Taiyang disease (disease of the Taiyang channel), characterized by fever and aversion to cold, with heat predominating over cold, and a faint pulse, indicates a deficiency of Yang Qi (deficiency of defensive and warming energy). Sweating methods should not be used again. Guizhi Er Yuebi Yi Tang (Cinnamon Twig Twice-Yuebi One Decoction) is appropriate.
In Taiyang Wind-stroke syndrome (Taiyang wind-pathogen invasion), if the pulse is floating and tight, accompanied by fever, aversion to cold, body pain, anhidrosis (absence of sweating), and irritability, Da Qing Long Tang (Major Blue Green Dragon Decoction) is the primary treatment. However, if the pulse is faint, there is sweating, and aversion to wind, Da Qing Long Tang must not be administered. Ingestion would lead to limb reversal-cold (cold extremities) and muscle twitching/tremors, which constitutes a therapeutic error.
In Cold Damage disease, if the pulse is floating and slow, the body feels heavy but not painful, and the heaviness occasionally alleviates, provided there are no Shaoyin syndrome signs, Da Qing Long Tang is indicated.
After sweating therapy, Guizhi Tang (Cinnamon Twig Decoction) should not be used again. If sweating and wheezing occur without significant surface heat, Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction) may be administered.
After purgative therapy, Guizhi Tang should not be used again. If sweating and wheezing occur without significant surface heat, Mahuang Xingren Gancao Shigao Tang may be administered.
Commentary: The aforementioned syndromes, whether presenting with slight exterior cold and abundant interior heat, or anhidrosis with irritability, or diaphoresis with panting, all belong to the category where exterior cold remains unresolved while interior heat has arisen. Thus, they are collectively categorized as Exterior-Cold and Interior-Heat Syndrome.
Guizhi Er Yuebi Yi Tang (Cinnamon Twig Twice-Yuebi One Decoction)
Guizhi (Cinnamomi Ramulus, 1.7 qian) Shaoyao (Paeoniae Radix Alba, 1.7 qian) Mahuang (Ephedrae Herba, 1.7 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 1.7 qian)
Dazao (Ziziphi Jujubae Fructus, 2 pieces) Shengjiang (Zingiberis Rhizoma Recens, 1.7 qian) Shigao (Gypsi Fibrosus, 2 qian). Boil Mahuang first, remove foam, then add other herbs and decoct together. Take warm. Commentary: Since there is cold on the exterior and heat on the interior, Mahuang, Guizhi, and Shengjiang are used to disperse exterior constraint; Shigao clears interior heat; Shaoyao clears Ying-level heat; Gancao and Dazao tonify Spleen essence.
Da Qing Long Tang (Major Blue Green Dragon Decoction)
Mahuang (Ephedrae Herba, 6 qian) Guizhi (Cinnamomi Ramulus, 2 qian) Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian, honey-fried) Xingren (Armeniacae Semen Amarum, 13 pieces) Shigao (Gypsi Fibrosus, 4 qian) Shengjiang (Zingiberis Rhizoma Recens, 3 qian) Dazao (Ziziphi Jujubae Fructus, 4 pieces). Boil Mahuang first, remove foam, then add other herbs and decoct. Take warm. Aim for a slight sweat. If sweating is excessive, apply Wenfen (Warm Powder) externally. Wenfen consists of Baizhu (Atractylodis Macrocephalae Rhizoma), Bencao (Ligustici Chuanxiong Rhizoma), Chuanxiong (Chuanxiong Rhizoma), and Baizhi (Angelicae Dahuricae Radix) ground into powder, mixed with rice flour for external application. Commentary: Since Wei-Qi (Defensive Qi) is obstructed and Ying-Qi (Nutritive Qi) is constrained generating internal heat, Gancao and Dazao tonify Spleen essence; Shengjiang and Xingren descend Lung Qi; Mahuang and Guizhi vent the constraint of Ying and Wei; Shigao clears internal heat causing irritability.
Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction)
Mahuang (Ephedrae Herba, 4 qian) Xingren (Armeniacae Semen Amarum, 16 pieces) Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian) Shigao (Gypsi Fibrosus, 8 qian). Boil Mahuang first, remove foam, then add other herbs and decoct. Take warm. Commentary: After sweating, exterior cold remains unresolved, leading to Lung Qi constraint and heat steaming the skin and hair. Therefore, Mahuang releases the exterior, Xingren descends rebellious Qi, Shigao clears Lung Metal heat, and Gancao supports Earth (Spleen/Stomach), thereby resolving both exterior and interior conditions.
