Due to severe blood loss and exhaustion, Qi loses its foundation of support, leading to rapid breathing or even cessation of respiration, deviating from normal physiological patterns. Some mistakenly attribute this to Phlegm-Fire (Tan Huo, phlegm-heat syndrome) and inappropriately administer formulas that disperse Qi and resolve phlegm, which can be fatal. The correct treatment should focus on significantly tonifying Blood and Qi. If there are blood stasis masses (Yu Xue Kuai), Ren Shen (Ginseng Radix et Rhizoma) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) must not be used; if there are no masses, this formula may be used, removing Tao Ren (Persicae Semen) and adding Shu Di Huang (Rehmanniae Radix Preparata) and one slice of Fu Zi (Aconiti Lateralis Radix Preparata). If the feet are cold, add one qian of prepared Fu Zi, along with Ren Shen, Bai Zhu, and Chen Pi (Citri Reticulatae Pericarpium), followed by Bu Qi Yang Rong Tang (Tonify Qi and Nourish Blood Decoction).
Jia Shen Sheng Hua Tang (Ginseng-Added Birth-Expelling Decoction): Treats shortness of breath occurring immediately after childbirth. If blood stasis masses are present, do not add Bai Zhu.
Ingredients: Chuan Xiong (Ligustici Chuanxiong Rhizoma, 2 qian), Dang Gui (Angelicae Sinensis Radix, 4 qian), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma, 5 fen), Hei Jiang (Scorched Zingiberis Rhizoma, 4 fen), Tao Ren (Persicae Semen, 10 grains, skin and tips removed, crushed), Ren Shen (Ginseng Radix et Rhizoma, 2 qian).
Administration: Use one Jujube (Ziziphi Spinosae Fructus) as a guide, continue for two to three doses, then switch to the subsequent formula.
Bu Qi Yang Rong Tang (Tonify Qi and Nourish Blood Decoction): Treats shortness of breath and rapid breathing postpartum. Suitable if blood stasis masses are painless.
Ingredients: Huang Qi (Astragali Membranacei Radix, 1 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 1 qian), Dang Gui (Angelicae Sinensis Radix, 4 qian), Ren Shen (Ginseng Radix et Rhizoma, 3 qian), Chen Pi (Citri Reticulatae Pericarpium, 4 fen), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma, 4 fen), Shu Di Huang (Rehmanniae Radix Preparata, 2 qian), Chuan Xiong (Ligustici Chuanxiong Rhizoma, 2 qian), Hei Jiang (Scorched Zingiberis Rhizoma, 4 fen).
Modifications: If hands and feet are cold, add one qian of prepared Fu Zi; if sweating is excessive, add one qian of Ma Huang Gen (Ephedrae Herba Cum Radice) and a small pinch of Fu Mai (Tritici Levis Fructus); if thirsty, add one qian of Mai Dong (Ophiopogonis Radix) and ten Wu Wei Zi (Schisandrae Chinensis Fructus); if constipated, add one qian of Rou Cong Rong (Cistanches Herba) and a pinch of Ma Ren (Semen Cannabis); if food stagnation involves wheat products, add one qian of Chao Shen Qu (Fomentatio Fermentata) and one qian of Chao Mai Ya (Hordei Fructus Germinatus); if food stagnation involves meat, add five fen each of Shan Zha (Crataegi Fructus) and Sha Ren (Amomi Fructus Rotundus).
Marginal Note: Mai Ya (Hordei Fructus Germinatus) has the adverse effect of suppressing lactation; use with caution!
Note: In some versions, Huang Qi and Bai Zhu are both two qian. When using Fu Mai to stop sweating, it should be stir-fried.
Verse:
Postpartum blood loss combined with fatigue, Qi unsupported leads to shortness of breath resembling asthma.
Mistakenly dispersing Qi and resolving phlegm endangers life; significantly tonifying Blood and Qi brings peace.
Jia Shen Sheng Hua Tang contains Chuan Xiong, Dang Gui, and Ren Shen, with Prepared Licorice, Scorched Ginger, and Stir-fried Peach Kernel.
For acute shortness of breath resembling asthma, administer promptly; guiding with Jujube yields miraculous effects.
Bu Qi Yang Rong Tang uses Ren Shen and Huang Qi, plus Dang Gui, Chuan Xiong, Bai Zhu, and Chen Pi.
With Prepared Licorice, Scorched Ginger, and Prepared Rehmannia, this formula is suitable when blood stasis masses are painless.
