Phlegm does not arise spontaneously but moves with Qi; Qi does not ascend spontaneously but rises due to Fire. After long-term accumulation, it adheres to the intestines and stomach, settling in convoluted and tortuous areas, forming nests where it resides. This is called "Old Phlegm." Its manifested symptoms are indescribable and cannot be treated by ordinary medicines. Therefore, Dahuang is used as the Sovereign herb to open the pathway downward; Huangqin as the Minister herb to suppress the flaming-upward Fire; Mengshi, with its fierce nature, travels through the intestines and stomach, tracking down to convoluted areas to sweep and cleanse them, thus serving as the Assistant; Chenxiang, capable of traversing the upper, middle, and lower jiao, opening the apertures of Feimen (anus) and Pomen (mouth/nose or sweat pores, here referring to facilitating excretion and Qi flow), serves as the Guide. However, efficacy depends strictly on proper administration: take a mouthful of water to wash the pill down the throat, then lie supine immediately, allowing the medicine to descend slowly in the throat and diaphragm area. Do not drink water or get up for half a day; avoid walking, sitting, standing, or speaking until the medicinal qi clears and expels the upper jiao Phlegm stagnation, after which normal activity may resume. Generally, after taking the medicine, if there is a sensation of thick, sticky obstruction in the throat, it is due to Phlegm-Qi rising and clashing with the medicinal force. After a short while, once the medicinal force prevails, peace will naturally return. If rapid purgation is desired, drink more plain warm water to induce it.
