The chest is the domain governed by the Lung. However, disease in the Shaoyang Gallbladder Meridian can also cause chest pain; this occurs when pathogenic factors have just entered the interior but have not yet penetrated deeply, thus resulting in chest pain. Ancient formulas employ Chaihu Tang (Bupleurum Decoction) with Zhiqiao (Fructus Aurantii Immaturus) for treatment. If there is no response, administering one dose of Xiaoxianxiong Tang (Minor Inverted Chest Granule Decoction) added to the original formula yields miraculous efficacy. Furthermore, if Wind-Cold invades the Lung causing chest fullness and pain accompanied by asthma, Ganjie Tang (Licorice and Platycodon Decoction) combined with herbs that regulate Qi and disperse Wind is appropriate. In cases of food stagnation blocking the area, emesis therapy should be used. The two conditions of Lung Abscess (Fei Yong) and Lung Withering (Fei Wei) are detailed in the chapter on Consumption Deficiency Diseases (Xu Lao), so they are not repeated here.
Chaihu Tang (See Shaoyang Syndrome in Cold Damage).
Ganjie Tang (See Sore Throat in Cold Damage).
Chest pain due to Cold Damage belongs to pathogenic invasion of the Shaoyang Meridian and is treated with Xiao Chaihu Tang (Minor Bupleurum Decoction). For miscellaneous diseases causing chest pain, pain on the left side indicates Disharmony of Liver Qi, treated with Chaihu Shugan San (Bupleurum Powder to Soothe the Liver). Emotional depression is treated with Xiaoyao San (Free and Easy Wanderer Powder). If accompanied by Liver Fire, Phlegm-Fluid, Food Stagnation, or Blood Stasis, herbs are added according to symptoms. Pain on the right side indicates the Liver transferring pathogenic factors to the Lung, treated with Tuiqi San (Push Away Qi Powder). In treating excess-type chest pain, Zhiqiao (Fructus Aurantii Immaturus) is used for the left side, and Yujin (Radix Curcumae Longae) for the right side; both are precise remedies. However, pain caused by Deficiency-Cold, which dissipates with warmth and stops upon pressure, requires warming and tonification, and one must not adhere rigidly to other methods.
Xiao Chaihu Tang (See Cold Damage).
For treating left-sided chest pain:
Chaihu (Radix Bupleuri), Chenpi (Pericarpium Citri Reticulatae) (each 1 qian 2 fen), Chuanxiong (Rhizoma Chuanxiong), Chi Shao (Radix Paeoniae Rubra), Zhiqiao (Fructus Aurantii Immaturus, bran-fried), Xiangfu (Rhizoma Cyperi Rotundi, vinegar-fried) (each 1 qian), Gancao (Radix Glycyrrhizae, honey-fried) (5 fen).
If lips are scorched and thirsty, with pain that comes and goes intermittently, it is Fire; add Zhizi (Fructus Gardeniae) and Huangqin (Radix Scutellariae). If a tendon feels raised along the Liver Meridian, it is Food Stagnation; add Qingpi (Pericarpium Citri Reticulatae Viride), Maiya (Fructus Hordei Germinatus), and Shanzha (Fructus Crataegi). If pain is fixed in location and does not move, being lighter during the day and heavier at night, it is Blood Stasis; add Guiwei (Radix Angelicae Sinensis Tail), Honghua (Flos Carthami), Taoren (Semen Persicae), and Mudanpi (Cortex Moutan). If there is dry retching and coughing that draws pain to the hypochondrium, it is Retained Fluids; add Banxia (Rhizoma Pinelliae) and Fuling (Poria). If one prefers heat and fears cold, desiring pressure from warm hands, it is Cold Pathogen; add Rougui (Cortex Cinnamomi) and Wuzhuyu (Fructus Evodiae).
