After childbirth, Qi and Blood suddenly become severely deficient. The entire body lacks blood to nourish the bones and muscles, leading to sudden symptoms such as lockjaw (trismus), rigidity of the teeth and jaws, and spasmodic twitching of the tendons and vessels in the hands and feet. These conditions resemble wind-stroke, epilepsy, or tetanus. Although they may be caused by deficiency fire floating upward accompanied by phlegm, treatment should fundamentally focus on tonification. One must not adhere to biased or heterodox methods using wind-dispelling and phlegm-resolving formulas, which would further deplete the postpartum woman's already weak constitution. The treatment method should first involve administering Shenghua Tang (Life-Building Decoction) to generate and invigorate new blood. If critical symptoms are observed, after three doses, Ginseng Radix (Renshen) should be added to rescue from blood collapse by tonifying Qi. If there is phlegm-heat, small amounts of Tangerine Peel (Juhong) and Stir-fried Scutellaria Root (Chao Qin) may be added; Bamboo Sap (Zhuli) and Ginger Juice (Jiangzhi) can also be included. However, Phellodendron Bark (Huangbai) and Coptis Rhizome (Huanglian) must never be used together; caution is advised!
Zirong Huoluo Tang (Nourishing Blood and Unblocking Collaterals Decoction): Treats postpartum conditions resembling wind-stroke, characterized by lockjaw, neck stiffness, and tendon spasms due to blood deficiency.
Ingredients: Chuanxiong Rhizome (Chuanxiong, 1.5 qian); Angelica Sinensis Radix (Danggui), Prepared Rehmannia Root (Shudi), Ginseng Radix (Renshen) (each 2 qian); Astragalus Membranaceus Radix (Huangqi), Poria (Fushen), Gastrodia Elata Rhizome (Tianma) (each 1 qian); Glycyrrhiza Uralensis Radix et Rhizoma (Zhi Cao), Citrus Reticulata Pericarpium (Chenpi), Schizonepeta Spike (Jingjie Sui), Saposhnikovia Root (Fangfeng), Notopterygium Rhizome et Radix (Qianghuo) (each 4 fen); Coptis Chinensis Rhizoma (Huanglian, 8 fen, stir-fried with ginger juice).
Modifications: For phlegm, add Bamboo Sap (Zhuli), Ginger Juice (Jiangzhi), Pinellia Tuber (Banxia); for thirst, add Ophiopogon Tuber (Maidong), Kudzu Root (Gegen); for food stagnation, add Hawthorn Fruit (Shanzha) and Amomum Fruit (Sharen) to digest meat, and Medicated Leaven (Shenqu) and Hordeum Germinatum (Maiya) to digest grains; for constipation, add Cistanche Stem (Roucongrong) 1.5 qian; for excessive sweating, add Ephedra Root (Mahuang Gen) 1 qian; for palpitations and fright, add Ziziphus Seed (Zaoren) 1 qian.
Tianma Wan (Gastrodia Pill): Treats postpartum wind-stroke presenting with mental confusion, slurred speech, and impaired movement of the four limbs.
Ingredients: Gastrodia Elata Rhizome (Tianma, 1 qian); Saposhnikovia Root (Fangfeng, 1 qian); Chuanxiong Rhizome (Chuanxiong, 7 fen); Notopterygium Rhizome et Radix (Qianghuo, 7 fen); Ginseng Radix (Renshen), Polygala Tuber (Yuanzhi), Biota Seed (Baiziren), Dioscorea Rhizome (Shanyao), Ophiopogon Tuber (Maidong) (each 1 qian); Ziziphus Seed (Zaoren, 1 liang); Asarum Root (Xixin, 1 qian); Arisaema Cum Bile (Nanxing Qu), Acorus Tatarinowii Rhizome (Shichangpu, 1 qian).
