After the eruption of smallpox (or chickenpox), if heat-toxin remains unresolved and affects the Small Intestine (Small Intestine, an organ responsible for separating clear from turbid fluids) causing urinary retention, it is appropriate to promote urination. If promoting urination alone does not lead to improvement, one should employ methods to raise clear Yang and resolve heat; once the upper orifices are unblocked, the lower orifices will naturally become patent. Even better is to clear the Lungs; when Lung Qi is clear, it can transmit fluids downward to the Bladder. Most importantly, nourishing Yin is crucial; when Kidney Yin is replenished, it can naturally facilitate water metabolism.

If the toxin affects the Large Intestine causing constipation, purgation is indicated. However, since the post-smallpox state involves deficiency, nourishing Blood is preferable; when the intestines receive adequate blood nourishment, stool passage will naturally resume.

Regarding diarrhea following smallpox, there are two main categories: Diarrhea accompanied by normal appetite indicates that pathogenic heat accelerates digestion ("killing grains"); thirst indicates that pathogenic heat scorches body fluids; a forceful pulse indicates internal heat; a forceful and rapid pulse indicates intense pathogenic heat. This constitutes diarrhea due to heat entering the Large Intestine, treated with clearing heat and promoting diuresis. Conversely, if appetite is poor, there is no thirst, and the pulse is fine and weak, this indicates deficiency of interior Qi failing to secure food and fluids. Even slight thirst in this context results from fluid depletion; treatment should involve modifying Lizhong Tang (Regulate the Middle Decoction). Given the significant Qi and Blood deficiency after smallpox, diarrhea is generally treated with warming and tonifying methods, applying gentle tonification to address excess symptoms (treating deficiency as if it were excess by supporting the root). However, for cases involving discharge of crusts, pus, and blood—indicating heat-toxin entering the Large Intestine causing dysentery-like discharge—Siwu Tang (Four Substances Decoction) added with Scutellariae Radix (Huangqin) and Coptidis Rhizoma (Huanglian) is suitable. The condition will resolve spontaneously after the pathogen is expelled; one must not use astringents to forcibly stop diarrhea.

Following the crusting stage, swelling and pain of the scrotum, resembling a melon, indicates heat binding in the Bladder. There are three treatment approaches: One may soothe the Liver to restore its function of free flow and excretion; one may clear the Lungs to restore their function of descending and transforming fluids; or one may directly treat the Bladder by clearing heat, dissipating bound Qi, and promoting diuresis. Differentiation between excess and deficiency is essential to select appropriate therapy. For excess patterns, treat the affected organ directly; for deficiency patterns, treat via the generating cycle (inter-organ treatment). Externally, fumigate and wash with a decoction of Zanthoxyli Pericarpium (Huajiao), Salt, Allii Cepae Bulbus (Congtou), and Kochiae Fructus (Difuzi) to guide the dispersion of bound Qi.

Vomiting after smallpox is mostly attributed to residual toxins in the Stomach, but presents as either cold or heat syndromes. Symptoms such as mental restlessness, thirst, urgent feeding, accumulation in the chest followed by projectile vomiting, reddish complexion, hot palms and soles, preference for cool environments, or simultaneous vomiting, thirst, and diarrhea, indicate heat-toxin. In contrast, immediate vomiting after milk/food intake, bluish-white complexion, cold extremities, clear urine and loose stools, vomiting without thirst, or diarrhea with cold palms and soles, indicate cold vomiting. Heat-toxin accounts for 70-80% of cases, while Stomach deficiency with cold accounts for 20-30%. Vomiting due to food stagnation is characterized by vomiting upon smelling food and inability to eat; Muxiang Da'an Wan (Aucklandia Peaceful Digestion Pill) is indicated. Polydipsia with immediate vomiting upon drinking water is termed "Water Rebellion" (Shuini), caused by pathogenic heat carrying retained fluid upward; Wuling San (Five Ingredient Powder with Poria) is the primary prescription.
