For those of high status who live in leisure and ease, whose nourishment is excessive, and who suffer from damage due to alcohol and food, resulting in retained fluid in the Zhongwan (CV12) region with bloating and oppression, epigastric fullness, diaphragmatic obstruction, and acid regurgitation, Muxiang Dao Yin Wan (Aucklandia Guiding Drink Pill) can be taken to treat these conditions.
For laborers whose diet is coarse, clothing thin and cold, and who endure heavy physical exertion, suffering from all types of damage due to alcohol and food, resulting in abdominal fullness and oppression, and frequent vomiting of sour fluid, Shijin Wan (Appetite-Boosting Pill) can be used for treatment.
For various cases of internal water retention and qi stagnation with edema, presenting with stabbing pain in both hypochondria, abdominal fullness preventing eating, dizziness, and vertigo, Chatao San (Tea-Regulating Powder) can be used to induce mild emesis. After expelling one or two sheng (approx. 0.5-1 liter) of cold phlegm-fluid, subsequent administration of Qixuan Wan (Seven-Circulation Pill) will lead to recovery. Muxiang Binglang Wan (Aucklandia and Areca Nut Pill) and Dao Yin Wan (Guiding Drink Pill) are also excellent choices. Harsh drugs such as Crotonis Fructus (Badou, 巴豆) and Mercury preparations should not be used.
For various accumulations that do not resolve, Wuyou San (Carefree Powder) can be taken, inducing bowel movements three to five times per month. Guiling Baizhu Wan/San (Cinnamon, Poria, and Atractylodes Macrocephala Pill/Powder) and Miaogong Wan (Wonderful Merit Pill) may also be employed. It is strictly forbidden to consume raw, hard, sticky, slippery foods, or substances that stir wind and generate heat.
Whether male or female, if one suffers from Bone Steaming Heat-Lao (a chronic consumptive disease characterized by yin deficiency and internal heat), presenting with withered and dry skin, thick and sticky sputum, pain in the four limbs, red face, dry lips, irritability, restless sleep, occasional panting and coughing, loss of taste in food, fatigue, weakness, spontaneous sweating, yellowish complexion, and emaciation, the *Neijing* states: "In men, it arises from insufficiency of essence; in women, from stagnation of blood." Initially, Chatao San (Tea-Regulating Powder) should be used to induce mild emesis; subsequently, Daoshui Yugong San (Water-Guiding Yu Gong Powder) should be administered to induce mild purgation, allowing the bowels to move three to five times; thereafter, medications such as Chaihu Yinzi (Bupleurum Decoction), Guiling Ganlu San (Cinnamon, Poria, and Sweet Dew Powder), Sofeng Wan (Wind-Searching Pill), Baizhu Tiao Zhong Tang (Atractylodes Regulating Middle Decoction), Muxiang Binglang Wan (Aucklandia and Areca Nut Pill), and Renshen Xijiao San (Ginseng and Rhinoceros Horn Powder) should be selected based on the patient's deficiency or excess. If hemoptysis, hematemesis, or hematochezia occurs, this indicates blood loss, and emesis is contraindicated, as it may cause confusion of the spirit. The *Neijing* states: "Blood is the substance of the human spirit." Therefore, those who have lost blood should not be subjected to emesis. One must absolutely avoid administering harsh, hot drugs such as Ginger (Shengjiang, 生姜) and Aconite (Fuzi, 附子). If taken, they will make food intake difficult, cause muscle wasting, and ultimately lead to a critical condition. The pathology of Wu Lao (Five Exertions/Labor diseases) often results from modern practitioners' failure to understand the principles of releasing the exterior and purging the interior. It is strictly forbidden to apply moxibustion on the wrists and above the lateral malleolus during the summer months. The wrist point, Yangchi (TE4), is an area where muscles are thin, and moxa sores here are the hardest to heal. Some patients undergo dozens of moxibustion sessions on points such as the chest, Zhongwan (CV12), below the umbilicus, back-Shu points, and Zusanli (ST36), or even use burning needles, yet achieve no effect, only causing suffering to the patient. Is this not worth deep reflection? Patients with lao diseases often have strong appetites and cravings. Any edible item should be provided according to the recommendations of *Shiliao Bencao* (Food Therapy Materia Medica). Spinach, mallow soup, ice water, and cool foods should never be prohibited, as food and fluids must enter the stomach to allow the pulse channels to flow. If restrictions are too severe, the stomach gate will close; if the stomach gate closes, the body will inevitably become emaciated; if the body is emaciated and the pulse is empty, this is a sign of death. All types of lao diseases can be managed following this principle.
