1. Crimson Clouds Surrounding the Crown: The pustules cover the entire body normally, except for a patch of crimson redness on the crown of the head. This indicates heat-toxin accumulating in the Bladder (Bladder Organ). It requires clearing heat and promoting diuresis.
2. Purple Clouds Pouring Over the Crown: The pustules cover the entire body normally, except for the crown of the head which is purple, dry, and sunken without rising. This is incurable.
3. Du Meridian Reaching the Barrier: The Qi Hui point is located below the sole of the foot, named Yongquan (KI-1, Springing Well). If pustules appear here, it indicates that the eruption has fully manifested and prognosis is excellent; alternatively, some say if redness or pus formation appears first at this site, it is a very bad omen, requiring urgent preservation of Yuan Qi (Original Qi).
4. Black Gauze Covering the Crown: Symptoms inevitably include hoarseness and blockage of the throat, panting, and coarse breathing. This indicates depletion of blood and failure of Qi, with loss of Yuan Yang (Original Yang). This is incurable.
5. Pear Blossoms Spreading Over the Crown: This is due to deficiency and cold of Qi and Blood, unable to stimulate luster. Urgently use Baoyuan Tang (Preserve the Origin Decoction) added with Tianxiong (Aconiti Lateralis Radix Preparata), which may still be curable.
6. Clouds Obscuring the Forehead: Also known as "Overturned Cauldron" or "Covered Head". The pustules are distinct and lustrous over the whole body, except for a patch of blood blisters on the forehead like clouds. This is caused by intense guest-heat in the Heart (Heart Organ). Urgently administer substances like Rhinoceros Horn (now substituted with Water Buffalo Horn).
7. Duckweed Spreading on the Forehead: Symptoms include extreme heat, sore throat, restlessness, and madness. Treatment should urgently focus on clearing heat, promoting diuresis, and detoxifying.
8. Black Gauze Falling on the Forehead: A patch of black qi covers the forehead firmly. This indicates simultaneous exhaustion of Yuan Yang and Blood/Qi. This is incurable.
9. Ash Dusting the Yintang Area: This condition indicates insufficient blood in the Heart. If accompanied by abdominal distension and dry throat, treatment should primarily tonify Qi and Blood while supporting the interior. Use Baoyuan Tang (Preserve the Origin Decoction) added with Chuanxiong (Ligustici Chuanxiong Rhizoma), Guizhi (Cinnamomi Ramulus), and Honghua (Carthami Flos). If it extends to the nose, it is called "Pillar in Midstream" or "Toxin Stagnating at Yingxiang", belonging to the Lung (Lung Organ).
10. Red Silk Brushing the Face: Treatment should primarily cool blood and detoxify. If pustules are numerous and form continuous red flesh patches, it is incurable.
11. Poplar Flowers Facing the Face: Treatment should use Neituo San (Internal Support Powder) added with Tianxiong (Aconiti Lateralis Radix Preparata).
12. Red Beads Circling the Lips: This is due to extreme heat in the Spleen Channel (Spleen Organ). Urgently clear fire and detoxify.
13. Black Rice Staining the Lips: Symptoms inevitably include hoarseness, coma, fixed eyes, and cold limbs. Death occurs on the third or seventh day.
14. Rosy Brocade Piercing the Chest: This is due to extremely intense fire-toxin. This is an incurable condition.
15. Purple Clouds Spreading on the Chest: This is caused by blood stasis and Qi stagnation with toxin attacking the chest. Symptoms inevitably include clenched teeth, tremors, and cracked, scorched lips, possibly leading to death within moments.
16. Black Chess Pieces Arranged on the Chest: This indicates extreme hyperactivity of Heart Fire and appearance of the True Zang color. Due to the rapid nature of fire, death is decided by the third day.
17. Willow Catkins Flying on the Chest: This is due to withered and weak Qi and Blood. Use Baoyuan Tang (Preserve the Origin Decoction) added with Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Guizhi (Cinnamomi Ramulus), and Fuzi (Aconiti Lateralis Radix Preparata).
18. Peach Blossoms Reflecting on the Back: Pustules all over the body are red, vibrant, blooming, and round, but densely red on the back. Treatment should primarily clear fire, cool the Heart, and detoxify.
19. Duckweed Floating on the Back: Pustules on the back are dense, red-purple, and forming continuous patches. This is incurable.
20. Toxin Obstructing the Three Granaries: Also known as "Silk Chest". Pustules form patches on the chest, which is the location of the Front-Mu points of the Five Zang Organs, where Qi gathers, and where the Pericardium Channel pours. There is no hope for survival.
21. Flower Studs Slanting in Piles: Pustules are particularly numerous on the temples. If they turn purple-red, especially clear heat and resolve toxin.
22. Black Sand Falling on the Back: This condition is particularly incurable.
23. Owl Flame Obscuring Hearing: Also known as "Locked Voice". Pustules are particularly numerous in the ears. The ear is the opening of the Kidney (Kidney Organ); it is contraindicated for pustules to appear or crust over first in the ears. If the ears become hot and black, beware of transformation; black corresponds to the Kidney, indicating an ominous sign.
24. Clouds Spreading on Fish Back: This indicates complete exhaustion of blood in the Five Zang Organs. This is incurable.
25. Red Scales Piercing the Abdomen: This is due to extreme heat in the Stomach (Stomach Organ), resulting in hard constipation and scanty, painful urination. Treatment should focus on harmonizing.
26. Three Yin Channels Gathering Toxin: The big toe area is Taichong (LR-3), belonging to the Jueyin Liver Channel. The sole is Yongquan (KI-1), belonging to the Foot-Shaoyin Kidney Channel. The side of the thigh is Shangqiu (SP-5), belonging to the Foot-Taiyin Spleen Channel. If toxin gathers here, the prognosis is ominous.
27. White Pear Falling into the Abdomen: This indicates failure of both Qi and Blood. Within half a month, convulsions will occur leading to death.
28. Grapes Falling to the Ground: Pustules between the buttocks and coccyx are red-purple and patch-like, resembling grapes. This is incurable.
29. Rotten Millet Dwelling on the Buttocks: Pustules are normal everywhere else, except for a patch on the buttock resembling rotten millet husks with a foul odor. Urgently perform tonification and support to expel toxin, and warm the Lower Yuan (Lower Original). Survival may be possible.
30. Pomegranate Flowers Scattered in the Wild: All pustules are normal, except for redness on the four limbs and cracking of lips and mouth. This indicates extreme heat in the Heart, Spleen, and Lung channels. Treatment should primarily cool blood, clear fire, and detoxify.
31. Lotus Coins Penetrating Water: Also known as "Detached Cheeks". Pustules are exclusively dense on both cheeks. If they become scorched-purple and spirit dull, it is ominous.
32. Poplar Flowers Falling from Branches: Pustules are exclusively gray-white on the four limbs. This is due to Qi coagulation and Blood stagnation. Use Bazhen Tang (Eight Treasures Decoction) added with Fuzi (Aconiti Lateralis Radix Preparata).
33. Owls Gathering on Both Cheeks: Also known as "Rouge Slapping the Face" or "Orange Peel Face". Pustules are exceptionally numerous and purple-red on both cheeks. This is due to extreme heat in the Liver and Lung organs. Urgently clear heat and detoxify.
