The ear is the gathering place of the zong vessels (major meridians) and the passage for Kidney Qi. In some children, excess Qi in the Kidney meridian causes heat to rush upward to the ear, leading to the stagnation of body fluids which transform into pus or clear fluid. The conditions involving Jue Yin, Foot Yangming, Hand Shaoyin, and Foot Taiyang are particularly severe. Investigating the causes, there are seven primary origins: excess heat, yin deficiency; phlegm; fire; qi obstruction; Liver wind; fetal origin toxins; and disease manifestation. There are five main symptoms: tinnitus, otalgia, deafness, ear swelling, and otorrhea. What is excess heat? It refers to surplus Kidney Qi and accumulated heat rushing upward, causing fluid stagnation and condensation, resulting in purulent otitis (Nong Er). The term "purulent otitis" encompasses five types: those constantly discharging yellow pus are called Nong Er; those discharging red pus are called Nong Er (Note: Text says Nong Er, context implies blood-related); those with a gangrenous odor are called Hu Er; those discharging white pus are called Chan Er; those with empty tinnitus and occasional green pus are called Nang Er. Although the names differ, they all stem from accumulated heat obstructing upward or wind-water entering the ear. If not treated promptly, it may lead to long-term deafness. The treatment method should focus on clearing fire, nourishing blood, or removing dampness and resolving toxicity. What is yin deficiency? Symptoms include heat in the palms and soles, emaciation with dark complexion, thirst, dry intestines, large pulses at the cun position of both chi positions, occasional itching, deafness, and tinnitus. The treatment principle is to nourish yin and soothe the Liver. What is caused by phlegm? Symptoms include qi stagnation, dry mouth, no pain but itching, heavy body, and wiry pulse, accompanied by tinnitus and otorrhea. The treatment involves Er Chen Tang with Zhu Li (Bamboo Sap). What is caused by fire? Whether due to sudden anger, indulgence in desires, excess fire, or deficient fire, leading to deafness and pain, the treatment uses Huang Qin (Scutellariae Radix), Lian Zi (Coptidis Rhizoma), Dang Gui (Angelicae Sinensis Radix), and Bai Shao (Paeoniae Radix Alba). What is qi obstruction? Caused by anger injuring the Liver, phlegm generated by fire, sudden stroke, or qi deficiency from chronic illness, leading to deafness and tinnitus. The treatment focuses on relieving depression and regulating blood, with external methods for guiding and unblocking. What is Liver wind? Caused by fire obstructing the upper jiao, leading to sudden severe pain, discharge, or swelling; or due to excessive anger and alcohol consumption, where damp-heat combines, causing ear swelling and pain. The treatment aims to calm the Liver, clear heat, and dispel wind. What is fetal origin? Due to parental negligence, congenital toxins attack and rush, causing pus and odor to flow, forming sores with surrounding redness and swelling, recurring intermittently. The treatment involves resolving toxicity and nourishing the Kidney. Additionally, wind entering the brain and stagnating in the Hand Taiyang meridian can cause qi blockage and deafness. If wind-dampness combats, ear sores arise. Pointing at the moon with a finger may cause sores behind both ears, known as Yue Shi Chuang (Moon Erosion Sore), along with frostbite or insect bite injuries. External causes are treated externally. Swelling and pain at the ear root and gum belong to upper jiao wind-heat, affecting the Yangming and Shaoyang meridians; treat with Stomach-clearing, acrid-cool dispersing methods. For intense excess heat, use Jiu Zheng Da Huang (Wine-processed Rheum Palmatum Rhizome) to induce mild purgation. Deafness after a major illness indicates blood depletion and qi weakness; treat with Di Huang Wan (Rehmannia Pill). Sudden severe pain in the ear, feeling like insects running inside, bleeding, discharge, or dryness, unbearable pain, can be stopped immediately by burning She Tui (Ophichus Skin) until carbonized, grinding it, and blowing it into the ear using a goose quill. This utilizes the snake's ability to shed skin to resolve dispersed fire.
