Pediatric Yin Jaundice is formed by the dual deficiency of Spleen Dampness and Kidney Cold; this is a most critical condition. If treating Yin Jaundice by warming the Spleen, ancient methods primarily prescribe Lizhong Tang (Replenish the Middle Decoction) with added Yinchen Hao (Artemisiae Scopariae Herba).
If treating Yin Jaundice by warming the Kidney, ancient methods primarily prescribe Yinchen Sini Tang (Artemisiae Scopariae and Aconite Root Decoction) (containing ingredients such as Zhi Fuzi (Prepared Aconiti Lateralis Radix), Bei Ganjiang (Dried Ginger Rhizome), Yinchen Hao (Artemisiae Scopariae Herba), and Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma)).
[True's Note] The syndrome of Yin Jaundice is entirely distinct from Damp-Heat; it stems fundamentally from the collapse of Qi and Blood. Since Qi fails to generate Blood, Blood collapses; since Blood fails to nourish the complexion, the complexion deteriorates. Any case of jaundice presenting without any Yang syndrome signs or Yang pulse manifestations is classified as Yin Jaundice. This may occur after a severe illness or due to long-term depletion of the Spleen and Stomach, causing the color of Spleen Earth to manifest externally on the body. Symptoms invariably include a preference for stillness over activity, a preference for dimness over brightness, mental fatigue, faint speech, aversion to cold, reduced appetite, weakness in the four limbs, possibly loose stools, and urine resembling paste. These are all heavy signs of Yang deficiency, standing in stark contrast to Yang Jaundice, like ice and charcoal. The prescription consists of Chao Dangshen (Stir-fried Codonopsis Radix) 1.5 qian, Sheng Baizhu (Raw Atractylodis Macrocephalae Rhizoma) 2 qian, Bai Biandou (White Lablab Seed) 1 qian, Hua Juhong (Exocarpium Citri Reticulatae Viride) 0.5 qian, Pao Jiangtan (Charred Ginger Rhizome) 0.5 qian, Bai Guishen (Angelicae Sinensis Radix) 0.8 qian, and Zhi Gancao (Honey-fried Glycyrrhizae Radix et Rhizoma) 0.6 qian. Add one and a half cups of water, decoct to seven-tenths volume, and administer.
Phlegm is originally transformed from food and drink. Why does the transformed substance become Phlegm? Because the generated Jin-Ye (Body Fluids) fail to distribute evenly throughout the body, stagnating in the chest and transforming into turbid fluid; thus, medical texts often attribute the responsibility to the Spleen. It is stated that when Spleen Qi is clear and pure, Phlegm is not generated; when Spleen Qi is turbid and mixed, Phlegm forms. Pediatric Phlegm mostly arises from irregular breastfeeding or feeding, or excessive consumption of rich and greasy foods, leading to the Spleen and Stomach's inability to transport and transform. If the child possesses abundant Yin Qi naturally, it transforms into Yin (Fluid Retention); if the child possesses abundant Yang Qi naturally, it transforms into Phlegm. Medical texts also state that the branch of Phlegm lies in the Spleen, while its root lies in the Kidney. This is because the Spleen belongs to the Postnatal Foundation, and the Kidney belongs to the Prenatal Foundation. Any disease of the Postnatal Foundation inevitably has its roots in the Prenatal Foundation. Superficial Phlegm should be dispersed, Heat Phlegm should be vomited out, Excess Phlegm should be purged downward, and Cold Phlegm should be dried. Although Phlegm changes in myriad ways, in children, it is only divided into two types: Dry Phlegm and Damp Phlegm. This is the simplest and most essential effective method for treating pediatric Phlegm syndromes.
Pediatric Dry Phlegm occurs when Phlegm is stirred by Fire. When Fire is excessive, Phlegm becomes abundant and sticky, Qi rebels causing wheezing and coughing, restless sleep at night, red face, dry mouth, and yellow-red urine. For mild cases, ancient methods primarily prescribe Qingqi Huatan Wan (Clear Qi and Resolve Phlegm Pill) (containing Dan Nanting (Arisaematis Cum Bile), Banxia (Pinelliae Rhizoma), Juhong (Exocarpium Citri Reticulatae Viride), Zhishi (Aurantii Fructus Immaturus), Xingren (Prunus Armeniacae Semen), Gualouren (Trichosanthis Semen), Huangqin (Scutellariae Radix), and Bai Fuling (Poria), ground into powder, formed into pills with Ginger Juice, and taken with Ginger Soup). For severe cases, ancient methods primarily prescribe Su Ting Gun Tan Wan (Perilla and Lepidium Rolling Phlegm Pill) (containing Suzi (Perillae Fructus), Ku Tingli (Lepidii Semen), Dahuang (Rhei Radix et Rhizoma), Chenxiang (Aquilariae Lignum Resinatum), Huangqin (Scutellariae Radix), and Qing Mengshi (Calcaris Calcitae)), formed into pills using water suspension, administered according to the child's excess or deficiency status, delivered with Ginger Soup.
[True's Note] Dry Phlegm is necessarily viscous and difficult to expectorate, involving substances like Nanting, Banxia, and Ginger Juice. One must worry that the accumulation will become harder and harder, hence Tingli, Mengshi, and Chenxiang are used together to break up the accumulation. One must also worry that aggressive breaking will weaken the patient further, so only Ku Xingren (Bitter Apricot Seed) 2 qian, Gualouren (Trichosanthis Semen) 1.5 qian, Chuan Beimu (Fritillariae Cirrhosae Bulbus) 1 qian, Xin Zhuru (Fresh Bamboo Shavings) 1.5 qian, Fu Chao Zhiqiao (Bran-fried Aurantii Fructus) 0.8 qian, and Laifuzi (Radish Seed) 0.5 qian are used, with Zhuli (Bamboo Sap) 10 drops as the guiding agent. In severe cases, add Tian Tingli (Sweet Lepidium Seed) 0.3 qian to this formula.
