Treatise
Li Dongyuan stated: According to the *Treatise on Yin and Yang Corresponding to Phenomena* (*Yin Yang Ying Xiang Lun*), "All meridians belong to the eyes." It also states, "The eyes can see when they receive blood nourishment." The essence of the five zang organs and six fu organs all ascend and pour into the eyes to form the eyeballs. The cavity where the eyeball resides is called the "eye" (Yan). The essence of bone manifests as the pupil; the essence of sinew manifests as the black eye (iris); the essence of blood manifests as the general network within the eyelids; the essence of qi manifests as the white eye (sclera); and the essence of muscle forms the constraints that bind the eyelids. These structures envelop the essences of sinew, bone, blood, and qi, merging with the meridians to form the optic nerve system, which ascends to connect with the brain and emerges posteriorly from the neck. Therefore, the pupil and the black eye follow the principles of Yin, while the white eye and red vessels follow the principles of Yang. Thus, the harmony of Yin and Yang results in clear vision. Consequently, the qi and blood of the five zang organs, six fu organs, twelve regular meridians, and three hundred and sixty-five collaterals are all derived from Spleen Earth and ascend through to the eyes to produce clarity of vision. Hence, the eyes are the emissaries of the Heart, and the Heart is the residence of Shen (Spirit). If spirit becomes disordered and cannot remain steadfast, sudden perception of extraordinary phenomena may occur. If pathogenic factors invade the eyes, Jing (Essence) will scatter; once Jing scatters, diplopia occurs, where one object is seen as two. Due to mental distress, irregular diet, and excessive labor, the Spleen and Stomach become weak, and Heart Fire flares up vigorously. This causes the blood vessels to boil and blood flow to reverse. As the classics state, "If Heaven is not bright, the sun and moon lose their light, and pathogenic factors harm the orifices." This refers to the situation described. The Spleen is the head of all Yin organs, and the eyes are the congregation of blood vessels. Therefore, when the Spleen is deficient, the essence of the five zang organs loses its direction and cannot restore clarity to the eyes. The Heart belongs to Ministerial Fire (Jun Huo), primarily responsible for storing Shen, and should remain quiet and peaceful. Ministerial Fire (Xiang Huo) acts on its behalf; this fire is located in the Pericardium and governs the nourishment of all blood vessels by the eyes. Since labor and reckless activity, combined with pathogenic invasion, damage the blood vessels, various diseases arise. Those treating eye diseases who do not first regulate the Spleen and Stomach and nourish blood to calm the Spirit, but instead treat only symptoms without addressing the root, fail to understand the primary principles of treatment. Students of medicine must pay close attention to this.
Pulse Diagnosis
A surging and rapid pulse at the left Cun position indicates blazing Heart Fire. A wiry and surging pulse at the Guan position indicates flourishing Liver Fire.
Wiry and surging pulses at both the right Cun and Guan positions indicate Liver Wood carrying the momentum of Ministerial Fire, insulting the Metal it cannot overcome (Lung) and controlling the Earth it overcomes (Spleen).
Methods
Zhu Danxi stated: Eye diseases belong to Wind-Heat, Blood Deficiency, Mental Fatigue, and Kidney Deficiency.
Liu Wansu stated: When the disease is in the Fu organs, it is a Exterior syndrome, requiring the expulsion of Wind and dispersion of Heat. When the disease is in the Zang organs, it is an Interior syndrome, requiring nourishment of Blood and calming of the Spirit. For example, sudden blindness, dullness and dryness with nebulae, excessive discharge and tearing, all indicate Wind-Heat. Another view holds that spots entering the eye indicate flourishing Liver Qi manifesting in the Exterior, suitable for dispersing methods to remove them. If the eyes are dull and weak, unwilling to look at objects, with internal cataracts showing black flies and dilated pupils, these are Interior syndromes indicating Blood Deficiency, Mental Fatigue, and Kidney Deficiency, suitable for nourishing Blood, replenishing Water, and anchoring the Kidney for regulation.
Dilated pupils are caused by acrid and hot substances. Acridity promotes dispersion, and Heat exacerbates it. One should expel Wind-Heat, cool the Blood, and benefit the Blood to收 (gather) the scattered Qi. Huang Qin (Scutellaria Root) and Lian Lian (Coptis Rhizome) are bitter and cold, serving as the Sovereign drug to eliminate rampant pathogenic factors. Dang Gui Shen (Angelica Sinensis Root Body) and Sheng Di Huang (Rehmannia Root) nourish and cool the Blood, serving as the Minister drugs. Wu Wei Zi (Schisandra Fruit) is sour and warm with a floating nature, capable of contracting dilated pupils. Di Gu Pi (Lycium Bark) and Tian Men Dong (Asparagus Tuber) drain Heat and benefit Qi. Alternatively, Zi Yin Di Huang Wan (Nourishing Yin Rehmannia Pill) is most excellent.
For chronic dullness and dimness, Shu Di Huang (Prepared Rehmannia Root) and Dang Gui Gen (Angelica Sinensis Root Base) serve as Sovereign drugs, assisted by Qiang Huo (Notopterygium Rhizome), Fang Feng (Saposhnikovia Root), and Gan Ju Hua (Chrysanthemum Flower).
For acute onset of redness and swelling, Huang Qin (Scutellaria Root) and Fang Feng (Saposhnikovia Root) serve as Sovereign drugs to drain Fire; Lian Lian (Coptis Rhizome) and Dang Gui Shen (Angelica Sinensis Root Body) serve as Minister drugs to nourish Blood. Qiang Huo (Notopterygium Rhizome), Chai Hu (Bupleurum Root), Sheng Ma (Cimicifuga Rhizome), Bai Zhi (Angelica Dahurica Root), and Gan Cao (Licorice Root) serve as Envoys. Add a small amount of Bai Dou Kou (Black Cardamom Seed) if the sclera is red.
Another prescription: Treat pain due to Blood Heat stagnation using Si Wu Tang (Four Substances Decoction) plus Cao Long Dan (Gentiana Scabra Root), Fang Ji (Stephania Root), Fang Feng (Saposhnikovia Root), and Qiang Huo (Notopterygium Rhizome). Another view: For severe Heat rushing upward causing eye pain, use Lian Lian (Coptis Rhizome) to drain Fire and Dang Gui (Angelica Sinensis) to tonify Blood.
For internal cataracts and dimness due to labor and irregular diet, use Man Jing Zi Tang (Vitex Decoction).
Man Jing Zi (Vitex Fruit), Ren Shen (Ginseng Root), Gan Cao (Licorice Root, honey-fried), Huang Qi (Astragalus Root), Huang Bo (Phellodendron Bark), Bai Shao Yao (White Peony Root, wine-fried), each 1 qian.
Finely chop, decoct in water, and take orally.
Another prescription: Treat eye pain. Soak Sheng Di Huang (Rehmannia Root) in wine, crush, and apply to the eyes. Also, make a powder from Cao Wu (Aconitum Carmichaelii Lateral Root), Nan Xing (Pinellia Tuber), Gan Jiang (Dried Ginger), and Gui Zhi (Cinnamon Twig), mix with vinegar, and apply to the soles of both feet. Frequently wash the eyes with Niu Xi Gao (Achyranthes Ointment).
For obese individuals with Wind-Heat congestion ascending, causing eye pain:
Fang Feng (Saposhnikovia Root), Qiang Huo (Notopterygium Rhizome), Jing Jie (Schizonepeta Spike), Jiu Qin (Scutellaria Root fried with wine), each 2 qian.
Decoct in water and take orally.
For thin individuals with eye pain, this is due to Blood Deficiency combined with Heat. One must use Blood-nourishing herbs, adding a small amount of Wind-dispersing herbs.
Dang Gui (Angelica Sinensis), Sheng Di Huang (Rehmannia Root, washed with wine), Xuan Shen (Scrophularia Root), Fang Feng (Saposhnikovia Root), Jing Jie (Schizonepeta Spike), Ju Hua (Chrysanthemum Flower), Chuan Xiong (Ligusticum Rhizome), equal parts.
Finely chop, decoct in water, and take orally.

(Danxi Huo Tao) states: Li Dongyuan believed that the ability to see far but not near is due to Excess Fire and Deficient Water; the method is to tonify the Kidney, using Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill). The ability to see near but not far is due to having Water but lacking Fire; the method is to tonify the Heart, using Ding Zhi Wan (Stabilize Spirit Pill) plus Fu Ling (Poria). He also said: For inability to see near, take Di Huang Wan (Rehmannia Pill) in the morning; for inability to see far, take Ding Zhi Wan (Stabilize Spirit Pill) before bed. These all communicate with the Hand and Foot Shaoyin meridians. Thus, inability to see near is due to Kidney Water deficiency; inability to see far is due to Heart Blood insufficiency. For acute onset of eye redness and swelling, suitable herbs include Qiang Huo (Notopterygium Rhizome), Fang Feng (Saposhnikovia Root), Chai Hu (Bupleurum Root), Bai Zhi (Angelica Dahurica Root)...
