When a pregnant woman develops measles (Rash Disease), the treatment should be based on Siwu Tang (Four Substances Decoction) with modifications, adding Huangqin (Scutellariae Radix) and Aiyè (Artemisiae Argyi Folium). The primary principles are to calm the fetus and clear heat. If the fetal qi remains undisturbed, the measles will resolve spontaneously.
However, steaming heat toxins often injure the fetus. Although the fetus may be injured, the mother usually remains unharmed. This is because measles differ from smallpox: for smallpox, it is advisable to strengthen the interior, as the smallpox toxin resolves from the exterior; consequently, if the fetus falls, the mother may die. For measles, it is advisable to keep the interior relatively empty (referring to avoiding premature use of cloying or astringent herbs), as the measles toxin resolves from within; consequently, if the fetus falls, the mother survives. Nevertheless, rather than losing the fetus while saving the mother, it is far better for both mother and child to survive.
Moreover, ancients only knew to clear heat to calm the fetus, failing to consider that if heat-clearing methods are employed before the measles have erupted, the rash will struggle to emerge, and internal heat will deepen. This desire to protect the fetus ends up harming it. One should employ light, clearing, and dispersing methods to release the exterior, allowing the measles to erupt so that heat clears naturally. Subsequently, using yin-nourishing and heat-clearing methods ensures no harm to either the measles or the fetus; thus, without deliberately calming the fetus, the fetus remains secure.
Furthermore, Aiyè (Artemisiae Argyi Folium) and Sharen (Amomi Fructus Rotundus) are warm in nature and aromatic. After the lung qi has been severely damaged, administering such aromatic and drying herbs can trigger coughing and wheezing, which disturb the fetus. In a state where fluids are depleted and metal (lung) is dry, how can nourishment occur? This approach bears only the name of calming the fetus but has the reality of damaging it. I dare not follow these established methods of sages, lest I endanger the lives of future patients.
If other symptoms appear before the onset of measles, it is termed "Xiangjia Zhen" (Measles complicated by other conditions); if the condition transforms into other diseases, it is termed "Xiangchuan Bing" (Measles transmission disease). Families fear complication, and physicians fear transmission. Why? Generally, conditions like convulsions (Jingfeng) resolve along with the eruption of measles. This is described as moving from interior to exterior, where the six qi interact, and the toxins are resolved together. Conversely, if the measles toxin fails to penetrate, dispersion is incomplete, and management is improper, it may transform into other conditions leading to critical distress. This is described as moving from exterior to interior, with toxins stagnating within. Therefore, caution is essential during measles.
When sweating occurs all over the body during fever, this indicates that toxins are dispersing through sweat, the pores (Xuanfu) are open, and the rash emerges easily. If epistaxis (nosebleed) occurs, this indicates that toxins are resolving through bleeding. Both scenarios align with the principle of dispersion and should not be forcibly stopped. However, if sweating is excessive or bleeding does not stop, this is due to excessive fire-toxin forcing fluids to leak and blood to flow recklessly. One should use herbs such as Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Shengdihuang (Rehmanniae Radix Recens), and Fuxiaomai (Tritici Levis Fructus) to stop sweating, or primarily administer Renshen Baihu Tang (Ginseng White Tiger Decoction) or Huanglian Jiedu Tang (Coptis Detoxification Decoction). To stop bleeding, one may use Maohua (Spatholobi Caulis tip/infusion), Danggui Tou (Danggui head), Shengdihuang, Gancao (Glycyrrhizae Radix et Rhizoma), Mudanpi (Moutan Cortex), Xuanshen (Scrophulariae Radix), Niubangzi (Arctii Fructus), and Lianqiao (Forsythiae Fructus), or primarily use Xuanshen Dihuang Tang (Scrophularia and Rehmannia Decoction). Delaying treatment results in excessive sweating depleting yuan qi and excessive blood loss scattering spirit, leading to severe complications.
