Whenever the exterior heat is excessive, the smallpox lesions will inevitably become dry and withered; if the exterior is too cold, the lesions will be frozen and unable to erupt. If internal heat is excessive, there will be constipation; if the interior is too cold, there will be diarrhea. If Qi stagnation is severe, abdominal distension, wheezing, and chest fullness will occur. If the heat-toxin is suppressed and cannot ascend and vent, it leads to abdominal distension and madness, with toxic vapors becoming rampant, affecting both the exterior and interior heavily, which infants and children cannot bear. Therefore, the key to treating smallpox lies in five aspects: securing the exterior, harmonizing the middle jiao, regulating Qi, penetrating the muscle layer to vent toxins, and resolving toxins. Securing the exterior prevents the hazards of dryness or freezing; harmonizing the middle jiao prevents the worries of constipation or diarrhea; keeping the interior Qi solid, blood and Qi vigorous internally, and the Spleen and Stomach strong helps aid maturation, naturally avoiding itching, collapse, or regression of the lesions. Regulating Qi prevents the excesses of stagnation, wheezing, and fullness; penetrating the muscle allows heat-toxins to extend and reach the exterior, preventing them from lingering inside; resolving toxins allows for distribution and elimination from both inside and outside, avoiding disasters from residual toxins spreading. By achieving these five points, the syndrome of blood-heat stagnation and obstruction need not be worried about. However, fire nature is urgent and should be resolved quickly, allowing no delay.
