Dysentery due to latent summerheat persisting for a long time without cessation, accompanied by prolapse of the anus and sagging of the abdomen, soreness and weakness of the lower back, atrophy and weakness of the legs, and abdominal distension after eating, indicates damage to both the Spleen and Kidney with Qi deficiency leading to sinking of the middle Qi. It is appropriate to use Jingyue Buyin Yiqi Decoction (Zhang Jingyue's Yin-Nourishing and Qi-Boosting Decoction), adding Eucommiae Cortex (Duzhong) and Cuscutae Semen (Tusizi) among other herbs, to raise Yang Qi and secure Yin fluids.
Although dysentery originates from Spleen pathology, prolonged duration inevitably injures the Kidney. The current symptoms of anal prolapse, lumbar soreness, leg atrophy, and postprandial distension are due to Kidney deficiency failing to grasp and retain, and Spleen deficiency failing to transport and transform. If both organs are not treated simultaneously, recovery will be difficult. Therefore, Ren Shen (Ginseng Radix et Rhizoma), Gouqi (Lycii Fructus), Baizhu (Atractylodis Macrocephalae Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma) in Jingyue Buyin Yiqi Decoction are used to strengthen the Spleen; Shudi (Rehmanniae Radix Preparata), Shanyao (Dioscoreae Rhizoma), Danggui (Angelicae Sinensis Radix), and Duzhong (Eucommiae Cortex) are used to tonify the Kidney; while Shengma (Cimicifugae Rhizoma) and Chaihu (Bupleuri Radix) are added to ascend clear Yang Qi, allowing Yang to rise and Yin to be retained, thereby stopping the dysentery.
Prolonged dysentery from latent summerheat may cease intermittently or recur, with varying stool colors, yellowish complexion, and abdominal distension; this is termed "Rest Dysentery" (Xiuxi Li). It is appropriate to use Mi Zhongchun's Pishen Shuangbu Wan (Spleen-Kidney Dual Tonifying Pill) to tonify both the Pre-heaven and Post-heaven foundations.
Prolonged dysentery presenting with a sallow complexion and abdominal distension already reveals signs of Spleen deficiency. If it continues to fluctuate between remission and exacerbation, it indicates not only Spleen Qi deficiency with sinking but also a lack of grasping power in the Kidneys. Without using Mi Zhongchun's Pishen Shuangbu Wan to tonify both the Pre-heaven and Post-heaven sources, the Spleen and Kidney deficiency will persist, and the dysentery will not cease.
Dysentery from latent summerheat presenting with bloody stools, fever, thirst, abdominal pain with urgency and tenesmus, burning sensation in the anus, and a rapid, wiry, and slippery pulse indicates rushing damp-fire. It is appropriate to use Baitouweng Tang (Pulsatilla Decoction), utilizing bitter flavors to firm the Yin.
This pattern involves heat forcing downward flow and evil fire consuming fluids, resulting in abdominal pain with bearing down, extreme urgency, and frequent sensations of tenesmus. Additionally, the anus feels as if scorched, accompanied by fever, thirst, and a slippery-rapid pulse. Unless Baitouweng Tang is used for its bitter nature to firm the Yin and cold nature to clear heat, the dysentery will not stop. If the heat is severe, add Dahuang (Rhei Radix et Rhizoma) to purge accumulation; if the heat is mild but the pulse is deficient and rapid, with a dry tongue lacking fluids, dry throat, and dry mouth, indicating Yin deficiency with blazing fire, Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction) should be used to nourish Yin and clear heat as the primary treatment.
