Patient Meng (June): Yin qi is damaged first, while Yang qi acts alone and erupts. Malaria originating from the Yin aspect, characterized by fever without chills, is called Dan Nue (Cachectic Malaria). The pulse is wiry and rapid; treatment should employ Yinhú Qīnggǔ Yǐn (Silver Twig and Pearl Decoction to Clear Bone Heat).
*Glehniae Radix* (Beishashen), *Anemarrhenae Rhizoma* (Zhimu), *Moutan Cortex* (Fendanpi), *Poria Cocos* with Cinnabar (Zhufushen), *Hedysari Radix* (Yinhu), *Artemisiae Annuae Fructus* (Qinghaozi), *Uncariae Ramulus cum Uncis* (Chunnengou), *Nelumbinis Petiolus et Costula* (Qinghegeng), *Testudinis Carapax et Plastrum* (Danbiejia), *Lycii Cortex* (Digupi), *Rosae Rugosae Flos* (Meiguimei).
Certain patient: Internal injury due to overwork and fatigue, chronic malaria persisting for a long time. The pulse is wiry, rapid, and soggy. Treatment should focus on Tonifying the Middle and Augmenting Qi (Buzhong Yiqi).
*Codonopsis Radix* (Zhenfangdang), *Bupleuri Radix* stir-fried with turtle blood (Xiechao Chaihu), *Paeoniae Radix Alba* (Dongbaishao), *Poria Cocos* (Baiyunling), *Atractylodis Macrocephalae Rhizoma* honey-fried (Zhidongzhu), *Cimicifugae Rhizoma* charred and honey-fried (Zheiheimashengma), *Testudinis Carapax et Plastrum* (Danbiejia), *Glycyrrhizae Radix et Rhizoma* honey-fried (Qingzhigancao), *Astragali Radix* (Nenmian), *Citri Reticulatae Pericarpium* (Xinhui Chenpi), *Polygoni Multiflori Preparatus Radix* (Heshouwu), *Quinine Hydrochloride* stir-fried with ginger juice (Shengjiangzhi chao Kuaihong).
Patient Chen (June): Summer-heat and dampness mutually disturb the Yangming channel, compounded by damage to the spleen from melons and fruits. Spleen and stomach disharmony results in borborygmus and diarrhea with frequent episodes, epigastric stuffiness, abdominal distension, bitter taste in the mouth, and dark urine. The right-side pulse is wiry and slippery. Treatment should harmonize the middle and guide out stagnation.
*Coicis Semen* (Shengyiren), *Lablab Albus Seed Coat* (Biandouyi), *Perillae Fructus Fermented* (Banxiaqu), *Amomi Fructus Rotundus* (Baikourén), *Plantaginis Herba* (Cheqiancao), *Magnoliae Officinalis Cortex* processed (Zhi chuanpu), *Aucklandiae Radix* baked, combined with *Zuojin Wan* (Wuxian ban), *Polyporus* (Chiling), *Medicated Leaven* charred (Jiao liuqu), *Patchouli Herba* (Guanghuoxiang), *Citri Reticulatae Pericarpium* (Xinhui pi), *Polyporus* (Muzhuling), *Alismatis Rhizoma* (Zexie).
Alternatively, the original formula of Weiling Tang may be used.
Patient Shi: Stool has become loose and thin; mental expression is clear and moist, muscles are gradually regenerating. These are signs that the primordial Qi of the spleen and stomach has not yet recovered. The pulse form is wiry and slow. Treatment should regulate the middle.
The original formula of Shenling Baizhu San.
Jingyue's Wuyin Jian and Sishen Wan are both applicable.
Patient Cheng (Left side, June): Cold, summer-heat, dampness, and food stagnation mutually disturb the Yangming channel. The alternating cold and heat seem to gradually cease, but sweating causes pathogenic factors to sink inward, forming dysentery. Dysenteric discharge contains white accumulation. Defecation is accompanied by abdominal pain and tenesmus. The pulse is wiry, slippery, and soggy; the tongue coating is yellow and greasy. Treatment should harmonize the middle and guide out stagnation.
*Coicis Semen* (Shengyiren), *Aucklandiae Radix* baked, combined with *Zuojin Wan* (Wuxian ban), *Plantaginis Herba* (Cheqiancao), *Alismatis Rhizoma* (Zexie), *Patchouli Herba* (Guanghuoxiang), *Citri Reticulatae Pericarpium* (Chenpi), *Polyporus* (Muzhuling), *Polyporus* (Chiling), *Magnoliae Officinalis Cortex* processed (Zhi chuanpu), *Crataegi Fructus* charred (Zhatan), *Perillae Fructus Fermented* (Banxiaqu).
Alternatively, *Eupatorii Herba* (Zhike) and *Arecae Pericarpium* (Fupi) can be used to vent and drain.
Alternatively, add *Amomi Fructus* (Baiko) and *Zingiberis Rhizoma* baked (Wei jiang) to warm.
Note: Early-stage red and white dysentery also follows this method, which is a variation of combining Weiling Tang and Xianglian Wan. *Coicis Semen* replaces *Atractylodis Macrocephalae Rhizoma*; *Plantaginis Herba* replaces *Cinnamomi Ramulus* to unblock yang. The remaining three herbs combined constitute Wuling San.
