If the pulse is palpable in the upper part (cun position) but absent in the lower part (chi position), and the patient should vomit but cannot, the prognosis is poor and indicates a critical condition. A pulse that is tight in the yang (upper/cun) portion and rapid in the yin (lower/chi) portion indicates vomiting; a floating and rapid pulse in the yang portion also indicates vomiting. If the cun pulse is tight and the chi pulse is choppy, accompanied by chest fullness and vomiting, it indicates vomiting; a rapid pulse at the cunkou (radial artery) indicates vomiting; a tight and choppy pulse is difficult to treat. A tight and slippery pulse indicates vomiting and counterflow. A weak pulse with vomiting, frequent urination, slight fever, and cold extremities indicates a difficult-to-treat condition. For patients desiring to vomit, purgation methods must not be used. If vomiting is severe and painful, and the vomitus resembles the color of green vegetables, the condition is critical.
Xuanfu Daizhe Shi Tang (Rehmannia and Hematite Decoction) (Formula see *Shanghan Lun*)
Treats stomach qi deficiency with persistent belching.
Jintui Huanglian Tang (Advance-Retreat Coptis Decoction) (See *Shizai Yi*)
Herbs: Huanglian (Coptidis Rhizoma, stir-fried with ginger juice), Ganjiang (Zingiberis Rhizoma Praeparatum, processed), Renshen (Ginseng Radix et Rhizoma, steamed with human milk), Banxia (Pinelliae Rhizoma, prepared with ginger juice), each 1.5 qian; Guizhi (Cinnamomi Ramulus), 3 qian; Dazao (Ziziphi Jujubae Fructus), 2 pieces.
Advance Method: Take the first three herbs (Huanglian, Ganjiang, Renshen) without special processing. Add three tea cups of water, decoct until one cup remains, and take warm.
Retreat Method: Omit Guizhi, reduce the dosage of Huanglian by half, or add Rougui (Cinnamomi Cortex), 0.5 qian. Process each herb as described above, decoct and take. However, take Cuishi Bawei Wan (Eight-Ingredient Pill of Cuishi), 3 qian, on an empty stomach, and take the decoction when half-hungry.
Wuzhuyu Tang (Evodia Decoction)
Treats stomach qi deficiency and cold, dry heaving, vomiting of saliva/froth, and headache.
Ganjiang Huanglian Huangqin Renshen Tang (Dried Ginger, Coptis, Scutellaria, and Ginseng Decoction) (Formulas see *Shanghan Lun*)
Ke Yunbo stated: For patients with vomiting complicated by heat, who are not suitable for aromatic and damp-draining herbs like Xiangfu, Sharen, Chenpi, and Banxia, taking this formula clarifies the principle.
Changsun Xin Dian commented: Vomiting immediately after eating, without allowing food to stay in the stomach, is a sign of flaming fire. Therefore, the dosage of bitter-cold herbs is double that of pungent-warm herbs. This formula is not named Xiexin Tang because Xiexin Tang is specifically formulated for pi-ying (glomus hardness). One must understand that when cold and heat knot together below the heart, pi-ying forms; when cold and heat obstruct each other below the heart, ge-ni (regurgitation and counterflow) forms. They share the same origin but have different manifestations.
Herbs: Ganjiang (Zingiberis Rhizoma), Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), Renshen (Ginseng Radix et Rhizoma), each 1.5 qian.
Decoct with water and take warm.
Jupi Zhuru Tang (Citrus Peel and Bamboo Shavings Decoction)
Treats hiccups due to stomach deficiency.
When middle jiao qi is deficient, wind-wood from the lower jiao can ascend and interfere, causing grain qi to fail to transform properly, resulting in hiccups. Jupi (Citri Reticulatae Pericarpium) regulates the ascending and descending of middle jiao qi; Renshen (Ginseng Radix et Rhizoma) and Gancao (Glycyrrhizae Radix et Rhizoma) tonify the middle jiao; Shengjiang (Zingiberis Rhizoma Recens) and Dazao (Ziziphi Jujubae Fructus) disperse counterflow qi; Zhuru (Bambusa Textilis Caulis in Plaster) descends the wind-heat of gallbladder wood.
Jupi Ganjiang Tang (Citrus Peel and Dried Ginger Decoction)
Treats dry heaving, vomiting and counterflow, vomiting of saliva/froth, and stops hiccups.
