The song says: "One qian plus of Peach Blossom Yin powder, mixed with Licorice decoction enhances its power; just two characters of Blue Flower, poured into the mouth eliminates it immediately."
Febrile syndromes vary, so various formulas cannot be fully listed; they are detailed in their respective sections.

When a person resides in a quiet and clean environment, Yang Qi is dense and pathogens cannot harm. However, if one labors excessively in affairs, Yang Qi dissipates, allowing pathogens to invade and harm the body. Seven emotions injure Qi, diet injures the form, Wind-Cold-Heat-Dampness injure Shen, diet and sexual activity injure Yin. Thus, the *Classic* states: "Pathogens arise either from Yin or from Yang." Those arising from Yang result from Wind, Cold, and Heat; those arising from Yin result from diet, residence, and emotional extremes. Therefore, febrile syndromes vary, and treatments differ; there is a distinct difference between external contraction and internal injury, and between cold, heat, tonification, and purgation. For example, Shanghan (Cold Damage) and Shangfeng (Wind Damage) are external contractions. Pathogens move from the exterior to the interior; it is appropriate to use exterior-releasing methods to disperse them. This is the principle behind Mahuang Tang (Ephedra Decoction) and Guizhi Tang (Cinnamon Twig Decoction). Because these occur on cold winter days and manifest immediately, they are called Shanghan, and medicinal materials should be pungent and hot to overcome the cold. If the season is not cold, medication should adapt to changes. In warm spring months, pungent-cool medications should be used; in hot summer months, sweet-bitter-cold medications should be used. It is also said: Winter Shanghan that does not manifest immediately becomes Wenbing (Warm Disease) in spring and Rebing (Heat Disease) in summer; treatment methods must vary with the four seasons. There is also a type of disease called Dongwen (Winter Warmth), described as having the disease out of season. Winter is cold, yet warmth disease occurs; this is due to improper heavenly timing where Yang Qi leaks abnormally, so medications should not be hot. There is also a type of seasonal cold epidemic occurring during warm times; although the season is warm, cold acts as the pathogen; this is also due to improper heavenly timing where Yin Qi rebels abnormally, so medications should not be cold. There is also Tianxing Wenyi (Heavenly Epidemic Warmth) heat disease, mostly occurring between spring and summer, affecting entire households similarly; this is Liqi (Miasma Qi) of Heaven and Earth. Treatment should follow the seasonal command and incorporate Yungui (Luck Theory), using Liu Wan-su’s (Hejian’s) pungent-cool, sweet-bitter-cold medications to clear heat and resolve toxicity. All the above syndromes are caused by external pathogens of Heaven and Earth. If it is dietary and fatigue-induced internal injury damaging Yuan Qi, the *Classic* states: "With labor and fatigue, physical Qi declines, and Grain Qi cannot withstand [the strain]." (Meaning: Stomach deficiency and insufficient Grain Qi cannot cope with labor.) "Upper Jiao fails to function" (Meaning: Clear Qi does not ascend). "Lower Wan is obstructed" (Meaning: Turbid Yin does not descend). "Stomach Qi becomes hot" (Because turbid Yin does not descend, Stomach Qi becomes hot). "Hot Qi steams the chest" (Minor fire becomes major fire and flares upward), thus causing internal heat. This belongs to True Yang sinking and generating internal deficient heat. Therefore, Li Dongyuan proposed the theory of Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), using Renshen (Ginseng Radix et Rhizoma) and Huangqi (Astragali Radix), sweet-warm medications, to greatly tonify Qi and lift the sinking Qi. This uses Qi-tonifying drugs to supplement Qi deficiency. Furthermore, if mental labor and excessive sexual activity internally injure True Yin, and Yin Blood is damaged, then Yang Qi becomes excessive and transforms into Fire. This is the syndrome of Yin Deficiency with Hyperactive Fire and consumptive disease. Therefore, Zhu Danxi proposed the theory of "Yang Excess, Yin Insufficiency," using Siwu Tang (Four Substances Decoction) added with Zhimu (Anemarrhenae Rhizoma) and Huangbai (Phellodendri Cortex) to tonify Yin, thereby subduing Fire. This uses Blood-tonifying drugs to supplement Blood deficiency. Tonifying Qi and Yin both belong to internal injury syndromes. One type involves sinking Yang Qi, treated by tonifying Qi to lift it; the other involves rising Yang Fire, treated by nourishing Yin to descend it. One lifts, one descends; they are completely different. There is also Summerheat Injury in summer. Although it belongs to external contraction, it resembles internal injury and is very different from Shanghan. Cold injures the Form; Cold Pathogen lodges in the exterior, representing an Excess syndrome, so sweating is indicated. Summerheat injures Qi; Yuan Qi is damaged and dissipated by heat, representing a Deficiency syndrome, so tonification is indicated. This is what Li Dongyuan referred to as Qingshu Yiqi Tang (Clear Summerheat and Augment Qi Decoction). There is also illness caused by consuming cold foods during summer heat, injuring the interior, or seeking excessive cool breezes, injuring the exterior. This is not Summerheat harming the person, but self-inflicted illness due to Summerheat. Treatment should use pungent-hot exterior-releasing or pungent-warm middle-regulating medications, similar to Shanghan treatment. All these syndromes appear similar externally but differ greatly in Deficiency and Excess. Treatments are diverse and must not be erroneous. Therefore, one must carefully examine: if it is Shanghan, Shangfeng, or Cold Epidemic, use Zhang Zhongjing’s methods. If it is Warm Disease, Heat Disease, or Epidemic, use Liu Hejian’s methods. If it is Qi Deficiency, use Li Dongyuan’s methods. If it is Yin Deficiency, use Zhu Danxi’s methods. In this way, medications will not be misused, and human life will be preserved. People often see only fever and assume it is Shanghan external contraction, rashly using sweating drugs to release the exterior. If fever persists after sweating, they use exterior drugs to cool the muscles. If it is a Deficiency syndrome, how could this not be harmful! Moreover, upon hearing of fever caused by deficiency, some use tonifying drugs without distinguishing between Qi and Blood, treating Qi disease with Blood drugs or Blood disease with Qi drugs, causing much harm. Therefore, external contraction vs. internal injury, cold disease vs. heat disease, Qi deficiency vs. Blood deficiency, Water vs. Fire are opposites. If treatment is mistaken, mild disease becomes severe, and severe disease leads to death. Can one not be cautious?
The *Classic* states: "Spring Qi is in the meridians, Summer Qi is in the collateral vessels, Late Summer Qi is in the muscles, Autumn Qi is in the skin, Winter Qi is in the bone marrow." Thus, pathogens often enter and lodge according to the Qi and Blood of the four seasons. Their changes are immeasurable. However, one must follow the Meridian Qi to dispel the pathogens.
Febrile syndromes are not limited to one type; the previous discussion was detailed, so it is not repeated here. Now, under the section of Febrile Syndromes, we elaborate on Yang Deficiency and Yin Deficiency. Regarding Yang Deficiency and Yin Deficiency syndromes: Fever day and night, heavier by day and lighter by night, with no taste in the mouth, indicates Yang Deficiency. Fever starting in the afternoon and stopping at midnight, with a taste in the mouth, indicates Yin Deficiency. Yang is whole, Yin is half; Yang can encompass Yin, but Yin cannot help but encompass Yang. This is natural law. Yang Deficiency is attributed to the Stomach; Yin Deficiency is attributed to the Kidney. Hunger and fullness injure the Stomach, leading to Yang Qi deficiency; sexual fatigue injures the Kidney, leading to Yin Blood deficiency. In terms of medication: Sweet-warm can tonify Yang Qi; bitter-cold can tonify Yin Blood. For example, Sijunzi Tang (Four Gentlemen Decoction) tonifies Qi, and Siwu Tang (Four Substances Decoction) tonifies Blood. If both Qi and Blood are deficient, prioritize sweet-warm formulas to tonify Qi, while also tonifying Blood, because abundant Qi generates Blood. If only Blood is deficient and Qi is not, avoid sweet-warm formulas to tonify Qi, because abundant Qi will further consume Yin Blood. Therefore, Yang Deficiency vs. Yin Deficiency, bitter-cold vs. sweet-warm, must be carefully examined and clearly distinguished. Deficient heat strictly prohibits sweating; reducing heat should not overuse cooling agents. Therefore, the five methods of Harmonizing, Extracting, Following, Breaking, and Belonging can be appropriately adopted. The *Classic* states: "Insufficiency of Yin Qi leads to internal heat, which is true insufficiency. Excess of Yang Qi leads to external heat, which is false excess." When a person's Yuan Qi is inherently weak, or due to improper living habits, dietary fatigue, or excessive mental effort, leading to seminal emission, white turbidity, spontaneous sweating, night sweating, or internal heat, evening tidal fever, fever, dry mouth with thirst, sore throat, cracked tongue, chest/breast distension, hypochondriac/abdominal pain, head/neck pain, dizziness, blurred vision, restlessness, insomnia, red/scanty urination, penile pain, dribbling urine, cold umbilical/abdominal region, emaciation, aversion to cold in limbs, rapid nasal breathing—all these heat syndromes are Rootless Deficient Fire. Taking Shiquan Dabu Tang (Comprehensive Great Tonifying Decoction) or Yi Qi Tang (Augment Qi Decoction) to consolidate the root will cause symptoms to subside. Attacking Wind-Heat would be a mistake.
For Yin Deficiency fever, Danxi used Siwu Tang (Four Substances Decoction) added with Zhimu (Anemarrhenae Rhizoma) and Huangbai (Phellodendri Cortex). How does this compare to Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) strengthening the master of Water to suppress Solar Fire? Or Bawei Dihuang Wan (Eight-Ingredient Pill with Rehmannia) benefiting the source of Fire to eliminate Yin Shadows? These represent the wonder of seeking similarity in energy.
There are cases where severe labor is compounded by Wind-Water-Summerheat, manifesting like malaria or nighttime fever with cough. Physicians only recognize labor and do not realize external pathogens have sunk inward. Mistakenly administering tonics causes pathogens to stagnate in the blood vessels, moving up and down with Qi. The heat resembles Yin Deficiency with Hyperactive Fire wandering through meridians. This cannot be treated as Yin Deficiency with Hyperactive Fire. Instead, use light and ascending agents like Chaihu (Bupleuri Radix), Gegen (Puerariae Radix), Qianghuo (Notopterygii Rhizoma et Radix), and Fangfeng (Saposhnikoviae Radix), assisted by Renshen (Ginseng Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Fuxiong (Chuanxiong Rhizoma), and Xiangfu (Cyperi Rhizoma) to guide and disperse. However, the pulse for Yin Deficiency with Hyperactive Fire is choppy, rapid, and forceless; the pulse for External Pathogen Internal Depression is wiry, rapid, and forceful. This is the distinction.
There are cases where improper diet causes afternoon fever, dry mouth, fatigue, red/scanty urination, and sore legs. All Yin Deficiency symptoms appear, but the root is Spleen Deficiency. The Spleen is Extreme Yin and generates Blood, but derives Qi from the Stomach. Use sweet-warm formulas to generate Stomach Yuan Qi and remove great heat. Do not mistakenly use bitter-cold, which further injures Spleen Blood. If it truly belongs to Kidmer Channel Yin Deficiency, it is also because Kidney Yang Deficiency cannot generate Yin. The *Classic* states: "Without Yang, Yin cannot be generated; without Yin, Yang cannot transform." Do not mistakenly administer Zhimu (Anemarrhenae Rhizoma) and Huangbai (Phellodendri Cortex), which injure the vital Qi of the Stomach. Instead, nourish Water and Fire in the Kidneys to consolidate the root and tonify Spleen Earth, thereby nourishing the source of transformation. This is infallible.
Some call fever "Labor Fever," meaning hard physical labor prevents retention, causing Yuan Yang to float outward. This is what Li Dongyuan calls Internal Injury. If one uses dispersing methods, Yuan Yang will completely escape. If one uses bitter-cold, Deficient Yang will instantly perish.
The *Classic* states: "Yang Deficiency leads to external cold; Yin Excess leads to internal heat; Yang Excess leads to external heat; Yin Deficiency leads to internal cold." Yang receives Qi from the Upper Jiao to warm the skin and muscles. If Cold Qi is outside, the Upper Jiao becomes obstructed. If the Upper Jiao is obstructed, Cold Qi remains alone outside, causing shivering. If there is labor and fatigue, physical Qi declines, Grain Qi is insufficient, Upper Jiao fails to function, Lower Wan is obstructed, Bone Qi becomes hot, and Hot Qi steams the chest, causing internal heat. If the Upper Jiao is not unobstructed, skin becomes dense, pores close, Xuangu (mysteric gates) are blocked, and Wei Qi cannot vent, causing external heat. If Yang Qi rebels upward, Cold Qi accumulates in the chest and does not drain. If it does not drain, warming Qi departs, leaving Cold alone. Blood coagulates, vessels become obstructed, the pulse is large and choppy, causing mid-body cold. If one feels aversion to cold at night but is calm by day, Yin Blood is flourishing in the Yin portion. If one feels aversion to cold both night and day, it is Heavy Yin without Yang. If one is calm at night but feels aversion to cold by day, Yin Qi is flourishing in the Yang portion. If one has fever by day but is calm at night, Yang Qi is flourishing in the Yang portion. If one is calm by day but has fever and irritability at night, Yang Qi has sunk into Yin. If one has fever and irritability both day and night, it is Heavy Yang without Yin. Tidal fever at Zi/Wu hours, and all fevers with aversion to cold, indicate pathogens in the Half-Exterior Half-Interior. Fan (restlessness) belongs to Yang, is rooted fire, mostly originating from the Heart. Zao (agitation) belongs to Yin, is rootless fire, mostly originating from the Kidney. In heat disease, if the pulse is rapid but does not throb upon pressing, it is Cold Excess repelling Yang, actually not heat. If symptoms resemble cold but pulsation is strong upon pressing, it is Heat Excess rejecting Yin, actually not cold. Cun Kou (Inch position) pulse is faint, indicating Yang insufficiency; Yin Qi enters Yang, causing shivering aversion to cold. Chi position (Cubit position) pulse is weak, indicating Yin insufficiency; Yang Qi sinks into Yin, causing fever. Internal injury fever is self-injury of Yang Qi, unable to distribute, descending into Yin portion, becoming internal heat, which is Yang Deficiency. Thus, the pulse is large and forceless, belonging to Spleen and Lung; use Buzhong Tang (Tonify Middle Decoction) to raise and tonify Yang Qi. If Yin Deficiency fever, it is self-injury of Yin Blood, unable to control Fire, causing Yang Qi to rise and become external heat, which is Yang Excess. Since it is Yin Deficiency, the pulse is rapid and forceless, belonging to Heart and Kidney; use Liuwei Wan (Six-Ingredient Pill) to cultivate and tonify Yin Blood. Although there are distinctions among Yin-Yang and Qi-Blood, it ultimately stems from Spleen-Stomach Yang Qi insufficiency and inability to transport and transform. All cold-cooling agents should be prohibited.
For irregular fever, Yang floats outside, and there is no fire inside. Fatigue and poor appetite indicate Middle Qi failing to transport. Dry mouth preferring drink indicates drawing water to save oneself, meaning no water inside. Pulse large and forceless indicates Yang Deficiency; pulse rapid and forceful indicates Yin Deficiency. The method should be moistening tonification to gather floating Yang. Even if the pulse throbs strongly, this is True Qi Deficiency and Pathogenic Qi Excess.
Shanghan fever is Cold Pathogen entering the Defensive layer, Yang Qi struggling, causing external heat. Yang Qi governs the exterior; injured by Cold, it loses function, thus causing heat. The pulse is tight and forceful, indicating external Cold Pathogen injuring Defense; treatment focuses on the exterior. Summerheat injury fever is Fire Pathogen injuring the Heart, Yuan Qi dissipating, and pathogenic heat lodging inside, thus causing fever. Profuse sweating, lack of Qi to move, pulse empty, slow, and forceless, indicates external Heat Pathogen injuring Nutritive layer; treatment focuses on the interior. Generally, pathogens affecting the hair and skin are mild; those injuring muscles are slightly severe; those penetrating organs are most severe.
After external contraction or internal injury, Yuan Qi in the body is already deficient, and pathogenic heat is not yet cleared. Tonifying deficiency here will not remove heat; clearing heat here will not withstand deficiency. Half-tonifying, half-clearing is ultimately not a good method. Therefore, tonifying deficiency has two methods: one tonifies Spleen, one nourishes Stomach. After malaria/dysentery, if Spleen Qi is weak and food cannot be transformed, tonify the Spleen. After Shanghan, if Stomach fluids have been exhausted for a long time and new ones not yet generated, nourish the Stomach. The difference between these two is vast. Clearing heat also has two methods. Initial disease heat is Excess Heat; use bitter-cold drugs to clear it. Post-major illness heat is Deficient Heat; use sweet-cold drugs to nourish it. The difference is vast. Moreover, Human Primal Qi resides entirely in the Stomach. Insufficient fluids, generating fluids is tonifying deficiency. Declining Stomach Yang Qi, warming is tonifying deficiency. Spleen Yin insufficiency: tonifying Qi must include moistening agents. Spleen Qi Deficiency Cold: tonifying middle must restore Yang. However, the source of Primal Qi lies especially in the two Kidneys. Cold: warm it; Heat: nourish it. When Yang Harmony is moderate, neither excessive nor insufficient, this is called "Minor Fire generates Qi," allowing Qi to warm and Blood to moisten naturally.
Deficiency and Excess syndromes can all cause fever. If not recognized accurately, treatment will be erroneous. Generally, Renying (Human Welcome) pulse larger than Qikou (Qi Opening) indicates external contraction; Qikou pulse larger than Renying indicates internal injury. Floating-rapid is external heat; sinking-rapid is internal heat; floating-large-forceful is external heat; sinking-large-forceful is internal heat; floating-large-forceless is Deficiency; sinking-thin-forceful is Excess. Tight pulse with aversion to cold is called Shanghan; slow pulse with aversion to wind is called Shangfeng. Full pulse with high fever is called Shangre (Heat Injury); empty pulse with body heat is called Shangshu (Summerheat Injury). Fever with energetic spirit, able to speak forcefully is Excess; fever with fatigued spirit, reluctant to speak, forceless is Deficiency. Heat upon initial palpation but not after prolonged pressure indicates Interior Yang floating to Exterior, which is Deficiency. Heat upon initial palpation increasing with prolonged pressure indicates Interior Heat penetrating Exterior, which is Excess. Constant high fever, head, feet, and body feeling like burning iron is Excess. Intermittent fever, head hot and feet cold, indicates Rootless Fire floating to Exterior/Upper, which is Deficiency. Dry mouth desiring cold drinks in large amounts is Excess; dry mouth desiring warm drinks in small amounts is Deficiency. High fever with sinking-thin pulse, or extremely large/rapid pulse changing upon pressure, is Deficiency. Mild fever with consistent flooding-rapid pulse is Excess. Body heat without sweat, closed urination/defecation is Excess; body heat with sweat, free urination/defecation is Deficiency. Exterior pattern with heat belongs to Exterior; No-Exterior pattern with heat belongs to Interior. Fever with aversion to cold is Yang; No fever with aversion to cold is Yin. Fever during autumn/winter (concealment periods) is mostly Excess; fever during spring/summer (growth periods) is mostly Deficiency. Overall, heat comes from the interior. Either external pathogens disturb Clear Yang, or internal stagnation steams to create Major Fire. Apart from these two Excess patterns, the rest are either Qi Deficiency failing to retain Yuan Yang, or Yin Deficiency failing to suppress Thunder Fire. Tidal fever has Yin-Yang distinctions. Morning tidal fever from Yin to Shen hours (Yang phase): Yang Qi operates, heat is in the Yang portion, governed by Lung Qi. Evening tidal fever from Shen to Yin hours (Yin phase): Yin Qi operates, heat is in the Yin portion, governed by Kidney Qi. One clears Lung, one nourishes Kidney. For Qi Deficiency tidal fever, use Renshen (Ginseng Radix et Rhizoma), Huangqi (Astragali Radix), Shudi (Rehmanniae Radix Preparata), Fuzi (Aconiti Lateralis Radix Preparata); this is "Warmth can remove great heat." For Blood Deficiency tidal fever, use Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Muguanpi (Lycii Cortex), this is "Nourishing Yin retreats Yang."
Generally, if the body is hot but the pulse is harmonious and appetite exists, this is Exterior disease without Interior disease. Thin porridge and thick drinks, let nature take its course; only regulate, do not abolish. Often seen: abolishing food due to heat causes Middle Qi to daily weaken, turning non-diseased Interior into diseased. If body is hot, pulse is faint, and appetite is lazy, it indicates Exterior disease Excess and Interior Qi Deficiency. Only regulate Spleen and nourish Stomach to gather Deficient Yang. Fire and Yuan Qi are mutually exclusive. When Yuan Qi recovers, fire subsides naturally; this is "Sweet-warm can remove great heat." If body is hot, pulse is wiry-rapid-forceless, this is Yin Deficiency failing to gather Yang; nourish Yin to retreat it. If initial disease body heat with floating-huge-forceful pulse, it is external contraction; treat from exterior. If initial disease body heat with sinking-rapid-forceful pulse, it is internal injury; treat from interior. Over time, exterior-interior distinction blurs; Yuan Qi is inevitably damaged. One should treat based on Yang Deficiency or Yin Deficiency, focusing on the root. Initially, distinguish exterior-interior; long-term results in general deficiency. This is (Zhang)'s humble opinion.
Disease with heat and Fire lives; Heart pulse flooding is the sign. No Fire dies; sinking-thin is the sign. Sinking-thin or rapid dies. Floating-choppy, choppy with body heat dies. Heat with quiet pulse is difficult to treat. Full pulse with sweating not resolving dies. Empty pulse with continuous heat dies. Three
