Zouma Gan (Running Horse Gangrene) is mostly caused by Qi deficiency with cold invasion, leading to stagnant heat accumulating internally; or due to long-term accumulation from consuming sweets, sour foods, salt, and greasy substances, causing accumulated heat to rush upward. This results in teeth becoming scorched, blackened, and rotting, occasionally bleeding clear blood. The blood gathers and turns into pus, which becomes foul-smelling and breeds "worms" (microbial erosion), eroding the mouth and teeth. In severe cases, it may perforate the cheeks, making breastfeeding and eating difficult, accompanied by asthma-like breathing and fever, leading to critical danger. The treatment method should involve internal administration of clearing and resolving agents to first remove accumulated heat. Externally, rinse with warm saline solution and insufflate medicines that eliminate saliva and draw out heat. One must absolutely avoid using purely cold-natured astringents, as this would prevent the stagnant heat from venting, causing deeper ulceration. It is named "Zouma" (Running Horse) because teeth belong to the Kidney. Once Kidney Qi is deficient, deficient fire and pathogenic heat rush directly to the Upper Jiao, moving with the speed of a running horse.
Toothache is primarily due to Stomach Fire in the Yangming channel. Internal administration of stomach-clearing formulas is suitable, along with external application of medicated powders to disperse fire pathogens. However, if there is Qi deficiency, insufficient Spleen and Stomach function, or excessive intake of cold-natured medicines suppressing Yang Qi within the Spleen Earth, resulting in contrary body fever, one should use Huoyu Tang (Fire-Stagnation Decoction) or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If ineffective, then Bayi Dihuang Tang (Eight-Ingredient Rehmannia Root Decoction) added with Niuxi (Achyranthes Root) and Wuweizi (Schisandra Fruit) should be used to guide the heat downward. This is because teeth are considered the surplus of bone and are mostly caused by deficient fire in the Kidney channel.
Treatment for Kougan (Oral Gangrene):
Niuhuang (Calculus Bovis, 1 fen), Renzhongbai (Calculus Ureae, 4 fen), Qingdai (Indigo Naturalis, 4 fen), Bingpian (Borneolum Syntheticum, 1 fen), Xiangya (Homo Sapiens Elephantis Dentis Cornu, calcined to ash, 4 fen), Zhenzhu (Margarita, roasted in milk, 7 grains), Baimati (Equus Hemionus Hipparionis Unguis, calcined to ash, 4 fen), Huhuanglian (Coptidis Rhizoma var. chinensis), Xuexie (Draconis Sanguis, each 4 fen), Shexiang (Moschus, a small amount). Grind into fine powder. Clean the affected area with cold, strong tea and insufflate.
Treatment for Yagan (Gum Gangrene) and Toothache:
Shengma (Cimicifugae Rhizoma, 6 fen), Shigao (Gypsum Fibrosum, 1 qian 2 fen), Lianqiao (Forsythiae Fructum, 1 qian), Shengdi (Rehmanniae Radix Praeparata, 1 qian 2 fen), Niubangzi (Arctii Fructum, 1 qian, ground), Danpi (Moutan Cortex, 8 fen), Jiegeng (Platycodonis Radix, 4 fen), Gancao (Glycyrrhizae Radix et Rhizoma, 3 fen), Jingjie (Schizonepetae Spica, 4 fen), Bohe (Menthae Haplocalycis Herba, 4 fen). Add Dengxincao (Junci Medullae) and decoct in water.
Qingdai (Indigo Naturalis), Lvhui (Aloe), Hulian (Coptidis Rhizoma var. chinensis), Huangbai (Phellodendri Cortex), Ercha (Camelliae Sinensis Folium processed), Pengsha (Borax, each 5 fen), Bingpian (Borneolum Syntheticum, 1 fen). Use cold, strong tea to clean the mouth and insufflate. This formula is extremely effective for treating pediatric Zouma Yagan (Pediatric Running Horse Gum Gangrene); without timely treatment, rapid necrosis leads to death.
Use Baigou (White scale from women's urinal, calcined, 1 qian), Tonglv (Chlorocopperum, 2 qian), Shexiang (Moschus, 1 fen), Bingpian (Borneolum Syntheticum), Niuhuang (Calculus Bovis, each 1 fen). Grind finely and apply to the affected area for immediate cure.
