The "Five Extremes" refer to five conditions that appear fatal: first, hanging (self-strangulation); second, crushing or compression injuries; third, drowning; fourth, fainting caused by evil qi invasion (Yanmei); and fifth, poisoning.
For those who have hanged themselves, if the incident occurred from morning to evening, they can certainly be treated even if the body has become cold; if it occurred from evening to morning, treatment is difficult due to the prevalence of Yin Qi. However, I have witnessed cases where individuals were saved despite the time elapsed from evening to morning, so one must not give up easily. The rescue method involves first carrying the person down, stuffing the anus with bedding, then slowly untie the rope without abruptly cutting it. Next, massage the chest. If breath is exhaled from the mouth and there is a faint respiration, immediately administer a decoction made from 2-3 qian of high-quality Cinnamomi Cortex Heart (Rouguixin). If the body is stiff, have two people blow air into both ears using a bamboo tube, then insert Banxian Wan (Half-Immortal Pill) into the nostrils and blow powdered medicine into the ears. As long as warmth remains in the heart region, survival is possible even after a day.
For those crushed by falling objects or beaten by impact, rescue is possible as long as warmth remains in the heart region. Position the patient in a lotus position similar to a monk meditating. Have one person hold their hair while inserting Banxian Wan (Half-Immortal Pill) into the nose. Administer a decoction of Guang Sanqi (Notoginseng Radix et Rhizoma) 2-3 qian in wine. A decoction of Qingmuxiang (Aucklandiae Radix) in wine is also effective.
For drowning victims, lift them out and place them prone across a cow's back; if no cow is available, use a stool instead, allowing water to drain. Insert Banxian Wan (Half-Immortal Pill) into the nostrils or use Chubi San (Nose-Snuffing Powder) blown into the nasal cavity. Administer fresh Ginger Juice (Shengjiang Zhi). Rescue is possible provided no blood flows from the nostrils.
Yanmei refers to remaining unconscious after dreaming. This is caused by the obstruction of turbid Qi and stubborn phlegm. First, blow Tongtian San (Sky-Penetrating Powder) into the nose, followed by administering Suhexiang Wan (Styrax Pill). Alternatively, administer juice extracted from Leek Roots (Jiu Gen), Ginger Juice (Jiang Zhi), or scallion white mixed with liquor. If a lamp was originally lit in the sleeping area, the person may survive; if no lamp was present, do not shine light directly on the face, but only place a lamp at a distance. Another method involves having someone bite the thumb hard and spit on the face, which can restore life.
Regarding poisoning, arsenic (Pishuang) and realgar/orpiment mixtures (Xinshi) are the most severe. Immediate rescue can be achieved by drinking juice extracted from Small Thistle Root (Xiaoji Gen). Alternatively, treat with Huangfan San (Yellow Alum Powder), which is reportedly highly effective. For self-inflicted throat cuts, if the trachea is not completely severed, suture it immediately with hemp thread, apply a thick layer of Jinchuang Yao (Metal Wound Medicine), and bind tightly with cloth; spontaneous healing occurs within ten days.
