from entering the Heart, thus allowing the Spirit to remain calm. Protecting the Heart relies entirely on the strength of the Pericardium. If the Pericardium is also deficient and cannot shield the Heart, the Qi of various Zang-Fu organs enters the Heart directly to consume Heart Blood. The Pericardium becomes anxious, unable to care for the Monarch internally or defend against the organs externally. Consequently, it cries out loudly, and its manifestations resemble madness, creating a helpless situation. Thus, although the symptoms appear like Heat, it is not actually Heat. The treatment must greatly tonify the Blood in the Heart, allowing the various Zang-Fu organs to draw upon it for self-nourishment, so they no longer disturb the Heart. Then the Heart remains peaceful, and the Pericardium is also tranquil.
Prescribe Anxin Tang (Heart-Calming Decoction).
Danggui (Angelicae Sinensis Radix) (2 liang)
Chuanxiong (Ligustici Chuanxiong Rhizoma) (1 liang)
Shengdi (Rehmanniae Radix) (5 qian, stir-fried)
Danpi (Moutan Cortex) (5 qian, stir-fried)
Sheng Puhuang (Typhae Pollen) (2 qian)
Gan Heye (Nelumbinis Folium) (1 leaf, as a guiding herb)
Decoct in water and take orally. After one dose, the mania subsides, and lochia is discharged. This formula uses Chuanxiong and Danggui to nourish Blood; why also use Shengdi and Danpi to cool Blood, which seems unsuitable for postpartum? One must understand that the reason lochia rushes to the Heart is due to deficiency-Heat attacking. Cooling within tonification causes no harm. Moreover, adding Heye, which connects the seven orifices, guides pathogenic factors outward. Not only does it prevent internal damage to the Heart, but it also assists Puhuang in resolving lochia. However, it can only be used temporarily to settle mania; excessive use may cause adverse effects. Be cautious.
Marginal Note: After the mania subsides following medication, it is advisable to take Jiawei Shenghua Tang (Modified Decoction for Generating New Blood): Danggui (Washed with wine) 1 liang 1 qian, Chuanxiong 3 qian, Taoren (Prunus Persica Seed, crushed) 1.5 qian, Jingjie Sui (Schizonepetae Spica, charred) 1 qian, Danpi 1.5 qian. Take four doses for excellent results.
Rhyme:
Within two or three days postpartum,
Lochia fails to discharge, Blood attacks upward.
Delirious speech, shouting, even running about,
Illness arises from the emptiness of the Blood Chamber.
Anxin Tang contains Danggui and Chuanxiong,
Shengdi, Danpi, and Puhuang taken together.
A small amount of Heye opens the seven orifices,
Small doses yield efficacy; excessive doses are futile.
