Huangdi asked Qibo, saying: I have heard that there are "Five Prohibitions" in acupuncture. What are the Five Prohibitions?
Qibo replied: They refer to prohibiting needling when it is inadvisable.
Huangdi said: I have heard that there are "Five Depletions" in acupuncture.
Qibo replied: One should not apply reducing methods to those who are already severely depleted.
Huangdi said: I have heard that there are "Five Excesses" (or Errors) in acupuncture.
Qibo replied: The application of reinforcing and reducing methods must not exceed proper limits.
Huangdi said: I have heard that there are "Five Rebellions" (or Contradictions) in acupuncture.
Qibo replied: When the disease pattern contradicts the pulse, it is called the Five Rebellions.
Huangdi said: I have heard that there are "Nine Appropriatenesses" in acupuncture.
Qibo replied: To thoroughly understand the theory of the Nine Needles is called the Nine Appropriatenesses.
Huangdi said: What are the Five Prohibitions? I wish to hear about the times when needling is contraindicated.
Qibo replied: On Jia and Yi days, when the Wood element is in its peak phase (Zicheng), do not needle the head, and do not apply the "Shaking off Dust" method (a sweating technique) to the ears.
On Bing and Ding days, when the Fire element is in its peak phase, do not apply the "Shaking off Dust" method to the shoulders, throat, or Lianquan (CV23).
On Wu and Ji days, when the Earth element is in its peak phase during the four seasons, do not needle the abdomen, and do not perform the "Removing Claws" method or water-draining techniques.
On Geng and Xin days, when the Metal element is in its peak phase, do not needle the joints of the thighs and knees.
On Ren and Gui days, when the Water element is in its peak phase, do not needle the lower legs and shins.
These are called the Five Prohibitions.
Huangdi said: What are the Five Depletions?
Qibo replied: When the body and muscles are already severely wasted, this is the first depletion.
After massive blood loss, this is the second depletion.
After profuse sweating, this is the third depletion.
After severe diarrhea, this is the fourth depletion.
After childbirth and massive bleeding, this is the fifth depletion.
Reducing methods must not be used in any of these cases.
Huangdi said: What are the Five Rebellions?
Qibo replied: In heat diseases, if the pulse is quiet and still, this is the first rebellion.
If the pulse becomes rapid and restless after sweating, this is the second rebellion.
In fixed Bi syndrome (painful obstruction due to dampness) with immobility, muscle erosion, body heat, and absent pulses on one side, this is the third rebellion.
When excessive sexual activity leads to physical wasting, accompanied by body heat, a sallow and dull complexion, followed by hematochezia with dark, heavy blood, this is called the fourth rebellion. (Old annotations define "Yin" as excessive sexual intercourse; however, I believe conditions such as intestinal discharge of pus and mucus, seminal emission, stranguria, and night sweats, where fluids and Qi scatter and fail to consolidate, also fall under this category.)
When alternating chills and fever lead to physical wasting, and the pulse is firm and bounding, this is called the fifth rebellion.
The text lists five items first and then narrates three, without any transitional explanation, making the chapter structure insufficient to discuss. Nevertheless, the writing style is rigorous and strict. Note: The details regarding the "Nine Appropriatenesses" can be found in the previous chapter "Guan Zhen" (Official Needles).
